ACCIDENT VICTIM DIED FROM COMPLICATIONS APPROXIMATELY 2:00 A.M. 1-5-90. DISCUSSED WITH WORK FORCE THE PROPER WAY TO MOUNT AND DISMOUNT EQUIPMENT. PED, STRUCK TIRE OF LOADER AND FELL TO THE GROUND. IMPACT OF THE FALL CAUSED BROKEN RIBS.

Employee reported on 1/25/2012 that while driving a CAT 785 Rock Truck he hit a bump in the haul road causing him to bottom out on his seat resulting in a lower back strain. Following a complete review of the incident, management was unable to determine validity of the report and is filing this 7000-1 according to Part 50 recommendations to report questionable injuries.

EE injured was working in the area around a D11R Dozer. He stepped on a rock that was covered with mud. On the next shift he reported incident to superintendent and denied medical treatment offered. That evening the EE visited a Dr. for evaluation. The EE injured deemed to have a level 2 sprained/strained ankle. Investigation pending by company.

EE was proceeding down the haul road when he up shifted his truck from 1st gear to 2nd gear, when he shifted he shifted the transmission to the high side causing it not to go in gear. He proceeded down the steep grade out of gear and riding his brakes approximately 900ft before coming to a left hand turn and turning truck over on its side.

Employee was working to remove the teeth from an excavator bucket. After cutting the teeth from the bucket EE used a sledge hammer to dislodge the teeth. While pounding the teeth off the bucket a piece of metal broke from the hammer striking his leg. EE was taken by ambulance to Western Maryland Health Systems where he received 5 stitches to repair the laceration.

A blast was detonated causing material to slough off the outside of a hillside near the shot. The material displaced mud from an existing pond creating a wave of mud. EE's truck slid down an embankment, causing a contusion to his head.

At approximately 8:30 pm Injury descended the fuel truck to re-fuel a rock truck in the pit area. While walking to the rear of the fuel truck he stepped on a rock and twisted his left ankle.

At the beginning of day shift on November 18, 2008 EE was attempting to enter his dozer to begin the pre-shift examination. The door of the dozer was frozen due to snow and below freezing temperatures. When he pulled on the door handle and it broke causing him to lose his balance and fall backward off of the dozer striking his head on the ground.

Employee was working underneath a water truck and was stung by a bee. Shortly thereafter he started to exhibit anaphylaxix systems. He was taken to the hopital, treated with injection and intravenous medications. Following treatment he was released and sent home. He returned to work on his next scheduled shift.

Employee was attempting to lift and place a cylinder in the service truck. With the crane controls in right hand, he was attempting to spot the cylinder inside the service truck with his left hand. The load started to swing and smashed the tip of middle finger of left hand between cylinder and side of truck. ER doctor applied a splint and released him to return to work.

After pre-tripping my truck, I saw who I thought was the attending supervisor for J & W Excavating. I needed to tell him I had to leave at 2:00 pm that day. As I went between the truck & the building to tell him, I tripped & landed on my left knee which caught the corner of a metal pan filled with water that my right leg had fallen into (causing the cut to knee).

Operator removed the fuel cap from 3500 Hitachie shovel to check fuel level. The fuel tank had built up pressure in it causing fuel to spray in the operator's face and right eye. Went to doctor and had eye flushed and received prescription eye medication. Returned to work same day.

On 1-9-2006 employee was loading holes at a shot location and injured left shoulder. The next day the employee was using a pry bar on a drill steel and injured right shoulder. MRI was performed and Doctor diagnosed rotator cuff injury of both shoulders. Reported information to employer on 1-25-2006. First lost day was 1-11-2006.

Employee was operating a Cat D11N dozer pushing rock towards a Komatsu loader when the highwall fell on the right side and partially covered up the machine. The operator was entrapped a short period of time and extracated himself by kicking out the window glass on the oposite door.

A DOZER AND ROCK TRUCK COLLIDED ON THE MAIN HAUL ROAD WHEN THE DOZER WAS BACKING ONTO THE HAUL ROAD TO MAKE ANOTHER PASS WHILE PUSHING TOP SOIL UP A SLOPPED BACK FILL AREA. THE DOZER'S RIPPER CONTACTED THE TRUCK'S CAT WALK, COLLAPSED THE DOOR ON THE OPERATOR'S SIDE AND SCRAPED THE TRCUK BED. WHEN THE DOOR COLLAPSED, THE TRUCK DRIVER SUFFERED A FRACTURE TO HIS LEFT ARM.

ACCIDENT VICTIM WAS OPERATING A HOE CLEANING SPOIL ALONG THE EDGE OF THE HIGHWALL IN PREPARATION OF LOADING COAL. A PIECE OF HIGHWALL MATERIAL DISLODGED FROM THE HIGHWALL, ROLLED DOWN THE HIGH WALL AND STRUCK THE HOE OPERATOR IN THE LEG.

ACCIDENT VICTIM HAD PARKED HIS TRUCK ON SLOPING GROUND TO BE LOADED WITH COAL. AFTER BEING LOADED HE APPARENTLY PLACED THE SHIFT SELECTOR IN REVERSE INSTEAD OF 1ST FORWARD GEAR. AS THE TRUCK W ENT BACKWARD, THE OPERATOR COULD NOT THE TRUCK AND JUMPED TO THE GROUND.
