An employee was working on a diverter gate when the gate released residual pressure and made contact with the employee on both their hands and chest. This incident is under privileged and confidential
On 5/19/2025 at 3:00 PM, after working on FM1, an employee was leaving the Mill area when attempting to step down onto a platform 2 feet lower than the Mill, and they sprained their ankle.
Wrench slipped, struck employee lip causing laceration.
The employee swung a pickaxe to break the built-up clinker Build up. Then, they felt a sharp pain in their right shoulder, stopped using the pickaxe immediately, and reported it.
At 11 AM on 2/18/2025, an employee was working on a concrete pump, attempting to drive off the bearing housing. The employee put a wedge in place and hit it with a hammer, but the wedge flew off it hit the employee in the mouth. This resulted in a lower lip laceration which needed 3 sutures.

On September 17, a technician was working on the HT.211.VF1 vibrating feeder. While grinding the employee stated that a piece of metal entered the left eye through their safety glasses. The employee was then sent to the eye doctor where a piece of metal was removed and the healthcare provider prescribed a prescription eyedrop.
Employee was working on a flapper of a pneumatic pump, and reported feeling a strained muscle in shoulder. Employee continued normal shift work, after reporting the incident to employee's supervisor. On 8/9 employee was placed on modified duty resulting in reportable incident.

Employee forced movement of mechanical parts of a fan drive motor while changing belt and failed to keep finger clear of pinch point of pully and drivebelt resulting in finger laceration.
While flipping blow bars in 2nd crusher, employee was using a hammer to drive in a blow bar when a piece chipped of and struck mechanic in left leg above knee cap. The incident became reportable on 7/7/2023 due to restricted duty.
EE reported was working in the lab assembling an industrial type shelf. EE placed a shelf to secure it to the structure. The shelving began to teeter. As EE reached to grab the shelf to prevent if from falling, EE injured right shoulder.

On Dec 6 2022, an employee was injured while clearing a material jam in diverted gate chute 311-MW1. The employee had removed a side access door and used a bar to dislodge the material. When the material cleared, the air pressurized gate activated hitting the bar and pinching the employees right hand wrist. The incident is under investigation.
The employee was wearing gloves and long sleeves while vacuuming the roller crusher. Dust got in the glove and then on the wrist. The injury was diagnosed as a chemical burn. This incident became reportable on 07/11/2022.
Employees were stuck in the Blending Silo elevator, total of an hour and a half from the time they reported been stuck and that they got freed by the Fire department.

A production employee was working around a supplied airline when it disconnected from the hose reel and began to whip under the pressure of the air and struck EE in the left knee. This event was under investigation and was confirmed to be probable by Dr on 02/19/2021.

The operator was going into the muck pile when EE ran into a boulder on the left side of the bucket. This incident was initially reported as a First aid but was upgraded to a Modified Work duty on 6/25/2020.
While carrying metal frame, slipped on wet, muddy concrete. Frame landed on right hand causing fracture of right little finger.

PRODUCTION TECHS WERE REINSTALLING DOOR ON FM1 2ND COMPARTMENT WHEN THE LIFTING CHAIN BROKE. WHEN THE LIFTING CHAIN BROKE IT STRUCK AN EMPLOYEE IN THE LEFT HAND CAUSING LACERATION TO THE PINKY AND RING FINGER. THE EMPLOYEE WAS WEARING GLOVES AT THE TIME OF THE INCIDENT. THE EMPLOYEE WAS TAKEN TO THE HOSPITAL FOR MEDICAL CARE.
On 7/22/2019 an employee reported that the previous week, EE had gone home and broke out in a rash after cleaning in an area with clinker dust. EE said that due to the nature of the rash EE had sought care at the ER during EE's days off.
A Maintenance employee was driving a bearing off of a shaft, a piece of metal hit the employee in the left wrist and broke skin. Employee had gloves and long sleeves on but the piece hit in a small gap between glove and shirt.
EE reported that they were replacing a side liner at work. It was tight area. EE was twisting and turning and felt a pull to the middle back.
While clearing a chunk of shale, employee struck left thumb against a chute bracket while lancing.

Employee was in the finish mill using an air hammer and it slipped. EE fell down and right forearm came into contact with some hot material causing it to blister.
Employee reported while removing the air filters when EE believed a piece of debris got onto EE's right eyelash and fell into EE's right eye. EE reported flushing EE's right eye immediately but feels a foreign body sensation lodged in the top right hand corner of EE's right eye.
Employee was walking up a flight of stairs and tripped. As they reached out with left arm to grab the railing and felt a "pop" in the left shoulder.
Employee was jack hammering when some of the dust went into EE's eyes and the back of EE's neck causing a chemical burn to the neck.
Employee slipped on ice while walking
Employee was walking and slipped on a snowy small incline in a decommissioned area of the plant. Employee said that they had pain in the left knee and thigh area. They felt a "pop" and could not straighten leg.
EE reports that EE was doing a lot of sledge hammering yesterday in awkward positions. EE states that EE's lower back was sore but did not think anything of it until EE got home last night.
Employee reported was lifting a water jug to place into a cooler when employee felt a 'pop' in right biceps area.
Employee experienced back pain after doing wear measurements in the Vertical Roller Mill.

A chipping hammer got stuck on a port hole at the bulk silo during silo cleaning and inspection. While attempting to dislodge the chipping hammer, the employee struck the bit collar with a 2-lb hammer. A piece of metal broke off and lodged in the employee's upper right leg area.
The hot material started a fire on apron belt conveyor. The fire started around 23:00 conveyor stopped shortly after the fire started. Fire squads arrived at 23:20 and put out the fire within 90 minutes. At 00:30, the fire was out and the fire fighters remained on site to secure the area until 02:47.
Dust collector was plugged with dust, employee tried to beat it down and use a bar. Then employee proceeded to blow out the dust collector with an air lance and some of the dust blew into right eye under safety glasses.

Employee reported that EE was checking the air valves when EE twisted left knee. EE felt some pain but continued to finish the shift. EE was treated the day and it was determined to be a sprained knee.
Attempting to switch a flop gate from @2 to #1 Clinker. Hit a gate handle with a bar, missed the handle, felt a pull in right shoulder.
Employee stated that EE was using a hand control on a loader and felt a pop in right thumb.
Employee sustained a displaced fracture of the right ring finger while dismantling a Dresser coupling on the dust collector at the preheater. Employee was using two pipe wrenches when the coupling broke into and pinched EE's ring finger between the pipe wrench and a steel beam.
Carrying a pole by self down the stairs when EE turned and the pole pinched EE's finger between it and the handrail. This pinch fractured EE's right hand middle finger.
While working employee expressed severe pain in the groin area.

Employee was welding shims into an expansion joint, after welding three into place he removed his welding gloves, turned, and bumped his right forearm on an exposed shim causing a 4 cm laceration on his right wrist.
Employee stated that he was stacking bricks. He stacked a brick onto his hand. He says it was his own fault. He hurt his left pinky fingernail.
Stepped onto retractable fall protection.
Pinch finger tip between materials while handling
Employee was removing existing sheet metal flashing from arc flash duct opening. When he pulled on the sheet metal, it broke loose and a razor-like shard of sheet metal struck the employee in the forearm causing a laceration. He received 8 stitches and returned to work the same day on full duty.

Employee was dismounting 990 model loader, when his foot slipped on the ladder causing him to lose his balance and fall. He landed on his right wrist and sustained a fracture.

Employee was using a skid steer to move a pile of snow. While scooping the snow, the bucket contacted a piece of railroad track which caused the equipment to stop suddenly. The operator complained of some neck pain related to whiplash. This injury did not meet report ability requirements until 1/2/2014 during a follow up visit.
Employee was unplugging a chute. A tool in use moved in an unexpected direction causing the employee to lose balance and strike the left hand on a piece of angle iron causing a cut. 10 stitches were needed.
Yard laborer employees foot metatarsal bone was fractured after the raised 1400 pound barrell of grinding balls he had accidentally stepped under slipped out of the lifting device from a height of several inches and came down on top of his foot. His steel toe safety boot did its job as designed.

During loading of the haul truck in the quarry post shot landing area, the loader bucket hit a rock in the floor of the landing area (toe in the muck pile). The sudden impact caused the employees jaw to close quickly and abruptly breaking a tooth.
Grinder cut to left fore arm requiring 8 stitches. NOTE this was just reported to me. The supervisor listed this as first aid treatment. The only medical bill sent listed exam. The employee is no longer employed with us. No one mentioned any stitches until today.
Walking through chain hanger section of kiln, got foot hung up between hangers and fell striking head on a hanger.
Striking a pin with a 20 lb. hammer. Pin diverted hitting finger between pin and hammer handle.

Employee was measuring Bulk Silos, stepped over the transfer line and came down on his left foot turning his ankle causing a fracture to the fibula.

Employee was welding in a kiln, when a chain hit him on the side of the head. Employee was wearing a hard hat and safety shield.
Jack hammering castable at 295 foot section of cement kiln. A piece of coating fell from above & struck left thigh causing him to fall.
Employee was struck by a drill steel (12"x4-1/2") when it fell from a Gill Rock Beetle Drill. Employee suffered collapsed lung, fractured vertabrae, ribs and collar bone.

Employee using a battery operated concrete grinder when grinder struck a burr in the concrete causing the grinder to jump and lacerated employee's left index finger requiring 8 stitches.
Was bending over obtaining sample, felt a burning sensation in R lower back.
Foreign object in eye, requiring it to be removed.
Slip and fall same level, jammed right shoulder, continued working. Surgery on 2/11/2011 on right shoulder will restrict duties.
A slide gate door was dropped onto the left ring finger, causing a fracture to the finger.
While shutting South, center cooler door, employee pinched right index finger, cutting finger that required 5 sutures to close.

While pulling a rope attached to a bucket filled with castable refractory, the rope slipped and when caught, pulled on the right shoulder.

Hearing loss of 25.667 found in right ear after audiological exam.
Employee was helping to lift a metal plate. While he was setting the plate down, the metal plate slipped, cutting the employee's finger.
Person pushing wheelbarrow when it tipped over resulting in strain to right shoulder. Person returned to regular duties 10/29/2008. On 03/02/2009 person missed work as a result of surgery on shoulder.
While cutting conveyor belt to make a splice, knife slipped and went into his right knee.

While tightning 2" bolts on a top plate of the primary crusher using a 24" pipe wrench and a 10 lb. hammer employee strained his lower back. Employee was in a bent over position while performing the work.
When the kiln was turned and water came in contact with hot kiln chains and materials, a sudden positive pressure was created throughout the kiln system, ejecting steam and hot dust through an open door on the drag conveyor, causing the EE to be burned.
EE was greasing 773 Dump Truck, got his feet tangled in the hose and fell to the ground, injuring his left shoulder.
Slipped and fell on steps going down into packhouse basement. Hit his right elbow and shoulder.

Employee pulled muscle at ribs across center of chest as a result of lifting large tire.
As the employee lifted materials onto a shaft rack, his foot slipped on a piece of wood that was at the bottom of the rack. In his attempt to break his fall, he strained his left shoulder.
EE sustained internal injuries when forklift He was operating struck a post. EE's chest/abdomen then struck the steering wheel causing internal trauma to torso.
Employee bent down to pick up hose and sharp pain occurred in lower back.

EE WAS GRINDING ON A PIECE OF CHANNEL IRON WHEN THE PIECE SLIPPED OFF THE WHEEL OF BENCH GRINDER ALLOWING LEFT HAND TO MAKE CONTACT WITH GRINDER.
While employee was climbing down a ladder from a yard loader, his foot slipped from the ladder, causing him to strain his right arm and shoulder.
DURING ANNUAL MAINTENANCE, EMPLOYEE WAS WORKING IN COOLER. HE WAS GOING TO FILL HEATERS AND LOST FOOTING TRYING TO CROSS ACETYLENE LINES ON FLOOR. HE TRIPPED AND HIT KNEE ON UPPER LEDGE OF NEX T GRATE.
EE WAS TAKING COVERS OFF AIR SLIDE SEPARATOR. BOLTS WERE OUT. WHILE PRYIG ON IT TO LOOSEN RTV IT FELLOFF TO THE SIDE & THEN FELL & SMASHED THE BACK OF EE'S LEFT FOOT.
