Mostrando los 16EE was cleaning out the hopper. EE slipped on the belt, causing EE to fall on a corner that was steel. This cut EE's shin. Thought nothing of it at the time, cleaned it, antibiotic ointment, and thought that would be all. 5 days later EE's leg began to hurt, EE went to the doctor, & found out the leg had been infected. This led to further treatment which involved time off.
EE was walking from the backhoe to the loader and slipped and fell on the ice.
Employee unhooked the chain from the conveyor and dropped it over the end. EE was walking back down and the chain accidentally re-hooked on a beam and jerked the conveyor and that is when EE lost balance and fell to the ground on EE's back.
EE was taking large rocks out of the cone crusher and setting them on the ledge so they can throw them out and one of the rocks rolled off the ledge and came in contact with EE's 3rd/Ring Finger on the right hand.
EE was assisting to tear down the screen box. EE got thumb caught between the deflector plate and the screen box.
EE was working on the jaw of the crusher and was holding a piece of it while coworker was fastening bolts when EE's leg slipped out from underneath EE and EE then fell on right leg and knee. They did have the system on Lock out tag out.
Employee was rolling a conveyor belt to the back of the pickup and ee was unable to stop it when it got to the back of the pickup. The belt rolled to the ground and hitting the employee causing ee to fall to the ground and the belt then roll over right leg.
EE was doing the morning inspection and while getting into the loader, the door closed before EE got hand in and the finger got caught between the door frame and the loader.
Replacing roller on conveyor and was tapping clip out of roller when something fell in right eye. EE had safety glasses on but they were fogged up so EE moved them down just a little to let the moisture out of the lenses.
Employee exited the loader without placing hard hat on his head and was removing a stuck rock from the feed conveyor when rock fell and hit employee in the head. A hard hat was provided to the employee and he had it in the loader that he just exited out of to remove the rock.
Holding tail pulley leg on conveyor trying to reinsert pin into leg and the weight of the leg came down and pinched finger between leg & frame of the conveyor. He smashed the tip of his right index finger just before the first knuckle.
Was removing rock from feeder and rock fell off lid and hit him on his forehead causing the laceration.
Was wearing safety glasses and was hand grinding when a foreign object got into his eye.
Rain and strong winds. Employee was preparing to move plant cleaning cut receiving hopper. He slipped and fell into hopper hurting shoulder.
Crew was moving conveyors & testing alignment, when ee saw a build-up of material in hopper of 1 of the conveyors. Ee the climbed on to conveyor & pushed the built-up material with his foot-large chunk of material slid in hopper & ran through hopper where ee's hand was braced causing his finger to be pinched between angle iron & chunk of material
