While making repairs to the coal exhaust fan, employee was rolling a belt manually. Employee's right ring finger and pinky were caught between the belt and pulley resulting in fractures and sutures.

Employee was using a jigsaw to cut siding on #11 conveyor in area. While cutting Employee was taken to first aid where the eye was flushed again. Employee was then taken to local eye care center where debris was removed and antibiotic eye drops prescribed. Employee was wearing required PPE at the time of incident.

Employee had pain in right knee after dismounting bulldozer #2572. Employee reported to on-site clinic for examination. Employee returned the following day for follow-up visit and was referred to off-site clinic for treatment. Employee was placed on modified work.

While operating bulldozer employee noticed pain on the left side of their neck. The stiffness progressed overnight.

EE was standing on top step of lift pump, stepped backwards, foot slipped EE fell on right buttocks. No complaints, over time had discomfort, on 9/5 EE was seen in clinic & later seen by co. dr. EE had prior back problem, referred to specialist no restrictions that would limit duties. F/up visit w/specialist on 1/26/18 condition had not improved, EE was placed on modified duty.

The EE was operating the Hemtt fuel truck. It was raining and EE was running behind schedule because of the rain. EE was driving down a steep ramp to fuel a water jack and states that the brakes did not work. Before EE got to the bottom of the ramp EE decided to jump from a moving vehicle. Had EE stayed in the vehicle at the slow rate of speed EE may not of been injured.

Employee was working in the #4 dragline tub changing the tub paper, while crawling in the area EE's legs got wet from standing water, grease and oil.

Employee stated that during a power outage ee was trying to close a gate that normally opens and closes with an electric gate opener. Gate was switched to manual setting, in process of closing the gate the employee strained left shoulder.

Employee alleges that while driving into work on the plant entrance road the vehicle they were driving was struck from behind while sitting stationary causing injuries that required ambulance transport to the hospital. Reason for late filing of 7000-1, Mine Operator was unaware plant entrance road (location of incident)was in the mine jurisdiction.

Employee was removing & replacing the bucket teeth on an excavator. Employee cut them with the torch and was using a hammer to break them free when a piece of metal struck the employee in the leg resulting an ER visit to remove the foreign object. Released to work the following day.

Employee was using cutting torch to cut a hole in a piece of steel plate when some slag blew back striking side shield and running into left eye.

Employee was descending stairs and stepped on short step and left ankle rolled causing employee to fall to knees. Employee was placed on restricted duty on 2/17/2017.

Employee was climbing into back of truck to retrieve shovel and fell backwards onto the ground landing on employee's left elbow. Employee did not have pain initially but soreness increased, continued to perform regular job. The need for rotator cuff repair was diagnosed and employee had left shoulder surgery on 4/27/17.

While dumping #16-1 Screw Conveyor, an Operator was approximately 10' from the conveyor while a front end loader operator was removing hot rock. As the front end loader approached, a flash occurred which caused hot product to get on both of employee's legs. Employee was taken to first aid where burns were treated, then employee was taken to a local medical facility.

Hearing illness left ear

Right hand discomfort. Employee states ee was operating a manual valve on tails line & felt pain in right hand. EE later adjusted packing on pump & felt pain again. Employee was diagnosed with carpal tunnel syndrome on 12/7/16 and placed on modified duty.

Employee was tightening a band around a 36" pipe on the recycle lake line at the west side of Phos Acid. Employee's hand slipped and contacted the edge of the band. The contact resulted in a cut on right hand that required four sutures to close. Employee was wearing gloves at the time of the incident.

Employee stated they were opening a window in the office at SRM Mobile Shop and while lifting the window they felt a slight pain in the right shoulder. Employee was seen by an off-site physician on 10/10/17 and was placed on modified work.

Employee stepped off of a step on the side of a flatbed truck and twisted right ankle. Sprain with a hairline fracture

Employee was pulling boards at #28 spillway when employee noticed some discomfort in employee's left middle finger, employee had ongoing problem and surgery was performed on 6/20/17 for trigger finger due to repetitive work.

Employee was changing drill bit on a press drill. Employee failed to engage lock fully and as EE removed bit the drift pin fell. Employees left thumb was struck. Thumb bone was fractured.

Employee was washing up under raw bins and some liquid splashed onto legs. The next night EE noticed redness to both legs. Employee reported to the clinic on 6/30/16 to report skin irritation to both legs.

Working with float gang, employee was holding a spud on the head side of the bolt while attempting to replace a 45 degree fitting at #2 lift on the short north tailings line. Employee's finger tip was caught when the spud turned causing an avulsion to left middle finger.

Employee was attempting to trim a plastic tab on hard hat halo attachment using a pocket knife when the knife slipped and caused a laceration to left thumb below the first joint.

Employee was replacing dump rope on the 2550 dragline. When repositioning the dump socket to enable safe removal of the wedge, the socket shifted, pinching employee's right index and middle fingers between the dump socket and drag chain. Employee was wearing gloves at the time of the incident.

Employee diagnosed with right carpal tunnel syndrome.

Employee was standing near 10-wheeler boom truck about 12-15" from hoist cables at #2 D/L. Two other employees were using a 8 lb. sledge hammer and RR maul to drive wedge out of hoist socket. A piece of metal sheared from the maul and traveled approx. 12-15" striking employee in back of right calf.

Employee reports that for a month has been having bilateral shoulder pain. Employee states that the discomfort is worse when about to get off shift, employee operates a pit car. When employee sleeps and wakes right arm feels numb. No specific injury.

Employee was preparing to put lock on lock box, he went to turn and walk off and felt a twist in his right knee. Employee returned to company clinic on 4/21/16 with complaint of right knee pain from 2015 incident. Employee being referred for evaluation.

Miner was operating a skidder when the blade on the front of the skidder abruptly fell without warning causing the skidder to stop suddenly. The miner's face, body and left leg contacted the steering wheel and steering column causing his mouth to clench down forcefully causing several chipped teeth, a laceration to the chin requiring 4 sutures and severe bruising to his left leg.

An employee was performing hands free hydro blasting on the #3 cold cyclone, the water contacted hot product when the blockage broke through and caused steam to blow back up the pipe. The employee operating the unit stepped off the peddle, and then tried to quickly back away. He stumbled over the foot pedal and fell.

Float Crew was changing out a 7' long 20" exp. joint on matrix pump. Bolt holes were not lining up on flanges. Float Crew Leaderman(FCL) used a lifting strap as choker around exp. joint. FCL tightened down on strap & signaled operator to hoist up. As operator started hoisting, the strap broke. FCL left hand was still on strap causing it to suddenly shift.

Miner was using a 2.5 gallon urea (DEF fluid) container with a flexible funnel to fill a DEF fluid tank. While lowering the jug after filling, miner stated a few drops of DEF fluid entered his eye causing a minor chemical burn.

A mine electrician sustained burns to his arm when a live 7,200 volt power cable arc flashed at a junction box. Employee was taken to first aid for initial treatment and then to a local medical facility for evaluation and treatment. Employee was then transported to regional burn unit for treatment of his burns, and for further testing.

Employee was walking at Mill flotation. He stepped in an unprotected trench. Grating that covered the trench had been removed, water was covering the area due to a pump failure at the time. Received an abrasion to his right shin and reported that his hip was sore. He returned to work at that time. Became a recordable on 6/23/15 due to the employee having physical therapy.

Frozen ground conditions

Employee was in the transfer station assisting with trying to get the #39 conveyor back running (frozen). He was walking back towards the head pulley area. As he passed under the 17-3 flop gate platform, he was concentrating on his walking/working surfaces and bumped his head on the 17-3 flop gate platform resulting in a laceration to his forehead.

Employee was cleaning up around 1-4 mesh reject pump. He stepped in soft reject material and rolled his right ankle.

Employee was driving a 3/4 ton pickup down ramp. Contract company was moving 16" pipe across bottom of ramp. Employee's truck hit the pipe causing pain in his right arm/shoulder. Was seen at orthopedics and prescribed physical therapy.

While performing dragline repair work, employee's fingers were pinched between a pin component and flange while the pin component was being moved into position. Resulted in lacerations on two fingers requiring sutures.

Employee was installing transmitters at the #5 tailings circuit booster when he became overheated and began to feel ill.

Employee operates a large dragline that utilizes a joystick for control and began noticing pain and numbness in his hand. Referred to a doctor 10/7/14 diagnosis is rupture tendon left hand.

Employee had been working on a different dragline. He was using joystick in cab of dragline and having intermittent numbness in tips of fingers. Employee returned to company clinic on 4/21/16 with complaint of CTS symptoms of right hand pain (notification in July 2014). Employee being referred for evaluation.

Employee was adjusting the air flow on the green float bank B-Cell when the middle finger on his right hand got caught between a moving paddle wheel and the guide frame for the tee handle used to adjust the air.

Employee was operating a CAT 740B haul truck in block 8 cut and drove thru a dip in the road while staging up for loading.

Employee was walking carrying bucket & felt sharp pain in right shoulder. He set bucket down and had difficulty picking it back up due to pain. MRI on 06-06-14 revealed torn rotator cuff.

Operator exited the cab of the crane and walked across the deck toward the ladder to climb down. Just as he approached the ladder, he slipped and fell to the ground landing on his feet and then buttocks. Employee was taken to hospital with complaints of pain in his right ankle/foot. Xrays revealed a fracture a small heel bone that required a cast to heal.

A crane was being used to lift a load and the employees right fore finger was pinched between the load and the base. Employee was taken to the local medical facility where the wound was cleaned, and he received 3 stitches. He was also sent to the local hospital for x-rays and this showed no bone fracture. No prescription medications were given.

Operations was in the process of relocating a de-energized power cable. Employees were positioned as spotters as a backhoe was used to pull the cable. One employee was on the opposed side of the cable from the others. As cable was pulled it became hung up on a berm. As employees signaled for the backhoe to stop the cable broke through the berm and struck employee at the knee.

The employee allegedly walked off of a stair well and fell approx 3 ft to the ground fracturing his elbow. The injury was reported on day after the alleged incident and there were no witnesses to the fall. The validity of the claim is questionable.
