Employee was operating a skid-steer in the wet plant and struck a concrete curb. This resulted in the employee striking the lap bar of the skid-steer in the abdominal area, and striking their left knee on the interior cab. The employee was wearing a seat belt at the time of the incident.

Mostrando los 10On 3/23 employee was working as the ground person at our safe rack, EE was walking to a truck, and bumped EE's knee on a conduit that goes to our kiosk. Upon stepping back EE stated that EE took a mis-step and felt pain in EE's knee. Employee did not report this incident, we noticed EE limping and questioned EE as what had happened.
Employee was in route to clear an obstruction in a surge bin; while ascending a vertical ladder the employee lost their gripping and fell to the platform below. This fall was approximately 9 feet, and employee landed on right knee which caused a laceration to that knee. Said ladder is in good condition, with a safety gate at top landing, and has a proper back cage installed.
Employee approached me after the pre-shift safety meeting stating that employee's hip had been bothering employee for approximately one month. Employee said that it seems to be more aggravated when employee is running the 320E excavator. Employee was sent to seek medical treatment and was placed on restricted duty.
Employee was hooking a wash hose, to wash out the tunnel. Upon turning on the water supply valve only a little water came out and then it stopped. While employee was reaching to turn off the water and the hose blew out below the valve, resulting in skin abrasion and local swelling to the right wrist. The hose that was being used was a 2 inch lay flat PVC hose.
Wind blew sand that went under employees' safety glasses and in left eye.
Employee was assisting the maintenance crew change bags on the bag house and it started to rain so they set the cover to the access hole back on top of the hole to wait out the rain, after doing so employee stepped on cover which was not secured. Upon doing so the cover tipped upward resulting in the employee's right foot falling into the access hole.
Employee was moving floc bag and stepped on wood rolling ankle
EE was helping with rotex screens and lost balance and strained back.
An employee was standing next to a load of 3 26'x 24"x6" I-Beams while a forklift was trying to reposition them away from the stack., one beam started to fall and the employee reached out to stop it, pinching finger between the beams. The injury caused a fracture and laceration to the tip of left middle finger.
