TIMBERS HAD BEEN LOADED WITH A FORK LIFT & TAKEN UNDERGROUND. THE EMPLOYEE WAS IN THE PROCESS OF SETTING TIMBERS; HE TOOK OFF HIS GLOVE TO GET A PIECE OF CHALK OUT OF HIS POCKET & PLACED HIS HAND ON THE TIMBERS. A SNAKE (COPPER-HEAD) HAD BEEN BROUGHT INSIDE WITH THE TIMBERS & THE SNAKE BIT THE EE ON THE LEFT INDEX FINGER. THE EE WAS TAKEN TO A MEDICAL CENTER FOR TREATMENT.

HE FAILED TO GAIN OPERATOR'S ATTENTION, WHEN CABLE SLACK WAS PULLED OUT, THE CABLE STRUCK HIM IN THE LEFT CALF AND KNEE. HE CONTINUED TO WORK UNTIL MARCH 6 AT WHICH TIME HE HAD AN MRI.

EMPLOYEE WAS INSTALLING A ROOF BOLT AND ACCIDENTLY STRUCK THE BOOM LEVER CAUSING IT TO DROP DOWNOFF THE BOLT. HE THOUGHT THE BOLT WAS GOING TO SET UP AND HE USED HIS RIGHT HAND TO MOVE THE BOLT OVER FOR ALIGNMENT. AT THE SAME TIME HE APPLIED UP PRESSURE TO THE DRILL POD AND HIS RIGHT MIDDLE FINGER WAS CAUGHT BETWEEN THE PLATE AND ROOF. THE TIP OF FINGER BEHIND NAIL WAS SEVERED.
