The injured was trying to roll a rock off the haul road and strained his back.

While hauling overburden, the injured was backing up for the shovel to load him and the shovel bucket hit the bed of his end dump truck, jarring and straining the injured's neck. He missed one full shift and went home early during another shift.

While steam cleaning a piece of equipment, the injured accidently squeezed the trigger of the wand and hit the right side of his right foot. The outside of the foot received 1st and 2nd degree burns.

AN EE'S PICKUP SHORTED OUT & CAUGHT FIRE UNDER THE HOOD WHILE PARKED IN THE EE'S PARKING LOT. NO ONE WAS AROUND, BUT A MECHANIC APPROX 300' AWAY WHO HEARD IT WHEN A TIRE BLEW OUT. THE FIRE PRE VENTED THE HOOD FROM BEING OPENED & 4 FIRE EXTINGUISHERS COULD NOT REACH THE BASE & PUT IT OUT. FIRE BURNED LESS THAN 45 MINUTES. NO OTHER DAMAGE OR INJURIES INVOLVED.

WHILE AUGERING HOLE #1054 AT 360' DEEP THE AUGER EITHER INTERSECTED THE PREVIOUS HOLE OR A VOID ALLOWED OXYGEN TO ENTER THE HOLE. A METHANE GAS FIRE WAS TRIGGERED AND BURNED LESS THAN 4 MINUTE S TOTAL AT THE BROW OF THE HOLE. THE FIRE WAS EXTINGUISHED WITH A WATER HOSE AND DILUTION IN THE AIR. THERE WERE NO INJURIES OR EQUIPMENT DAMAGE.

THE INJURED WAS HELPING OPEN A PREVIOUSLY MINED HOLE THAT HAD ACCIDENTLY BEEN PLUGGED. THE METHANE BUILDUP AT THE ENTRANCE IGNITED WHEN A SMALL HOLE WAS OPENED AND THE GAS WAS BLOWN DOWN WIND TO THE INERT GAS BURNER CAUSING FIRST AND SECONDDEGREE BURNS TO THE INJURED'S FACE, ARMS, AND HANDS. FIRE BURNED OUT IN LESS THAN 60 SECONDS.

TO DETERMINE IF THEY HAD POSSIBLY INTERSECTED THE PREVIOUS HOLE OR GOTTEN INOT THE ROOF OR FLOOR, THEY HAD TO OPEN THE PREVIOUS HOLE TO SEE AND LISTEN. THIS HOLE HAD BEEN CORKED BY A BULLDOZER WHILE CLEANING UP. WHEN OPENED, ACCUMULATED GAS IGNITED ON THE AUGER'S INERT GAS BURNER & BURNED < 60 SEC.
