Air arcing on a wash plant screen and received a flash burn on both eyes.
HE WAS PUSHING A BAR INTO THE SCREEN AND BENT HIS WRIST BACK.
Loading hydraulic pump into back of parts truck. Pump flipped over pinning finger between pump and truck bed.
Operator said his end dump was over heating. The operator stopped to check coolant level and as he was loosening the filler cap it came off suddenly and sprayed his face and chest and arm with hot coolant. It resulted in first and second degree burns on his face, arm, and chest.
Lifting and cleaning heavy equipment parts.
Tearing down and cleaning engine parts. He was lifting and moving heavy parts and twisted back.

IT WAS DAR AND EEE WAS TRYING TO HURRY TO GET CRUSHR FIXED. SHOULD HAVE LOOSENED BRACKETS BEFORE TRYING TO PUT BELT ON.
EMPLOYEE WAS REMOVING OBSTRUCTION FROM CRUSHER CHUTE WHEN MATERIAL DISLODGED AND MASHED HIS LEFTRING FINGER AND BROKE THE TIP.

EMPLOYEE WAS CUTTING METAL WHEN A SPARK ROLLED BACK AND HE LAYED HIS ELBOW BOWN ON IT. THE INJURED SITE LATER BECAME INFECTED.
