Employee twisted knee while climbing stairs to the lab.
Employee was climbing down steps on off-highway haul truck and slipped. When employee caught themselves on the rail, they injured their left shoulder.

An employee felt a pull in right calf while walking between railcars. Employee was diagnosed with a pulled muscle with no restrictions. On 11-15-23 they went for a sonogram. On 11-17-23 they were prescribed physical therapy. The first PT appt. was 11/21/23 for 6 sessions. We were informed today that their appts had started.

On 11/5/20 an employee was throwing sample bags into a dump hopper and felt EE's knee pop. EE's knee hurt for the remainder of EE's shift. At this point, EE feels EE does not want to seek medical care. The employee was taken on 5/26/2021 to Occ Med at Jackson TN. EE has been referred to an Orthopedic Physician in Paris for an MRI. EE received a prescription for Prednisone.

Employee was to load a bulk truck trailer at approximately 8:00 am. The employee stated that they opened the hatch with right hand. The left hand was resting on a rail or bar. The lid dropped onto a finger causing a laceration. The employee was taken to a local ER where they received 3 stitches.

An employee tried to dislodge a large cley knot, it spun and lightly pinned a hand against a beam. The leather glove bunched up and "pinched" the skin on the back of the right hand. When they pulled the hand away holding the knot the skin ripped away approx. 2 inches wide.

Employee was stacking bags from a bagging machine to a pallet. As the employee stacked a bag, EE felt a pop in EE's back. EE initially thought that EE had just turned the wrong way. After stacking two more bags the pain increased the employee reported to the supervisor and requested medical care. EE was placed on restricted duty.

Employee was operating an industrial vacuum cleaner when an actuator failed, causing his index and middle finger to be smashed between the vacuum and the collection hopper. His index and middle finger on his right hand were broken between the fingernail and knuckle. He was treated at the ER and released the same day.

Employee was driving a Mack road tractor with a dump trailer out of a clay pit when the truck lost traction and slid down the haul road incline. The dump trailer jack-knifed and lifted the tractor off the ground. When the tractor dropped back down to the ground the jarring from the wheels hitting the ground caused a compression fracture in his lower back.
EE left hand fingers were cut by a closing door while installing bags in the drier dust collector. EE received 3 sutures in 3 of his fingers.
EE went to the doctor on his own time with back pain. The diagnosis was a bulging disk in his lower back. EE was off work on doctors orders from 6/22/2011 to 8/28/2011. On 9/7/2011 the doctor deemed EE's condition as work related. The doctor attributed the condition to vibrations from operating a dozer.
EE cut his index finger on his left hand. EE was coming down the stairs from a silo when he caught his finger on a jagged edge on the metal handrail. EE was treated at East Wood Clinic Urgent Care in Paris and received 7 sutures.
EE was helping change a tire on a John Deere 1812C pull scraper, when the wheel hub slipped and cut his right index finger. EE was taken to a local clinic where he received 5 stitches.

On 12/11/2007 a haul truck leaving the mine travling to the mill was involved in a one vehicle accident. The driver of the haul truck swerved to miss a dog in the road and over corrected causing the truck to leave the highway and come to rest in a field. The driver suffered a broken foot and the truck recieved damage to the front hood and cab area.
Miner went to the doctor,on his own time, because of shoulder discomfort. The Dr. diagnosed his conditon as a form of carpal tunnel requiring surgery.

EMPLOYEE WAS HLEPING PUT A MASTER LINK IN D-6 DOZER TRACK. HE WAS HOLDING HT EPIN WHILE ANOTHER EMPLOYEE WAS DRIVING IT IN. A PIECE OF METAL BROKE OFF HTE PIN AN HIT EMPLOYEE IN LEFT SIDE OF NECK. PUNCTURE WOUND IN LEFT SIDE OF NECK.
EMPLOYEE WAS WORKING ON SCREW CONVEYOR IN UPPER PART OF PLANT. CAME DOWN STAIRS AND BECAME WEAK & DIZZY. WAS CARRIED TO HOSPITAL E.R. & WAS DIAGOSNED AS HEAT EXHAUSTION & TOLE HE COULD RETURN TO WORK. 7-19-93 EMPLOYEE TOLE FOREMAN HE DID NOT FEEL BETTER. HE WENT TO DOCTOR & HE HAD A MILD BACK STRAIN AS WELL AS HEAT EXHAUSTION. WAS TOLD TO BE OFF WORK FOR ONE WEEK.
EMPLOYEE WAS INSTALLING CYLINDER ON BOOM OF CATERPILLAR EXCAVATOR, HAD CHAIN AROUND CYLINDER LOWERING ONTO BOOM. CHAIN BROKE ALLOWING CYLINDER TO FALL AND CAUGHT EMPLOYEE'S LEFT HAND BETWEEN C YLINDER AND BOOM. CONTUSION TO BACK OF LEFT HAND

EE WAS GOING UP AND DOWN ON A LADDER ATTACHED TO A DUST COLLECTOR CHANGING BELTS ON DUST COLLECTOR. WHILE COMING DOWN LADDER EE BUMPED HIS LEFT KNEE ON LADDER.
EE WORKING ON HAY STACKER USING CUTTING TORCH TO CUT A PIECE OF METAL. AFTER CUTTING TORCH OFF, EE LAID THE PEICE OF METAL ON A TABLE AND RAISED HOOD ON HELMET. A PIECE OF HOT SLAG POPPED OFF METAL AND HIT EE ON LEFT EYE LID AND SIDE OF NOSE.
EMPLOYEE WAS CLEANING OUT THE BED OF DUMP TRUCK SHOVEL WHERE CLAY HAD BUILT UP IN CORNERS. HE WAS BENDING OVER TO DO THIS AND WHEN HE WENT TO STAND UP STRAIGHT, HE HAD PAIN IN THE LOWER BACK.. HE WAS NOT LIFTING.

MOVING A 100 LB. BAG OF SODA ASH FROM PALLET, WHILE PULLING BAG OFF TOP OF STACK THE BAG FELL TO THE FLOOR JERKING EMPLOYEE FORWARD AS HE HELD ONTO END OF BAG CAUSING LOWER BACK STRAIN.
EMPL WAS OPERATING D-4 CATERPILLAR DOZER TO PULL FUEL TRUCK INTO MINE. WHILE CLIMBING OFF DOZER HE LOST FOOTING & FELL TO GROUND ON BACK & TAILBONE. EMPL REPORTED TO SUPERVISOR 6/12/89 HE WAS INJURED & CAME TO OUR OFFICE ON 6/20/89 TO FURNISH INFORMATION FOR THIS REPORT.
