While performing work on the jaw crusher, a jack accidentally struck the employee in the back of the head, when a weld broke on a plate that was used to secure the jack. This caused a laceration, requiring stitches and spinous process fracture.

A ROCK ABOUT THE SIZE OF AN ORANG E ROLLED DOWN SLOPE STRIKING SHOE THEN BOUNCED ONTO HIS SHIN CAUSING A SMALL CUT. INJURED HAD BEEN HAZARD TRAINED PRIOR TO ACCIDENT.
GOING ACROSS HOOD OF TRUCK TO ADJUST PASSENGER SIDE MIRROR. FOOT SLIPPED AND FELL TO GROUND, FALLING APPROX. 10-12' TO GROUND. FRACTURED LEFT HEEL.

BUT TO RETURN FOR FOLLOW UP 12-30-00. AT THAT TIME LEG HAD BECOME SWELLED. DOCTOR MADE INCISION TO DRAIN FLUIDS ON LEG. NO LOSS AT WORK OR RESTRICTIONS.

EMPLOYEE, ANOTHER EMPLOYEE AND PLANT FOREMAN BEGAN CHANGING ROLLERS ON CONVEYOR SC2 AT 7:30 AM. THEY TOOK A BREAK AT 8:30 AND AGAIN AT 9:30. AT 10:30 EMPLOYEE SAID HE DID NOT FEEL WELL. HE WAS SENT TO THE OFFICE TO COOL DOWN, 20 MINUTES LATER HE WENT OUTSIDE & STOOD FOR ANOTHER 40 MINUTES WHILE OTHER PEOPLE FINISHED. AT 11:30 FOREMAN SAID EMPLOYEE WAS NOT VERY COHERENT & EMS CALLED
EMPLOYEE AND ANOTHER WERE POSITIONING A PIECE OF SCREEN CLOTH IN PREPARATION FOR LIFTING CLOTH UP ON THE TOP DECK. EMPLOYEE LOST HIS GRIP ON SCREEN CLOTH AND AS SCREEN FELL, THE END OF ONE OF THE WIRES CUT THE INSIDE OF EMPLOYEE'S LEFT WRIST, INCLUDING ONE TENDON REQUIRING APPROXIMATELY 8 STITCHES TO CLOSE THE WOUND.
