EE LOST CONTROL OF THE TUBE (WHICH WAS STANDING ON THE FLOOR). THE TUBE TOPPLED OVER AND STRUCK EE ON THE FOOT. NOT THE RESULT OF REG. NONCOMPLIANCE, NOT TRAINING/SKILL RELATED...
