Employee was working on an electrical unit was electrocuted.

EE was moving a barrel that had liquid and material in it and went to move it and tweaked lower back. Now when moving it side to side it has sharp pains.
Crusher was plugged and we were try unplugging it and I was on the crusher floor stabbing the side of the crusher with a pry bar to knock the rock down and while doing it my right wrist popped. I now have a sharp pain in my wrist, hard to lift anything and rotate. I did not stash it and it is not swollen just hurts when I move it
EE was cleaning by W-11 on the stand-up skid steer. EE went to back drag a pile and hit forward instead of reverse and hit the pile and pinned EE's hand under the conveyor frame to the top of the skid steer handle.
Prying a rock out and cutting EE's finger and got stitches
One employee threw a shovel up to another employee cutting the catchers hand.
Employee operating the unit, was struck with a come-a-long on their right hand due to being in the line of fire.

Employee reported 9/23/15 that he hurt his back 9/22/15 installing new shoes on the VSI. He refused medical treatment until 9/24/15.
Employee was shoveling off the F4 conveyor when his lower back locked up.
Employee was reaching for the handle on his loader when he struck his hand against it instead, causing it to bruise.
Rigger failed to pay attention to details of task. When trailer attachment moved, the employee slipped and fell. Falling 9' shock absorber deployed. The employee did not hit the ground or any other object. Employee was bruised and sore.
After checking the oil on her haul truck, she went to unlatch the hood and it fell on her right foot, bruising it.
Employee states he injured his upper back and neck while operating a haul truck. He did not seek medical attention until 4/13/15.
Employee was walking out of the warehouse when he slipped on the ice hurting his left hip, back, and right elbow. He didn't seek treatment until 2/25/15, where he was placed on restricted duty.

EE was attaching a cable on the fluid drive when the guard slipped, cutting his finger.
An employee was working on a drive belt on a screen unit in the mill. His back started to hurt later and into the next day.

During a belt splice, employee got between return and feed and used his back to push up on the feed resulting in injury.
Pulling breaker out of unused breaker box, fuse came down and hit L index finger
EE alleges that he was pulling himself into a truck and hurt his shoulder. He had predicted a few days before this that he was going to claim a shoulder injury.
EE fell off the ladder of his haul truck. He landed on the ground, fracturing both feet.

Employee was getting out of a stockpile loader and hit his knee on the door frame. We filled this out online on 11/18/2010 and reported on our 4th Quarter report. It wasn't until today that we realized that we had not pushed "Submit This Form to MSHA" after completing online. Sorry for delay.
Employee was lifting a back plate and strained his right arm. He did not seek medical attention and began losing time 12 August 2010.
Not a Dolese employee. Customer suffered apparent heart attack in his truck near Scale House. Customer died enroute to hospital.
EE was moving and oil barrel and felt pain in his lower abdomen.

Injured hurt his back while digging holes on a shot with a shovel.
Hot embers from fire two days ago blew across yard at mine and ignited cedar trees and a pile of cross ties. We reported again because fire was not extinguished within 30 minutes of discovery.
Mine neighbor had a controlled burn that he lost control of. Fire spread across line to mine property where a stack of junk loader tires caught on fire. Fire took more than 30 minutes after time of discovery to extinguish.

Employee was working with other employees to get a stuck tooth shank out of the 42 crusher. The tooth struck his chest.
EE was dragging a water hose. He slipped in mud and fell, striking his left hand on an I-beam.
Employee was using an extendalift to try to pull a post out of the ground. It came loose and struck his head. Four stitches were required.
Injury was carrying steel plates. He tripped on an extension cord and smashed his left hand between the plates and a work bench.
EE picked up a cable to hook to overhead crane. An I-beam rolled and fell, striking EE in the back of the head. Sutures were required.
Employee was cutting belt for the Qr2 conveyor. The blade slipped and cut his left arm. Stitches were required
EE was shoveling in a tunnel. His shovel hit a nail and injured his left wrist. He did not begin working restricted duty until 1 May 2008.
Employee was helping clean rock off a conveyor and strained his back.
EE was cutting belt with a box blade knife. The knife slipped and cut his right thigh. No lost time, no restricted duty but three sutures were required.

Employee strained his lower back and abdomen while lifting drive-line for water tank.
Employee was moving a troughing roller. The roller twisted and smashed his left index finger. Stitches were required.
EE was lifting a cable sling to hook onto a loader and strained his lower back.
EE's back started hurting after helping to load a flatbed truck with 50 lb. bags of rock samples and driving a flatbed truck. He did not seek medical attention until 7 June 2007 and began working light duty 8 June 2007.
Employee was a passenger in a mechanic truck when a loader backed off the ramp and struck the side of the mechanic truck. Employee's knee and back were injured. He is still working resitrcted duty.

EE strained his back while removing a rock sample from the scales.
EE was lifting and moving parts in the shop when he strained his lower back.
Employee was climbing into his loader to start his shift. When he pulled on the door, he lost his balance and fell to the ground. His right hand, head, and left hip were injured. He is losing time.
Employee was working with other employees to repair the brakes on a Caterpillar 980 loader. A spring pin was stuck. He was looking at it when it came loose and struck his forehead. He is working light duty and stitches were required.
EE reported that he felt pain in his shoulders after removing a piece of concrete from the bottom hopper at the wash plant. He did not seek medical attention until 14 July 2006. His first day off work was 14 July 2006.
Employee was using a magnetic drill to drill holes in a thick metal plate. While he was removing the metal shavings from the hole, the index finger on his left hand was cut. EE received two sutures and began working restricted duty 03/30/06
Employee was measuring screens in the screen rack. A screen slipped and mashed the little finger on his right hand. Employee began working restricted duty on 29 March 2006 but his job duties as a supervisor were not really affected.
EE, was getting into a water truck and slipped and fell, injuring his left arm and left leg.
Employee was backing a stockpile truck. He was looking behind him and turning steering wheel and felt pain in his back. He is working restricted duty.
Employee was fueling his loader at the fuel island and strained his right arm. He did not seek medical attention until 9 August 2006 and began working restricted duty 10 August 2006.
Employee was moving a metal cut-off saw in the truck shop and felt a twinge in his abdomen. Happened sometime after midnight. Not sure of time.

This is not a work-related injury, not covered by workers' comp but it did happen in the employee parking lot before Employee and co-worker clocked in. Employee was walking in the employee parking lot and was struck by co-workers personal vehicle. Right arm and right leg were bruised and strained. Employee missed work 8 & 9 Dec and returned 12 Dec to regular duties.
Employee was moving a piece of metal plate and strained his back and right shoulder.
Employee was operating a Caterpillar 980 loader loading rip rap when a rock fell off the bucket. He did not notice the rock and backed over it, injuring his back and left shoulder.
EE was carrying a sack of floor absorbent up the stairs. His right knee gave out and he struck his knee on the stairs.
Employee had opened a metal container of welding rods. When he reached inside the container, he cut his left palm on the metal pop top lid. There was no lost time and no restricted duty but stitches were required.
Employee was exiting a lift basket on the electric truck. He sprained his left ankle as he stepped to the ground. He was placed on restricted duty.
EE was operating a Caterpillar 980F loader. He reached over to turn on the heater fan and injured his left shoulder. EE did not begin losing time until 4/25/05.
EE was working with other employees to mount a tire on a Caterpillar 980 loader. While pushing on the tire to align the bolt holes, he strained his back.
EE was climbing onto a Caterpillar 980 loader. When he reached the top step, his foot slipped and he fell to the ground, landing in the 3" surge pile. He suffered cuts and bruises to his right knee and both hands.
EE was walking on a metal truss at the crusher building. There was frost on the truss and EE slipped and fell. He landed on a C-channel purlin.
EE WAS IN THE PROCESS OF CHANGING A 2" AIR SWIVELL, SLIPPED AND FELL APPROX 2' AND LANDED ON BACK
Employee strained his left shoulder while lining up I-beams on the crusher building to be welded.

Injured employee was working with another employee to install a conveyor. He was working below the other employee. The other employee cut off a piece of T-channel iron and as he was handing it down to injured ee, it slipped and fell approx. 1 foot and struck injured ee in the face, causing a laceration. There was no lost time - no light duty but sutures were required.
EE was working with other employees to pull sunatube hole liners from blast holes. He reported later that he had suffered a groin strain. He first sought medical attention on 11/10/04 and began working light duty at that time.
Employee was lowering a wear plate into a bin. A co-worker was operating the electric winch. The opening in the floor has a bi-fold door of 1/4" floor plate. Employee tripped on the door and fell into the wear plate. His struck the bridge of his nose on the plate. He is off work.
Employee stepped on a rock while doing a pre-operational inspection on a motor grader. He felt pain in his back but did not seek medical attention until 29 October 2004.
EE was greasing conveyor idlers with the belt moving. The grease hose was caught by the moving belt. The hose, gun, and Injured's left arm were pulled into the belt. She suffered 2nd and 3rd degree abrasion burns to her left arm. She is losing time.
EE was using a hydraulic bottle jack and a piece of 4" by 4" wood to straighten a dent in a dust control tube. Under pressure, the jack slipped and the 4x4 struck ee on the nose. His nose was fractured.
Crew was drilling in location where they couldn't quite level the drill. So when making steel connections the drillers helper was having to pull on the drill steel to get it to line up so they could make up. While he was pulling on the steel the driller lowered the steel to make the connection and at that time ee felt something pop in his right elbow.
Employee was walking down and incline and fell, spraining his right ankle. Employee is working restricted duty.
Employee was using a Caterpillar 980G loader to load a customer's trailer. While turning the steering wheel, his left elbow popped. The doctor diagnosed strained left elbow. He is working restricted duty.
Employee was walking on uneven ground and twisted his right knee in March 2004. He did not turn in a work-related injury but sought medical attention on his own through Indian clinics. His injury was determined to be work-related on 4 January 2005. He has been missing time since 13 September 2004.
EE WAS LYING ON THE G1 CONVEYER TAIL PULLEY CUTTING A ROLLER OFF THE CONVEYER. HIS RIBS WEREBRUISED.

EE WAS BENT OVER USING A CUTTING TORCH TO CUT A PIECE OF I-BEAM. WHEN HE STRAIGHTENED UP, HE FELT A SHARP PAIN IN THE MIDDLE OF HIS BACK. HIS BACK WAS STRAINED. HE MISSED ONE DAY OF WORK.
EE WAS USING A KNIFE TO CUT A WELDING LEAD TO INSTALL A CONNECTOR ON THE END OF IT. THE KNIFE SLIPPED AND CUT HIS LEFT THUMB. THERE WAS NO LOST TIME, BUT SUTURES WERE REQUIRED AND EE WORKED FI VE DAYS OF RESTRICTED DUTY.
EE HAD BEEN WELDING AT THE CONSTRUCTION SITE ALL DAY AND SUFFERED SOME KIND OF ALLERGIC REACTION WHICH CAUSED SWELLING IN HIS MOUTH AND RIGHT EYE. IT WAS NEVER DETERMINED WHAT HE HAD REACTED TO. THE EMERGENCY ROOM DOCTOR TOOK HIM OFF WORK FOR 10/30/2003 AND HE TOOK OFF WORK ON HIS OWN FOR 10/31/2003.
EE WAS LOADING A CABLE SLING ONTO A WELDING TRUCK. HE TWISTED AND STRAINED HIS BACK. HE IS WORKING RESTRICTED DUTY.
EE WAS LOADING REBAR ONTO A FORKLIFT. WHILE PULLING ON SOME OF THE PIECES TO GET THE LOAD BALANCED, HE EXPERIENCED BURNING PAIN IN HIM RT. ARM & SHOULDER. EE IS WORKING RESTRICTED DUTY.
EE WAS WORKING WITH OTHER EE TO LIFT A METAL COLUMN INTO PLACE AT THE CONSTRUCTION SITE. THE NEXT DAY HE REPORTED THAT HE HAD FELT A TWINGE IN HIS BACK WHILE LIFTING.
EE WAS CARRYING A THREE-TON RATCHET CHAIN HOIST FROM THE TOP FLOOR OF THE MILL BLDG. TO THE GROUND LEVEL WHEN HE FELT A PAIN IN HIS LOWER BACK.
EMPLOYEE WAS WORKING WITH OTHERS AT THE CONSTRUCTION SITE. THEY WERE MOVING REBAR TOWERS, CONSTRUCTED OF 8 PIECES OF 4-FOOT #8 REBAR TIED TOGETHER IN A SQUARE. HE INJURED HIS LEFT SHOULDER.
EE WAS WORKING WITH OTHER EE TO INSTALL HANDRAILS AT THE CONSTRUCTION SITE. HE ATTEMPTED TO MOVE A HANDRAIL WITHOUT ASSISTANCE AND STRAINED HIS BACK. EE DID NOT SEEK MEDICAL ATTENTION UNTIL 8 AUG 03
EE WAS STANDING ON A LADDER ATTEMPTING TO HOOK UP A COME-A-LONG. HE WAS HOLDING THE CHAIN WHEN THE COME-A-LONG FELL OFF THE LADDER, HIS LEFT ELBOW WAS INJURED. THE INJURY DID NOT BECOME REPORT ABLE UNTIL 9-16-03 WHEN HE DECIDED TO SEEK MEDICAL ATTENTION.
EE WAS WORKING WITH OTHER EE'S TO INSTALL HARD PLATE LINERS IN BINS AT THE CONSTRUCTION SITE. HE WAS IN THE BOTTOM OF THE BIN WHEN AN OXYGEN BOTTLE FELL THROUGH THE ACCESS AT THE TOP OF THE BIN & STRUCK EE
EE WAS WORKING WITH OTHER EE TO MOVE STEEL PLATES BY HAND. HIS BACK BEGAN TO HURT. ON 2 JUNE 2003, HE SAW A DR. AND WAS PLACED ON RESTRICTED DUTY.
