Two miners were moving a portable crusher with a remote control. The track on the crusher contacted a rock, ejecting it and forcefully hitting two miners. The miners were taken to the hospital to have the rocks removed.

Miner was helping move a piece of equipment when miner blacked out due to a pre-existing blood pressure problem (that employer did not know about) and fell on a trailer that was parked behind miner. Hitting head and back. Miner refused to go to the hospital at the time of the accident.
