Miner first reported injury while on days off 11/01/15 by text message to Plant Manager. Claimed injury, sprain of lumbar spine ligaments, occurred while operating a front end loader at an unspecified date/time the week prior. During a return to work physical examination on 11/04/15, Miner described his injury had occurred while swinging a sledgehammer on 10/30/15.
