THE OPERATOR HA8 BACKED THE DRAGLINE INTO MINING POSITION WITH THE CAB FACING THE PIT. ONCE IN POSITION THE OPERATOR FAILED TO APPLY THE PARKING BRAKES. THERE WAS NO SAFETY BERM BETWEEN THE TR ACKS AND THE PI7. THE MACHINE STARTED TRAVELING TOWARD THE HOLE. THE OPERATOR GOT OUT OF THE CAB AND TRIED TO CLIMB OVER THE SAFETY RAIL AS THE MACHINE WAS TRAVELING INTO THE PIT. THE MACHINE

Two employees were opening the hood of a dump truck. One of the employees stepped to the front of the truck to prevent the hood from moving too far forward and breaking the hinges of the hood. EE ended up taking the weight of the hood and this resulted in a small tear in EE's right bicep. EE is under lifting restrictions until an orthopedic appointment.

The employee was installing a discharge cover on a water pump. Once in position, the employee began to push the housing toward the pump. The pushing movement caused one of the chain hooks to twist allowing the hook to disengage. The housing dropped approximately 2 feet and struck the employee's right foot above the safety toe

A maintenance employee was injured while using a portable side grinder to cut a piece of channel. The cut-off wheel lodged in the cut and kicked back into the inside of left leg just above the knee. The laceration was deep enough to require stitches. The grinder had a guard installed and the employee had on all required PPE.

EE was sweeping the floor near a conveyor. As ee turned around to leave the area, ee struck right shoulder against the edge of a catwalk. EE lost balance, and fell forward but caught self with right hand on the conveyor structure but ee spun clockwise and left hand/wrist passed between the belt and the troughing roller. The EE received two stiches to hand.

As employee began to descend a ladder to dust collector ee missed a step. This caused ee to briefly hang by the right arm / hand. This resulted in two torn bicep ligaments.

Employee was pulling a rope that raises and lowers the Carbis safety cage on load out for bulk trailers. When pulling the rope to raise the cage from the trailer, ee right shoulder made a popping noise and immediate pain in shoulder. EE went to a physician on 2-9-2017 and was given restricted duty. EE returned to work on next shift.

Employee was loading trucks from the silo, which involves climbing up and down several steps to the loading platform and also climbing up and down the steps to the wash down platform, when EE became nauseous. EE was later diagnosed with heat exhaustion.
Employee picked up sample from the back of a pick-up truck, extended left arm to sit the sample on the dock at which point EE's shoulder "popped". At this time pain is not such that employee wishes to seek medical attention. Over time employee stated EE is beginning to have issues with shoulder. EE is now on restricted duty as a result of Dr. visit on 6/8/16

EE hit his shin when he slipped while climbing onto a Cat 938 loader. The result was a minor cut and bruise on his shin. This accident occurred on 12/11/2010 but did not become reportable until 12/28/2010 because of an infection. EE was admitted to the hospital to have the infection treated and fluid drained.
EE was lifting a 50lbs bag of clay when he felt a pull in his left arm and started having pain. An Orthopedic doctor diagnosed EE with left elbow strain and treated with a cortisone shot. His arm was placed in a elbow strap and he was instructed not to use it until his follow up appointment.

Mill foreman was walking down a handicap ramp leading from the office warehouse when he slipped on a slick spot and fell on his right knee.
A mechanic was driving a pin into a Cat loader bucket arm with a sledge hammer. The injured mechanic was holding the arm when the sledge hammer glanced off the pin striking his hand.
