EMPLOYEE WAS STANDING ON OUTBOARD OF THE EUCLID DUMP TRUCK (WHICH HAD SLID OVER A BANK AND FRONT AND REAR WHEELS OF RIGHT SIDE WAS STUCK IN THE MUD), TWO D9 DOZERS WERE ON BOTH ENDS TRYING TOT REMOVE THE TRUCK. THE EMPLOYEE BECAME FRIGHTENED THAT THE TRUCK MIGHT TURN OVER AND JUMPED.

AS HE WAS MOVING THE SEPERATOR HEADER PIPE FROM OUTSIDE TO THE PLANT HOIST,HIS FOOT FELL THROUGH A PIECE OF FLOOR GRADING CAUSING HIM TO SLIP AND HE REACHED FOR THE PIPE TO CATCH HIS BALANCE,T HE PIPE ROLLED OFF THE CART MASHING HIS RIGHT FINGER AGAINST THE BELT FRAMING.
HE WAS STEPPING FROM A LOADED BARGE ONTO THE DOCK.HE SLIPPED AND HAD FALLEN-STRIKING HIS HEAD ON THE FLOOR.HE SUSTAINED 15 SUTURES DUE TO THIS ACCIDENT.

HE WAS CHANGING A 10 H.P. MOTOR ON THE FLOOR PUMP MODUEL #1. WE BOTH PICKED UP THE MOTOR TO BOLT IT TO BASE AND MOTOR SLIPPED AND CAUGHT MY FINGER AND HAND ON THE RIGHT HAND. I WAS JUST TRYING TO GIVE HIM A HAND WHEN I WAS MAKING MY ROUNDS.
REOCCURRANCE TO PREVIOUS INJURY; CONTINUED BACK PAIN. EMPLOYEE CURRENTLY HAS BEEN RECEIVING CARE FOR INITIAL INJURY BY DR. NO NEW TRAUMATIC EVENT HAS OCCURRED.

THE VICTIM WAS INSTALLING A SHIEVE AND CABLE ON THE BOOM OF A CRANE. THE SHIEVE WAS IN THE AIR AN SLIPPED CATCHING THE VICTIM'S THUMB BETWEEN THE CABLE AND SHIEVE.
STRAINED BACK WHILE PULLING A PLUG OUT OF THE DRYER IN THE PREPARATION PLANT.
THE VICTIM WAS LOOSENING MOTOR MOUNT BOLTS AND SEND OF THE WRENCH SLIPPED OFF THE BOLT. THE WRENCH THEN STRUCK THE VICTIM IN THE FOREHEAD CAUSING TWO CUTS.
