The employee was using a large hammer and striking wrench to loosen the locking nut on a crusher mantle. The employee stopped to steady themselves and put a hand on the edge of the wrench. A burr had been raised on the wrench edge due to the hammer blows, which cut through the employee's glove and into their left palm.

The employee's left shoulder popped out of joint while climbing down into a jaw crusher to perform maintenance.
While changing V-belts on the #2 shaker screen, a come along was being used to lift the motor base. During this action, the tension spring rod broke, causing the spring to fall out and strike the employee on their right knee.
Employee was performing maintenance on a conveyor. Sunny, temps in the 90s, and high humidity. The employee had been taking breaks, and water and popsicles were available. EE reported feeling strange, lightheaded, experienced cramps in their arms, and vomited. Employee had been sick previous weekend, later confusion if issues were related to heat or previous illness.
While pulling on a piece of replacement rubber belt, the employee's foot was struck by the wooden piece connected to the rubber. The wood hit their foot, bruising it and caused them to step backwards awkwardly. Self-care was advised by care coordinator, but symptoms didn't resolve. On 4/19/24, employee was taken for additional treatment and received work restrictions.

EE CAME TO US SAYING HE HAD SURGERY SET UP BY DR IN MACON AND THAT DR HAD TOLD HIM HIS CONDITION WAS JOB RELATED. HE TOLD US HE HAD NUMBNESS IN HIS HANDS. WE TOOK HIM TO OUR COMPANY DR.

EE NOTICED A FIRE ON THE PIT LOADER. HE WAS UNSUCCESSFUL IN GETTING THE LOADER OPERATORS. ATTENTION. IN A HURRY, EE DISMOUNTED HIS HAULPAK TO GO TO THE AID OF THE LOADER OPERATOR. WHILE DOING SO, EE MISSED THE BOTTOM STEP WITH HIS RIGHT FOOT AND HIS RIGHT KNEE HIT THE TOP PORTION OF THE BUMPER.
HE WAS REMOVING A BROKEN HANDLE OUT OF A HAMMER WITH AN ALLEN WRENCH. HE MADE A MISS LICK AND HIT HIS INDEX FINGER, BREAKING IT.
INJURED WAS HELPING REPAIR 4 1/4' CRUSHER BY REMOVING 4 BOLTS THAT HOLD IN LINER. BACKING MATERIAL HAD SEEPED IN AROUND BOLTS. TO BRAKE THE MATERIAL OUT HE USING A 4' RR BAR. HE MISSED TARG ET AREA HITTING THE SIDE OF THE CRUSHER AND BAR BOUNCED OFF CRUSHER HITTING HIS LEFT FOOT RIGHT ABOVE THE STEEL TOE IN HIS BOOT, BRAKING HIS LITTLE TOE.
EE WAS TRYING TO LIFT THE TONGE OF A TRAILOR WITH HEAVY EQUIPMENT IN THE TRAILER SUCH AS A GENERATOR MOTOR & FUEL TANK HE PULLED MUSCLE IN GROIN AREA
I.NAME IN THE PIT LOADING TRUCKS. DIDNT HAVE A TRUCK,WAS SWEEPING FLOOR AREA OF PIT.LOADER BUCKET CAME IN DIRECT CONTACT WITH HIGH BOTTOM IN FLOOR CAUSING A SUDDEN JAR TO LOADER BRINGING LOADE R TO A COMPLETE STOP JARRING I.NAME NECK.
HE WAS TAKING OFF OLD SHEEY METAL OFF OF A 980-C ALSO STRAIGHTENING BRACKETS WITH HAMMER AND CHAIN HOIST AND REPLACING NEW SHEET METAL. AROUND 3:00 PM HIS NECK STARTED HURTING WHILE HE WAS LIF TING SHEET METAL.
INJURED WAS HELPING CHANGE SCREENS ON TOP DECK #2 TOWER TWISTING MOTION WHILE LIFTING. PULLED MUSCLE IN LOWER BACK. SCREENS WEIGHT WAS 225 LBS. CHANGING WAS DONE ENTIRLY BY HAND. INJURED I NITIALLY WEIGHT WAS 225 LB.S. CHANGING WAS DONE ENTIRELY BY HAND. INJURED FELT PAIN IN LOWER BACK BUT DID NOT REQUEST TO GO TO DOCTOR UNTIL THE NEXT DAY.

INJURED WAS ON ALL FOURS ATTACHING A CHAIN ONTO A GRAVITY TAKE-UP PULLEY WHILE OTHER WAS ABOVE ON THE CATWALK REMOVING A GUARD SO THAT HE COULD ATTACH THE OTHER END OF THE CHAIN AND COME-ALONG S TO LIFT THE DRUM. WHEN THE GUARD WAS REMOVED, A 5-LB ROCK FELL FROM UNDERNEATH THE GUARD AND HIT THE INJURED ON THE LOWER BACK. DOCTOR DIAGNOSED INJURY AND RECOMMENDED DAYS OFF FOR TREATMEN

EMPLOYEE WAS DRILLING HOLES IN BOULDERS AND LOST FOOTING, FELL ON ROCK. EMPLOYEE FELL APPROX. THREE FEET ONTO HIS BUTTOCKS AND DID NOT COMPLAIN OF ANY DISCOMFORT UNTIL NEXT DAY. EMPLOYEE WAS T AKEN TO COMPANY DOCTOR, AND WAS PUT ON LIGHT DUTY AND MILD PAIN PILLS UNTIL RELEASED.
