Visitor was walking up set of five steps at main office and lost his balance. He stumbled and fell on his bottom and then bump his head.
Worker was climbing down a fixed ladder and lost balance near the bottom. The worker fell backwards breaking his wrist.

Crusher Plant Operator lost his footing while loosening a bolt on a crusher. The wrench slipped off of the bolt and he slipped backwards, bruising his back.

EE WAS CLEANING UP AND WENT TO DUMP TRASH CONTAINER. HE STOPPED FORK LIFT SHORT OF A DROP IN PAVEMENT AND EMPTIED CONTAINER. WHEN HE RETURNED TO THE FORK LIFT AND STARTED TO BACK UP, THE FORK LIFT ROLLED FORWARD INTO THE DROP OFF. HE HIT THE ACCELERATOR AND THE LIFT SWUNG SHARPLY TO THE RIGHT AND PINNED HIS ARM BETWEEN A SUPPORT BEAM AND THE LIFT ROLL CAGE.
EE WAS SQUATTING DOWN WELDING ON A SCREEN FRAME AND LOST HIS BALANCE AND FELL BACKWARD. HE LANDED WITH HIS RIGHT HAND JAMMED IN THE DECKING GRID, RESULTING IN A BROKEN THUMB.
EE WAS WORKING MAINTENANCE ON AIR COMPRESSOR. AS HE CLIMBED DOWN FROM THE DECK-WORK HIS FOOT SLIPPED AND HE FELL, STRIKING HIS RIGHT LOWER BACK ON THE DECK.
