Employee was using a drill steel to beat on a retainer pin on the battery scoop. When the retaining pin loosed up, the drill steel hit the employee in the right ear resulting in a small laceration which required stitches.

As EE attempted to raise the mast, EE inadvertently activated the clamps and pinched EE's left thumb and index finger resulting in lacerations requiring stitches and a fractured bone in EE's index finger.
Employee was hanging 350 MCM cable to the mine roof in the 7th North set-up. A Wagner 25X was being used to lift the cable. The employee inadvertently placed EE's right hand on top of a pivot point resulting in EE's fingers being smashed. Laceration requiring stitches.
While operating the tailgate drum on the shear at shield #30 on the 8th South Longwall, The employee was struck in the back of the head by a lump of coal that had flown over the raise sloughing plate on the shear. Employee received a laceration which required medical treatment (Staples).
While mining in the 8th South longwall panel #10, there was a bounce and outburst of coal at the tailgate at shield #136, which resulted in the mining equipment being down for more than 1 hour. One person was injured and received medial treatment (stitches).
As the employee was operating the Wagner 25X, #9, the door on the operators compartment came open. The employee stopped the machine and reached out to the top of the door and slammed it shut. As the door shut it smashed EE's right hand between the door and the cab of the machine. Stitches were required.
Employee was standing on top of a load of cattle guards attempting to hook up a chain to unload the cattle guards from a trailer when EE's foot slipped on a cattle guard and EE strained left shoulder as EE grabbed ahold of the wire roof mesh to regain EE's balance.
Employee was standing on a ladder to change out bread crumb in the 4th East Mains section. The ladder slipped on the mine floor and the employee fell to the ground and hit right elbow.
Employee was hit in the face shield of helmet while operating the headgate cutter drum at shield #104 on the 8th South longwall face while employee was checking the flow of coal in the face conveyor.
Employee smashed the tip of left middle finger between a bottom roller and a spreader bar while loading the structure onto a belt trailer in the 6th North belt line. X-ray indicated a fracture to the bone in the tip of the left middle finger.
Employee collapsed at crosscut #1 in the rock slope in the secondary escapeway (beltline) while conducting the required secondary escapeway drill. Others walking with employee unsuccessfully attempted CPR.
Employee was attempting to loosen a 1" water line fitting from a spray block on the continuous miner, company #11-1, located in the 5th North Development Section. While applying pressure with a wrench, employee felt a sharp pain in left bicep.
Employee was gathering up surveying equipment and placing along the rib in the 7th North Development section. Moved surveying instrument and placed it along the rib when a piece of coal fell from the rib and struck the backside of EE's left hand requiring stitches.
Employee was carrying a piece of belt structure (belt rail) in the 5th North Development Section, when EE stepped on a lump of coal and twisted left ankle.
Employee was carrying a ladder down the stairs of the overcast on #7A beltline located at crosscut 46 in the 3rd East Mains, when employee lost balance on the 3rd from the bottom step and sprained left ankle.
Employee was standing on the roof bolter attempting to put a metal roof mat on top of the canopy and as EE stepped from one side of the bolter to the other side, EE lost footing and fell and hit head on the ATRS scissor hinge area, resulting in a laceration which required 4 stitches to EE's right eye brow.
While lifting a 50 pound bag of rock dust to fill the pod duster in the 6th North Development section, the employee strained EE's lower back as EE was tossing the bags of rock dust onto the pod duster.
The injured employee had just finished making repairs to a valve bracket on shield #8 on the 8th South longwall face. The shear was located at shield #40 cutting a taper towards the head gate. There was a bounce on the face. The injured employee was knocked down and hit EE's head on the valve bracket. 3 stitches were required.
An employee was re-aligning the #4 belt drive. The employee was using a hammer to beat a wedge between the belt drive and the metal stand for the drive. Employee missed the wedge and hit the hook of a come-a-long and the hook flipped up and hit employee in the chin. 2 stitches were required.
Employee was holding a timber in place to be set along the rib. Another employee was using a pry bar to pry the wire rib mess tight against the rib so that the timber could be set as close as possible to the rib. The pry bar slipped and smashed the injured employees left pinky finger against the timber. 6 stitches were required to close the wound.
Employee was building a Kennedy Panel regulator in the 6th North return. As EE was attempting to place a Kennedy flag on the upper corner of the regulator, the panel slipped down and cut EE's left index finger. The laceration was glued and employee was given a prescription by the doctor.
Employee was using a come-a-long hooked to a rib pin to lift the tongue of the trash trailer. When the tongue popped off of the hitch, the tongue of the trailer swung towards them and pinned left leg between the tongue of the trailer and the rear bumper of the #2 Getman.
Employee was installing a 1 1/2 inch fire drop onto the 6" waterline in the belt entry at the 1st North tailpiece. The 5 foot long by 1 1/2 inch pipe fell and stuck the employee in the lower right leg and right ankle resulting in a fracture to the right leg/ankle.
Employee was attempting to advance the ventilation tubing in the #4 entry of the 1st North Development Section. As EE and another employee lifted the fiberglass vent tube, EE felt a sharp pain in lower back.
Employee was roof bolting in the 1st North Rock Slope, employee was putting a 5 foot pusher steel into the 3 foot extension steel and as employee was holding the chuck wrench while pushing the steels into the hole, employee smashed right little finger between the chuck wrench and the clamps on the drill head of the roof bolter.
Employee was cutting a metal band off of a bundle of roof screens and when the band broke, one end of the band flipped up and hit EE's right arm resulting in a small laceration which required 3 stitches.
Employee was using a 12 inch pipe wrench at cross cut 13 in 7th South to remove a 2 inch pipe nipple from a fire outlet when the pipe wrench slipped and the employee strained their lower back while trying to regain balance.
Employee was transferring a bantam rock duster from the bucket of a scoop into the bucket of another scoop when the employee's grip slipped and left thumb was cut by a wire that was sticking out of the rock dust hose that was attached to the duster. Employee receives 4 stitches.
Employee was building a cinder block wall for an overcast, as they opened the mandoor to travel thru the door, the wall tipped over and the employee twisted their right ankle in the mandoor frame.
Employee was hang the miner cable along the rib in the last open crosscut in the 1st North development section when a piece of coal measuring 18 inches long by 18 inches wide by 8 inches thick fell from the rib and struck ee in the left knee.
Employee was changing a bad tire on the forklift located outside of the surface shop. EE was using a socket and a 1" breaker bar with a 3' cheated bar to break the lug nuts loose when the cheater bar slipped and the employee smashed their right pinky finger between the breaker bar and the steel wheel resulting in a laceration which required 3 stitches
Employee was walking down the longwall face from the headgate to the tailgate, foot slipped on the wet surface of the mine floor and as employee fell, struck left knee on the toe of shield #80. 5 stitches were required to close the laceration on left knee. EE was released to return to work but had a followup visit on 2/25/19 and is currently off work pending an MRI.
Employee was walking down the slope along #4 belt conducting the pre-shift examination when EE's foot slipped and EE fell to the ground. EE reported the incident. EE went to the doctor on 2/27/19 and had a MRI on 3/6/19. EE had a followup visit on 3/14/19 and was told EE had torn meniscus in the right knee. Lost time injury as of 3/14/19.
The employee was troubleshooting an electrical problem on the continuous miner in 1st North Mains. EE had opened up a hinged protective cover. Another employee inadvertently bumped the hinged cover and it fell smashing the injured employees right middle finger resulting in a laceration which required 11 stitches.
The injured contract employee was assisting to extract shield #89 from the extraction face when a rock measuring 21" long by 11" wide by 2" thick fell from in front of the walker shield and struck EE on the left foot.
The employee was dropping a vent tube from the mine roof when the nylon strap that was supporting the tube came loose and the employee caught Left small finger in the D-Ring of the strap and cut left little finger. 8 stitched were required.
Employee was installing a new haulage tram motor on the shear, as the employee moved a hydraulic hose out of the way, the motor shifted and smashed the employee's right index finger between the motor and the frame of the machine. 7 stitches were required to close the wound.
A contract miner was standing on the 3rd rung of a 6' ladder, as the miner leaned to one side and attempted to hang a belt chain from a roof bolt, the ladder tipped over and the miner hit head on the ladder as miner fell to the ground. 3 staples were required to close the wound. The miner was released to return to work on next shift.
Employee had finished drill a 16 foot deep hole and lowered the drill mast on the roof bolter. The sectional steel got stuck in the hole and as EE reached for the starter steel, the section steel fell from the hole and hit EE's right hand resulting in stitches.
Employee was a passenger in the passenger seat in an Isuzu pickup truck heading out of the mine. When the operator applied the brakes, the brakes failed and the pickup hit into the back of a parked pickup truck at the air doors at XC#1 on the rock slope.
Employee was walking down a set of wooden stairs at the overcast @ XC 29 in 5th South when the second from the top plank gave away and employee fell to the ground injuring left knee, right shoulder and neck.
Employee was operating an Isuzu truck at XC 27 in 3rd East Mains when the alternator starting squealing. The operator stopped the truck and got out of the truck. As employee was walking towards the front of the truck to open the hood, employee's foot slipped on the wet roadway and employee strained lower back.
While loading a vent tube into the bucket of the 3 1/52 scoop, the employee smashed left index finger between the tube and the bucket of the scoop, causing a laceration which required 11 stitches.
Employee was dragging a piece of Tensar mesh out of the face when left hand snagged an insulated cable hook. The hook punctured through the skin between index finger and thumb on left hand. Employee receive a tetanus shot and a prescription for an antibiotic and was released to return to work with no restrictions.
Employee was standing in the walkway at shield #10 and as the pan was being advanced, an anti-slew jack got hung up on the bears ear bracket and when it popped free the slew jack stuck the employee in the right knee. EE went back to the Dr. on 4/9/2018 and was given a work release for modified restrictions.
Employee was operating the tailgate drum of the shear at mid face grading bottom while cutting towards the headgate. The employee placed left hand on top of the spill tray and a rock was kicked up by the cutter drum and landed on top of employee's left hand causing a laceration which required stitches.
While operating the left side drill head of the RB-2 roof bolter, the starter steel became stuck in the mine roof. The operator lowered the drill box down 1/2 of the way and jerked the stuck steel out of the hole. The operator's small finger on left hand was pinched between the drill steel and the drill head of the machine. 3 stitches were required to close the wound.
Employee was walking past the loop line between the monorail and stageloader when the wash down hose that was lying on top of the loop line fell to the ground and landed on the handle of the ball valve, thus pressurizing the water hose which flipped around and hit the EE in the chin. Required 4 stitches.
A contract miner employee was injured at shield #41 on the longwall face when EE got right foot smashed with a slew jack as the face pan was being pushed forward suffering a contusion to the top of right foot.
The shear was cutting up hill at the headgate into the web for the push over. Employee was standing behind the shear at shield #20. The sloughing plate on the shear was in the raised position. Coal and rock was expelled from the face/roof onto the shear and bounced over the sloughing plate and hit employee in the right shoulder and air stream helmet.
