Changing bearing. Tail pulley slipped and smashed finger and thumb. Index finger was smashed, lost fingernail, had fractured bone and 4 stitches.

Employee was using a cutting torch cutting bolts off of screw, the nut fell off and burned through employees pant leg and in to his boot, burning his foot.
