Employee was in the process of removing abandoned discharge line that was stuck in the mud, when EE lost balance and fell backwards (same level fall) striking the left side of face on the road causing dirt, sand to enter left eye. Employee taken to local clinic where eye was flushed and RX given for eye ointment for the irritation.

Employee was dismounting an elevated platform using steps and handrails with 3 points of contact. One foot was on the ground, and when the second foot was going from the last step to the ground, the original foot slip out from under ee and caused them to fall.

Employee (office worker) experiences pain in left wrist when typing. Employee was diagnosed on 08/23/2016 with carpal tunnel sydrome.
The employee was on a 992 loader near the front of the track and was making their way back to the platform when the employee slipped and fell to the ground.

Employee felt pain in his lower back after decending from the cab to the track of an excavator. Later, the employee's back became worse. Employee was not able to complete normal work duties for the following two shifts.
Electrician stripping wire in well field felt particle enter his right eye. Employee rubbed his eye to remove the particle. After the end of the work shift, the employee experienced pain and irritation in the eye. The employee was taken to the local optometrist where a prescription was given for eye irritation.
On 10/18/2015 while a shift supervisor was en-route to the PS 500 substation to investigate why the main facility and the draggle had lost power, a fire was noticed under the 25KV power line next to the Turlington haul road south of Browns creek on the west side of the road.
While baling hay, the right rear roller bearing became hot. The heated roller bearing contacted dried hay causing a grass fire. The fire lasted about an hour.
Employee was in the process of removing the engine from 12019 hauler. The employee was using a pocket knife to cut a zip tie from a wiring harness. The employee was cutting in the direction of his left hand and the knife contacted the top first knuckle of the employee's hand causing a laceration
Power Plant security notified mine personnel of a fire located between C ramp and county road 235. Upon arrival to the area, supervision discovered a large brush pile burning under power lines. The local fire department was notified. The fire lasted approximately 1 hour and 20 minutes. The source of ignition is unknown.
A fire was discovered near the Turlington pump yard. The fire began off mine property on a nearby county road and traveled onto the mine site. The fire lasted approximately 33 minutes. The source of ignition is unknown.
Injured had backed his vehicle over a farm T-post. While trying to dislodge the post, his finger was caught in between the T-post and attached 1/8" steel cable.
As employee was depositing a bag of trash into a dumpster, he felt a small strain in his lower back.

Employee was walking across the hauler parking area, lost his footing and slipped in mud, causing a strain to the employees back.
Employee was operating a hauler while descending the #3 Stockpile ramp in route to the crusher. The hauler traveled over uneven terrain causing the seat to bottom out straining the employees lower back.
During a pre-op inspection on a piece of mobile equipment, the wind blew the door closed on the employees hand. The mishap resulted in a fracture and laceration to the tip of the employee's right index finger.
Empl was working in Pit when Shell cracked off face of Highwall and fell causing water & mud to rush over berm. After the incident, empl went to the back of truck got a hammer and began hitting his arm with a hammer. Empl claimed he was struck by flying debris from incident. Co-worker witnessed the Empl using the hammer and placing it in the truck once they were ready to leave.
Employee was on hands and knees assisting in dump rope change out at dragline. Employee stated he was simply guiding rope through socket and stood up, turned to walk away, and felt pain in his lower left short rib area. On 05/23/2014 the injury became recordable due to restrictions.
Employee was dismounting truck when grab iron broke. Employee fell backwards onto the ground. He injured his left shoulder, arm, and hip. Employee continued to work without restrictions or medical treatment. On 09-29-2014 he had rotator cuff surgery on his shoulder.

Employee alerted his supervisor because he was experiencing heart attack like symptoms. Employee stated he was feeling soreness across his chest and both arms and was transported to the nearest ER. Employee also stated that he had felt the same discomfort earlier that morning before work and the night before.
EE experienced a short fainting while driving a pickup truck to headquarters at days end. This caused him to leave the roadway, and into the ditch next to the haul road. There was no equipment damage but paramedic were called and he was transferred to hospital to be evaluated. It was determined that dehydration following a day or two of flu symptoms was the cause.
Mechanic was installing a hydraulic hose on D10R Cat Dozer and over-exerted his left arm, causing him to feel sudden discomfort in his shoulder. On 11/08/2013 the injury became recordable.
Crossarm failed and allowed electric line to touch the pole causing the pole to catch fire.
While running wire through conduit workers legs gave out and had a sharp pain in lower back area.
Employee reports neck pain when he turned his head to the right while he was operating a hauler. The employee alleges that it ties to an old injury dating back 2 years.
Mechanic was replacing #2 Swing retainer plate on 16001 Bucyrus Erie 1500 Dragline. As the ring was being held in place with a piece of angle iron, the ring shifted to one side and fell, catching the mechanic's left middle finger tip between the plate and the housing. Employee's injury was secured by fellow workers and employee was taken immediately to Emergency Room.
On 1-29-13 employee was operating motor grader when blade hit a rock causing the operator to jerk her back. On 2-26-13 employee begins losing work days causing injury to be reportable.
Worker was cleaning mud and dirt off drill probe when his glove got caught on pin/probe breaking his left arm.

Was walking in wash bay and slipped on mud causing his left knee to twist.
While moving furniture to conference room employee attempted to move a hearing booth by himself with a dolly. The booth was to heavy and fell back on his right leg/knee. Employee was then taken to clinic and determined he had sprained his knee.
At 6:00 pm a supervisor notices a brush pile fire that was getting out of control and at that time the fire became unplanned. The supervisor contacts dozers and water trucks by radio to help extinguish and control the fire. The supervisor then calls Safety and it was determined that MSHA needed to be called because the fire would be lasting beyond 30 minutes.
An employee was exiting his vehicle in the employee parking lot and stumbled on a rock. Later that day the employee noticed some discomfort in his left knee and related it back to the incident that happened earlier that morning in the employee parking lot. Employee continued working until feeling discomfort and then contacted his supervisor the following day.
Employee was climbing the ladder on a 992G loader. As the employee pulled himself up with his right hand, he swung his left hand up, striking his left hand against the ladder handrail, causing a fracture to knuckle.
Employee was sliding seat back on trackhoe and caught his right middle finger between the seat slide adjustment release bar and the seat frame. Employee refused initial medical treatment until the morning of 02/25/2012. Upon treating physicians diagnosis, positive x-ray showing bone chip to right middle finger joint, 02/25/2012 making this reportable.
Employee was getting equipment from a metal toolbox on a truck bed. Second employee tightened a strap securing box, causing lid to unexpectedly close. First employees hand was struck as lid fell shut. Laceration to employees left thumb requiring four sutures.

The employee felt a twinge in his lower back when he got up out of the chair in the operators cab of the dragline. On 06/19/2011, he was taken to see a doctor and on 06/20/2011, he was not able to work a full shift and this met the criteria for reporting.
Employee was sitting down, became dizzy, fell from a seated position to the ground, striking his face requiring 3 stitches. Employee had been recently ill with sinus infection.

Employee stepped sideways at shift change and aggravated his right knee. Diagnosed 09/08/2010 and placed on restricted duty. This is a continuation of an injury that occurred 09/15/2009 that was not reportable. The knee has continued to cause him aggravation since the initial incident of 09/15/2009.
Employee was removing a metal sign from a post when sign dropped down on employee's 5th phalange on left hand. Employee lost the tip of the finger. Employee was not wearing gloves at the time of the incident. **Employee had little finger amputated at the middle distal.**
Employee was carrying a fuel hose while climbing up the ladder on a fuel truck when he missed a step and fell to the ground landing on his back. Employee reported that his back was hurting and was taken to a Doctor and exaimed.
EE was replacing hauler parking brake bracket, was lining up the bolt holes and started to install the keeper bolts when the retainer bracket slipped and mashed EE's left thumb between retainer and tail housing shaft of transmission.
Employee was cleaning mud from the tracks of a D11 dozer. He moved toward the front of the dozer and slipped on some mud which was on an incline causing his left leg to fall in a drain.

Employee was using a utility knife to cut belt material. The utility knife slipped out of the belting and cut the employee's left middle finger requiring five stitches.

Employee was crossing a wet and muddy ditch when he stepped into a hole causing the right leg to stop and his body to go forward injuring the right knee. Upon further evaluation by an Orthopedic doctor, it has been determined that a procedure is required to repair the torn cartilage in the knee.
Employee removed a danger tag from a tool box. As he started away from the box, his right foot became entangled with a lanyard that had fallen to the floor from a safety harness hanging on the side of the toolbox. The employee fell to the floor of the shop fracturing his left wrist.
A fire broke out on the Cross-Pit Spreader between the 70-meter and the 20-meter. Inspections by structural experts revealed that a belt pulley possibly became extremely hot under the transfer point. The machine is not usable at the current time. Analyses are being conducted to determine if it is economic to refurbish or scrap the machine.
Employee was spotting for the pulling of a 1000-foot piece of disconnected trail cable. The employee who was standing near the cable was struck in both knees when the cable became tight and then dislodged, requiring surgery to the left knee and possible future repairs to the right knee.
Moving oxygen cylinder around in bed of field service truck. R index finger was caught in between cylinder and bed of truck.
As employee was setting a pump line, the line was hung at the end by a motorgrader causing the line to move. She fell to the knees and felt a pain in the right shoulder, tension in the neck, and pain down the right arm and into two fingers.
Employee was installing spherical bearing on the left side of a dozer. In the process of driving the bearing on, a piece of metal came off, hitting the employee on the right forehead temple requiring stitches.
Metal came off bow string when testing for strength, puncturing left hand between thumb and index finger becoming embedded, requiring removal and stitches.
Employee was welding a crack in the mast of a dragline. A piece of metal generated from the task embedded in the right eye possibly from wind blowing which was removed by an Ophthalmologist.

Employee was carrying mats up a catwalk on a dragline, extending the right arm to set a bag of mats on a landing and felt a pop in the right shoulder. On a follow-up visit with a doctor 02/06/2008, physical therapy was prescribed causing this injury to be reportable.
Unknown - employee said that it may have been from the 2 previous vehicle accidents he had been in.
Attempting to retrieve a section of discharge line. Employee placed his left hand on top of the line that had bolts sticking up, when a rock fell on his hand, cutting finger.
Employee was working in tight quarters. He bent over and his hard hat came off. As he was repositioning to retrieve the hard hat, he raised up bumping his head against a component bracket. He suffered a laceration requiring sutures.
Highwall slide pushing Hitachi Backhoe across pit up against spoil. The coal loading activity was halted. There were no injuries.
Employee was placing a spacer in between a jack stand and an end dump axle to raise the right front tire slightly off the ground. The load shifted and the axle fell on the employee's right little and ring finger tips causing hairline fracture.
As an employee was unhooking an air hose connection on an air line, sudden air pressure blew air containing small pieces of metal on the top of his left hand.

Employee was driving a coal hauler and ran over some bumps in the haul road and sprained/strained her back.
Employee was backing up a bulldozer across a ridge of dirt, hitting a depression in the coal shoving himself forward striking the right eyebrow on the corner of the computer screen display monitor. The injury required four sutures.
Employee jumped off the right track of a dozer and twisted his knee resulting in an injury to the knee joint.
Employee stepped on electrical conduit and sprained his right ankle.
Employee was assisting in repairing dragline bucket. The work required him to use pry bar while the plate was being tack welded into place. Later that night, he realized he had burnt his eyes from the welding activity.
Employee was grinding and a metal particle got in his left eye. The particle became embedded and had to be removed by doctor.
Employee was striking a slugging wrench with an 8 lb. hammer. He struck his little finger against the slugging wrench causing a slight fracture to the distal bone on the left little finger.
Employee hit head while walking under the discharge ram of a bucketwheel jamming the neck. On a follow-up visit 06/28/2006, physical therapy was prescribed causing this injury to be reportable.
Employee was positioning trail cable when it moved and mashed his right middle finger against a trail cable crossing block. The employee suffered a laceration requiring 4 sutures.
Employee was mounting backhoe. Foot slipped on ground as he was stepping up. Right elbow bruised when it hit ladder handrail. On 06/07/2006 saw Doctor. Fluid drained causing injury to be reportable.
While off loading compressed gas cylinders from truck a cylinder tipped over. Employee right middle finger was fractured when caught between cylinder and bed rail as he attempted to stop it from falling
Employee was getting in and out from under the dragline shoe. His left knee began to bother him. He did not report an injury until 05/25/2006. On 06/20/2006 the employee received medical treatment and the injury became reportable.
Employee was spray painting handrails outside of shop. Later in the shift his eyes became irritated and the next day he was examined by a physician and given a prescription for itching of eyes due to allergies.
Employee was walking in the shop and bumped right knee on the brace of a crane structure. He did not report the incident until 03/27/2006. Employee received medical treatment on 03/28/2006.
Employee received a contusion to his left leg when dragline drag rope struck him while handling rope. Note: On follow-up doctor's visit on 2/22/06, physical therapy was prescribed causing this injury to be reportable.
Employee was handling trash bag. He cut his left little finger on a sharp object when he pushed on the trash bag. The wound required 2 sutures and restricted duty.
Employees were erecting a mine limit fence in the 3-C area. Pickup truck catalectic converter ignited vegetation. Truck was totalled. Less than 1 acre vegetation burned. Fire lasted approximately 30-45 minutes. No injuries.

Grass fire in non-active area of mine. No injuries. No equipment damage. Fire allowed to burn out due to wooded area and terrain. Kept fire watch. Fire started from smoldering wood from previous fire (12/1/05 - fire did not last 30 min.).
EMPLOYEE WAS TAPPING A FRONT END LOADER BUCKET TOOTH OFF ITS BASE WITH A STEEL HAMMER WHEN A PIECE OF METAL FROM THE TOOTH STRUCK EE IN HIS LEFT BICEP. THE WOUND REQUIRED ONE SUTURE.
Employee was picking up choker and felt pain in his right elbow.
Employee was torqueing the mg set coupling bolts on a dragline with a hydraulic torque wrench. The wrench body pinched his left pinky finger between it and the coupling. This caused a laceration requiring stitches.
Employee was handling trail cable when he felt pain in lower back. Employee received multiple chiropractic treatments and restricted duty.
Tying rope to trackhoe bucket when operator moved bucket, trapping hand between bucket & rope.
Employee was sitting in electrician's truck. He turned to his left and felt pain in lower back.
While priming pump #9 EE hit the opening of pump #9 housing w/ his rt. hand while holding a 5 gal. bucket of water w/ his rt. hand - pump housing has a sharp edge.

Doing undercarriage on dozer. Missed grab iron stepping from track to engine compartment, lost balance and fell. Left buttock struck tilt cylinder. Dr diagnosed deep bruise to left buttock. Dr. prescribed phys.therapy (determined to be "med. treatment"). Employee did not report injury to mgmt. until 11/23/04. Received "med. treatments" from PT 12/02/04 which triggered reporting.
Employee was using an air hammer to loosen a tooth adapter on a Bucket Wheel Excavator so it could be removed. In placing the air hammer bit against the tooth adapter a piece of metal came off striking the left forearm and became embedded. The metal was removed at the emergency room by a doctor.
Note: While the injury occurred on 08/03/04, it did not become reportable until 09/09/04 when the employee was placed on Restricted Duty. Employee injured his left shoulder when descending ladder on hauler. MRI revealed torn rotator cuff. Employee had surgery 09/16/04.
Employee was installing a new idler on bulldozer in shop. As the idler was being lifted from the dozer, the employee grabbed the shackle attached to the wire sling and pushed on the load. His left hand was caught and mashed between the shackle and the grab iron on the dozer. He suffered bruising and a fracture to the left index finger bone behind the knuckle.
(The company did not have knowledge of medical attention until 9/22. According to medical records on 09/27. Employee had fluid drained off his right elbow on 05/21. We discussed with the mine's MSHA inspector on 09/23/2004.) Employee was straightening out air hose and bumped his right elbow on a light pole causing slight swelling and pain.
Employee was cutting a keeper to weld on a drag rope support roller shaft for a dragline using a plasma cutter. The tip became stopped up and he rubbed it on the index finger of the gloved left hand in an attempt to clean the tip. The trigger was activated causing the cutter to arc burning the end of the finger.
Employee was changing Bucket Wheel teeth. A sliver of metal came off the bit of the air hammer and punctured the employee's left arm. He was carried to the ER and the doctorm removed the metal.

WHILE TURNING THE SHEAVES TO GET THEM OUT OF THE TOWER, HE WAS HOLDING ON TO THE I-BEAM STRETCHING OUT TO TURN THE SHEA VES AND THEY SLOWLY SWUNG AND CAUGHT THE MIDDLE FINGER ON HIS LEFT HAND AGAINST THE I-BEAM. THE LACERATION ON HIS LEFT FINGER WAS CLOSED WITH THREE STITCHES.
EE WAS LIFTING AND PLACING A TRAIL CABLE SHOE ONTO THE BACK OF AN ELECTRICIAN'S TRUCK WHEN SHE FELT PAIN IN HER LEFT ELBOW. SHE WAS DIAGNOSED WITH TENDON STRAIN AND PLACED ON RESTRICTED DUTY.

EE WAS WALKING FROM FUEL ISLAND TO SHOP & STEPPED ON A LARGE ROCK [TURNED RIGHT ANKLE]. EE DID NOT REPORT THIS INJURY UNTIL 8/7/97.
EE WAS OPERATING DOWN THE HAUL ROAD GOING TO THE XPS OUTAGE. THE FORKLIFT ENGINE COMPARTMENT DOOR WAS LOOSE & MAKING A NOISE. WHEN HE TURNED AROUND TO LOOK AT WHAT WAS MAKING THE NOISE, A SMAL L PARTICLE FLEW IN HIS LEFT EYE.
EE WAS RE-SOCKETING DRAG ROPES. WHILE TWO OTHER EES LIFTED THE ROPES OUT OF THE DRAG SOCKET, IT FELL ON THE GROUND. EE THOUGHT HE WAS BACK FAR ENOUGH TO CLEAR THE ROPE. A BURR STRUCK HIS LEFT LEG [CALF] REQUIRING SUTURES.
EE WAS USING A HAMMER TO KNOCK AN EASE OUT INTO A BROKEN BOLT. WHEN PIECE OF METAL CAME OFF THE EASE OUT STRIKING HIM IN THE LEFT EYE. THE WHITE OF THE EYE REQUIRED TWO STITCHES. EE WAS NOT US ING DOUBLE EYE PROTECTION AND HAD SAFETY GLASSESDOWN ON NOSE.
EMPLOYEE WSS HOOKING UP A TRAIL CABLE SHOE CHAIN TO A DOZER. HIS FOOT SLIPPED CAUSING HIM TO FEEL A SHARP PAIN IN HIS LOWER BACK. THE ACCIDENT OCCURED ON 6/1/97. THE INJURY WAS NOT REPORTED UN TIL 6/25/97. EMPLOYEE WENT ON RESTICTED DUTY 6/25/97.
EE WAS CUTTING A PIECE OF EXPANDED METAL. THE METAL DIDN'T GET CUT AL THE WAY THROUGH SO HE GOT HIS CHAINLOCKS OUT TO BREAK THE METAL OFF, WHEN HE MOVE THE METAL UP AND DOWN HIS RIGHT RING FIN GER GOT CAUGHT, CUTTING FINGER. STITCHES. WAS REQUIRED.
NOT PAYING PROPER ATTENTION TO HIS ACITIVITIES AT THE TIME HE WAS PERFORMING TASK. DID NOT NOTICE THAT SUSPENSION LINK WAS SUSPENDED BY ONE PIN LAYING ON TOP OF ANOTHER WHEN HE KNOCKED IT OFF , AND OT NOTICING THAT HIS FOOT WAS DIRECTLY UNDER LINK.
EMP WAS HELPING LEVEL A PORTABLE BUILDING AND HIS KNEE GAVE WAY. EMP STATED THAT HE HAD THIS PROBLEM SEVERAL TIMES BEFORE.
EE WAS REACHING OVER TO PICK UP CAM BOLTS WHEN HE FELT A SHARP PAIN IN HIS LWOER BACK. EE RESTRICTED DUTY STARTED ON 4-17-97.
EE WAS REBUILDING D10 TILT CYLINDER WITHOUT PROPER TOOLS & STRAINED HIS BACK.
WHILE STRIPPING CABLES ON STEEL CABLE BELT, UTILITY KNIFE SLIPPED, CUT THUMB ON LEFT HAND 2 ABRASIONS, ONE LONG LACEERATION OF THUMB TO KNUCKLE REUIRED 5 STITCHES AND ONE SHORT LACERATION BESI DE OTHER LACERATION REQUIRED 2 STITCHES. END OF SHIFT, WENT TO ROOM.
EE WAS FUELING 992C CATERPILER FRONT END LOADER AT THE FUEL ISLAND. AFTER FUELING, EE REACHED DOWN TO HOOK THE FUEL NOZZLE ON THE LADDER SO HE COULD USE BOTH HANDS TO CLIMB DOWN. WHILE HOOKING THE NOZZLE, HE SLIPPED & FELL HITTING THE CEMENTSORE HIP & BACK
EMP WAS CLOSING DOOR BEHIND HIM AS HE ENTERED THE DOZER. FRACTURED RIGHT INDEX FINGER.

CLEARING BRUSH BY HAND FOR FENCH LINE AND CAME INTO CONTACT WITH MACHETE. EMPLOYEE REQUIRED FOUR STITCHES TO LEG LACERATION.
EMPLOYEE'S LEFT AND RIGHT WRIST HAD BEEN HURTING. SHE WAS DIAGNOSED WITH HAVING CARPAL TUNNEL SYNDROME. (SEE ATTACHMENT). NOTE: THE DATE AND TIMES OF THE ILLNESS ARE UNKOWN, THIS IS ALL THE IN FORMATION THAT WAS ON THE ACCIDENT REPORT.
EE WAS LIFTING AIR FILTER OUT OF BOX WHEN HIS HAND CONTACTED THE SHARP METAL EDGING SURROUNDING THE OUTSIDE OF THE FILTER CUTTING HIS LEFT PALM.
EE WAS DISMOUNTING TRUCK WHEN HIS FOOT SLIPPED CAUSING HIM TO HIT THE GROUND OFF BALANCE AND HE TWISTED HIS RIGHT KNEE.
EE WAS REMOVING A RACE FROM THE BUCKET WHEEL HUB BY HITTING ON A B & O HAMMER WITH A 12# SLEDGE HAMMER. WHILE STRIKING THE B & O HAMMER WITH THE SLEDGE HAMMER A PIECE OF METAL CAME OFF STRIKIN G HIS RIGHT ENTER THIGH.
EMPLOYEE WAS REMOVING AN IDLER CAP BOLT FROM THE LEFT REAR IDLER WITH A 1" AIR IMPACT WRENCH. WHILE REMOVING THE IDLER CAP BOLT HIS LEFT FOUR FINGERS GOT CAUGHT BETWEEN TEH BOLT AND FLOOR. H E RECEIVED A HAIRLINE FRACTURE TO HIS LEFT LITTLE FINGER.
EE WAS OPERATING 11054 D10N DOZER IN THE PIT DOWN 4-C RAMP TO THE RIGHT. HE WAS GOING AROUND THE HATACHI BACKHOE TO BACKFILL COAL WHEN HE SLID OFF THE COAL EDGE. THE DOZER FELL ABOUT 8' ON ITS SIDE. EE HAD SEAT BELT ON BRUISED BACK.
EE WAS TIGHTENING BOLT WHILE MOUNTING CRAWLER GEAR BOX. HAMMER HIT LEFT INDEX FINGER. FRACTURE FINGER AT SECOND JOINT.
