Employee was struck by a chain he was installing.

Employee was cleaning in the prep plant with a shovel. When he twisted around he felt pain in his knee. Employee worked until 08/15/2009. It took until 09/16/2009 for it to be determined from a peer review if this was a new injury and not a re injury. This is why the 10 day limit was passed.
Repairing on dryer.

Repairing in the wash plant changing an 8 in pipe when a piece broke and fell striking his lower leg.
EE was using a grinder when it kicked back off of the metal. The grinder slipped from his grip and struck his neck. He went to the doctor after work and received two stitches.
Employee was working on an D-11 dozer putting the track together when his chain slipped causing the track to strike his knee.

EE climbing onto his coal loader in the coal yard. He slipped climbing up the loader ladder and caught himself with his right arm causing some damage in his shoulder. This has been a medical only until 01/24/2007 when it went lost time.
LIFTING LIGHT PLANT ONTO HITCH OF TRUCK. LIGHT PLANT SLIPPED THROUGH HIS HAND AND FELL. IT LANDED ON HIS FOOT.
Employee climbed tank trailer and fell.
Slipped while climbing onto Bobcat Loader. Strained back muscle. Was medical only at the time but turned into lost time accident when employee had surgery on 4/26/06.
Changing spirals. Pushing up on the "t" to pull hose down onto it.
Ground was frozen in the morning and as the sun hit the haul road in this location it became slick. Truck became sidways on road and when it contaced a dry area it tipped over on its side.

EE was climbing onto dozer. He slipped and fell between push arms & track frame, causing pain to left knee & lower back. (HAD ARTHROSCOPIC KNEE SURGERY ON 8/18/05.)

EE WAS CUTTING A PIECE OF STEEL. GAS IGNITED AND BURNED THE FRONT OUT OF HIS PANTS.
