A laborer working in the Finishing Mills was cleaning a gypsum spillage. The employee was filling and carrying two five-gallon buckets; the employee stated their back was hurting and had trouble standing and walking with a sharp pain. They were treated for a back spasms.

Lunchtime arrived, the EPSA team stopped activities. However, the CEMEX owned loader continued to operate. After lunch activities resumed. The dumper operator while performing the reverse maneuver to unload material at the stockpile, backed to close to the berm and suddenly experienced a slide on the rear of the dumper, falling to a lower level.
Lack of communication between operator and ground rigger, pinched rigger's fingers between load causing laceration to fingers. Resulting in stitches and temporary light duty work.
Burn to ear drum from hot slag from oxy/acetylene cutting.
Employee finger was struck with hammer fracturing tip of right middle finger.
While closing a gate hopper on railcar, the bar used to turn the knuckle slipped from the knuckle and pinched the employees left middle ring finger between the bar and the railcar.
The employee was working off of a scissors lift to make a repair. At the same time another employee left a forklift running with no wheel chocks parked on a slope. The forklift rolled back down the slope and collided with the scissors lift. The employee was pinned between the handrail of the SL and air slide the employee was working on.
While working in the bulk silos, the miner got cement dust in their right eye. The miner flushed their eye but still had some irritation. Miner was taken to local urgent care where miner was prescribed an ointment which is what makes this case more than first aid.

Team lift of a purlin being performed when wind gust caused one employee to lose control of the purlin, which rolled onto right wrist lacerating it. Leather gloves & long sleeves were being worn; proper team lift techniques were followed. Bleeding was slowed and employee transported to ER for treatment - 10 stitches and a tetanus shot. Released with no work restrictions.

Miner was working to change out a bushing on a tipping valve arm, when the arm rotated down and pinched miner's left middle finger between the counter-weight arm and d ring which was welded to the feed pipe.

Kiln Auxiliary clutch housing fragmented striking employee on left shoulder blade resulting in a contusion and fractured rib.
Operator error resulting in accident while hauling overburden to stockpile. Operator failed to wear seatbelt, causing injury. Although operator displayed high proficiency and had driven this exact route 49 trips this same workday, 50th trip was attempted at excessive speed and caused this accident. Non-use of mandatory seat belt caused EE's injury. Operator relieved of mine duty.
Employee was descending HAC (high altitude conveyor) belt and lost footing and lunged forward and felt pain in right shoulder. Employee sought medical treatment on 4/29/2020 and MRI showed tear of right pectoral muscle. Employee out for corrective surgery on 5/1/2020.

Injured was reattaching a valve arm to yoke on an actuating cylinder. In the process of getting yoke and valve arm aligned EE used a hammer to drive keeper pin in place. The pin went through the yoke and valve arm and pinched the end of left index finger between the end of pin and valve housing. Resulting in a Laceration/fracture on the tip of the index finger.

Injured was pulling the slack out of load tarp, and when injured stepped backward off of the end of trailer injured was wearing fall protection - when harness stopped injured this caused left foot to contact ground (or weight caused toe to break)
Employee's pry bar slipped letting right arm travel forward into cement panel. Right forearm was cut above glove where contact was made with concrete panel. Laceration required five (5) stitches.

Injured was walking down the steps from control. Close to bottom of the steps his foot slipped and he injured his right ankle. After treatment, the injury was diagnosed as broken fibula at ankle.

Injured was closing cleanout door on preheater. The door latch was not in the correct position and when injured reached to reposition the latch, his finger was mashed between door and frame.
Employee was descending steps from the new heat exchanger system and when they stepped on the landing they felt a pain in the right knee. Employee sought medical treatment on 7/20/2015 and was prescribed pain and steroid medication.

Employee was welding in the baghouse. During the latter part of his shift he began to have eye discomfort and irritation which was diagnosed as arc flash eye burn.
Injured was cutting a piece of channel iron from #6 clinker bin. He removed the cut piece, and then reached into opening to remove the rest of the iron. The iron become loose and fell onto employee's left thumb. This resulted in a fracture.
Injured was walking, blacked out and fell to the concrete. Injured sustained lacerations to area above left eye. This required sutures
EE was using a screwdriver to "pop" some metal bands on a shipping crate. The screwdriver slipped causing his hand to contact a metal band on the crate. This resulted in sutures to his right hand small and ring fingers.

Injured had completed a power shot in preheater poke hole and had removed the system. He proceeded to look into the porthole to see how much buildup up had been removed and at that time some material overhead broke free and hot dust was blown onto injured's face and some material got into eyes

Injured was installing a door on #5 finish mill feed elevator. This door is secured with a top retaining bar. One side of the retainer was secured and the other side could not be because the door was not in correct position. Injured moved door and this caused the unsecured side of bar to swing down causing it to contact his left thumb. This resulted in a fracture.
Injured was exiting the pack house silos on the south side. His left foot did not clear the concrete blocks (used to stop water from entering these silos) and he fell to the ground injuring his left knee (torn ligament)

Employee was closing door on a new piece of production equipment and smashed end of right middle finger causing cut and fracture. Finger was caught between door latch and latch keeper. Employee was placed on limited duty with using right hand.
Injured was using an air lance to clean a plugged chute. He was standing on an 10" high platform. When the air lance broke free, injured lost his balance, fell backward and his left elbow contacted a handrail Injured did NOT seek medical treatment until 11/19/2012
Electrical contractor dropped a piece of 3/4 rigid conduit pipe approx. 4' long from a walk platform above a fuel silo. The pipe slid down the roof of silo and fell approx. 20' and hit the EE point down between his back and shoulder blade. The EE was working in a boom lift appr. 70' from ground level. EEG was diag.with fractured ribs. EE was released for work upon leaving doctor.
Injured was adjusting the calibration on a motion switch for H-50 dust collector screw. He accessed the switch from the wrong side of the guard and while descending the ladder his left thumb contacted a sharp edge on the guard.
Stepped out of truck onto a rock wrong way and felt a pain.
Injured was using a come-a-long to pull two sections of chain together on the south side of reclaimer harrow bars. When he put pressure on the hoist, a section of harrow bar above him fell and struck his back

This was a contractor employee who was observed by his fellow workers, gasping for air and breathing heavily. After making the employee comfortable as possible, an ambulance was called and he was transported to local hospital
Injured was installing a cover on a roller mill roll. The method used was putting an eyebolt into cover and lowering into place. A fellow co-worker realized that the eyebolt had broken and pushed injured out of the way. Injured was pushed into another roller mill roll and sustained contusions to right shoulder.

EE was installing a machine guard approx. 75 lbs. on a piece of Eq. on 4/12/11. EE's feet slipped on a hill of dust and he overextended his shoulder.Due to continued pain on 4/25/11 company made a Dr. appt. for him the EE RTW the next day. EE also received 2 Rx's and was waiting on X-rays when we met with him on 4/27/11 for the incident investigation. (See attachment)
Injured was trying to move a pallet and his feet got tangled in the plastic wrap at the bottom of pallet. Injured fell and hit left elbow Note: Injured did not seek medical treatment until 4/25/2011.

Injured was instructed to pour grinding aid into the feed chute on #6 finish mill. He poured it thru an access door. After pouring material into chute, he closed door and a flashback occurred. Inujured suffered 2nd degree burns on right side of neck and right arm. He also suffered a fractured little finger on right hand.
Employee was dismounting an L-50 front end loader. After reaching the ground, he apparently stepped into a depression in the ground and caused a sprain to his left ankle.
Injured was using a pickaxe to remove hardened material from roller mill table when he felt a pain to his lower back.
Injured was walking in the cooler room when he tripped on a bolt sticking out of floor. Injured threw his arms up to regain his balance and felt a pain in his left shoulder. Injured went to doctor on 6/10, but he was NOT put on restricted duty until 7/1/2010.
Injured was removing a keeper pin from the main chain on the reclaimer. When keeper pin was being removed, the injured contacted a bucket above his hand, causing a laceration to his right index finger.

Injured was working at additives tower and stamler additives feeder during this time period. Unknown what caused this injury, but it was related to overexertion during this period. Injury reported on 4/9/2010 but injured did not seek medical treatment until 4/12/2010. Diagnosed with Lumbar strain.
Injured was operating quarry control panel when he felt a pain in his lower back. Injured was subsequently sent to company doctor on 2/24/10 and was put on restricted duty. No known conditions contributed illness.

Employee was using a jackhammer and after use he noticed a pain in side and shoulder. Injured did not seek treatment until 10/6/09
Injured was moving some water pump hoses that had become twisted. The hoses were 8" diameter. When injured was moving these hoses he felt a pull in his groin area. Injured did not seek medical treatment until 9/29/09.
Injured was trying to retrieve an impact wrench from golf cart bed. In the bed was a broken shaft assembly that he had helped remove from reclaimer. While retrieving impact wrench, the shaft fell out of cart bed and the coupling end fell onto the floor and the other end fell on injured's left foot.
Employee was using vac. truck to clean loose kiln feed from top of homogenizing silos. Employee was holding the hose under his left arm and his right hand was on the end of hose. Something caused the right hand to slip and his right arm was sucked into end of hose, up to his shoulder.
Employee was working on electrical wires on the trolley for Clinker Crane. Employee tripped on steel support beam and to prevent himself from falling, he caught himself with right arm. Employee sustained a strain to right shoulder. Employee did NOT seek medical treatment until 7/13/09.
Injured was using a pry bar at the reclaimer to locate harrow chain into guides. The pry bar slipped, forcing the wrist to become twisted abnormally.

Injured was removing burner pipe from kiln. He was wearing goggles and when he removed goggles dust fell from the goggles into his right eye.
Injured was helping move rail cars and was standing on the ladder on the side of the rail car. When the cars were positioned, he reached around with his left hand to tighten the rail car brake. He strained a muscle in his left shoulder.
Employee was attempting to open a dust collector damper while the fan was still operating. This heavy resistance caused employee to sprain his right shoulder.
Injured was unhooking a 4-part crane block from a single crane block. During this, injured placed his hand on 4-part block and the hook, swiveled side to side and the injured's finger was pinched. Injury resulted in sutures.
Injured was doing some testing on palletizer and put his hand in the wrong place. He was adjusting the slide gate and put his left hand onto some moving parts. Injured left ring finger was caught.
Injured was using a manual rail car mover to move an empty cement car. Injured did not use both hands on the mover and the mover slipped, causing the forehead of the injured to contact the brake platform on the rail car.
Employee was dismantling a scaffold when a 7' scaffold post hit his knee and pushed his leg out.
Employee was walking on the west side of kiln coal mill and slipped on water and mud in this area. Recent rains had caused this area to acculumate water. Employee experienced sprain of left knee area.
Injured was descending steps on east side of preheater and slipped in some standing water(recent heavy rains) employee fell and twisted left knee. Injured did not seek treatment until 2/6/08

Employee was working inside discharge cone on finish mill. EE was using wrenches to work on water spray pipe. EE reported injury at this time, but he was not sent for medical treatment until 12/4/07. After medical treatment, EE was diagnosed with sprain in lumbar region.
EE was moving hot material from the kiln feed end chute with a skid steer loader. EE apparently hit the discharge chute and some hot material fell from overhead on his body. EE jumped out of loader and contacted some hot material that had not been moved.
EE was moving hot material from the kiln feed end chute with a skid steer loader. EE was dumping this material into a 980 loader bucket and some hot material splashed back and contacted EE. EE was burned on his hands, arms and face.
Employee was using (2) jacks to position kiln tire pads. One of the jacks slipped and EE's left index finger was pinched between tire pad and retaining block. EE failed to properly secure jack and failed to keep his fingers away from potential pinch points. Injury resulted in a pinched finger/fracture and required stitches to close wound.
Injured was using a poke bar to remove buildup from preheater duct under the tire chute. The tire feeder fed a tire into the chute and tire hit the poke bar causing the end to be forced down. The other end of the poke bar which was being held by injured was forced upward striking the employee in the jaw. Injured had four stitches inside mouth

Employee was helping to lift a crane brake rigging from work table to a pallet, as the brake rigging was being lowered, the adjusting rod for the rigging, put tension on the spring and the employee's right thumb was pinched. Employee required several stitches to close wound.
Employee was standing on a chute flange, working on a door on chute, employee slipped and fell approx. four (4) feet and his ribcage hit the chute flange

Employee was working on valve that withdraws cement from silo 9. When employee stepped back, his left foot stepped on guard covering opening in floor. His foot twisted causing his ankle to twist.
Employee was adjusting reject plow on conveyor belt. The plow needed more adjustment and fellow employee activated the plow to determine adjustment needed. Injured employee put his left hand in a pinch point of linkage.
Injured went to bottom drag area to retrieve some air hose. Injured reached up into drag, which was being worked on, and a piece of iron that had previously been cut fell on his left arm.

INJURD STEPPED OVER ROTATING MOTOR COUPLING ON OVERHEAD CRANE. THE ROUGH SURFACE OF THE COUPLING CAUGHT HIS RIGHT PANTS LEG CAUSING LACERATIONS AND BRUISE TO RIGHT LOWER LEG.

INJURED WAS REPLACING NO. 6 FINISH MILL DOOR WHEN DOOR SLIPPED CAUSING HIS LEFT LARGE FINGER TO BE CAUGHT BETWEEN DOOR AND FRAME RESULTING IN LACERATION REQUIRING SUTURES.
INJURED WAS ROLLIGN AIR HOSE ONO HOSE REEL. HE FAILED TO NOTICE THAT A PIECE OF KEY STEEL HA DBEEN STOVED IN ALOCATION WHERE HIS HAND WOULD STRIK IT WHEN HE TRNE DHTE CRANK, AND CONSEQUENTLY HIS LEFT THUMB WAS STRUCK AGAINST THE KEY STEEL CAUSING A LACERATION REQUIRING 4 STITHCES.
INJURED WAS REMOVING 3/8" X 24" X 54" SECTION OF INCLINED CLINKER CHUTE COVER BY USING CUTTING TORCH TO REMOVE BOLTS. AFTER REMOVING LAST BOLT HE PRYED COVER OFF WITH SCREWDRIVER CAUSING IT TO SLIDE DOWN INCLINED CHUTE TOWARD HIM. WHEN HE SAW DIRECTION COVER WAS MOVING HE JUMPED OFF LADDER HE WAS WORKING FROM TO FLOOR 45" BELOW AND FRACTURED A SMALL BONE IN HIS LEFT FOOT ON IMPACT.
ATTEMPTS WERE BEING MADE TO REMOVE DESTRUCTION FROM 2ND STAGE PREHEATER VESSEL SO KILN FEED COULD PASS THROUGH. A 3" HOLE WAS CUT IN DISCHARGE PIPE BELOW TIPPING VALVE AND A 6" HOLE ABOVE TIPP ING VALVE. WHEN THE OBSTRUCTION WAS REMOVED, A SUDDEN FLOW OF MATERIAL EXITED THE VESSEL AND SOME OF THE HOT KILN FEED GUSHED OUT THE 3" HOLE AND ENGULFED TWO EMPLOYEES CAUSING 1ST & 2ND DEGR

INJURED WAS USING PRY BAR TO REMOVE BELT ROLLER WHILE STANDING ON CONDUIT. THE BAR SLIPPED CAUSING HIS FOOT TO SHIFT ON CONDUIT. EMPLOYEE FELL FORWARD AND SUDDEN MOVEMENT OF HIS FOOT CAUSED A CHIP FRACTURE (4X7 MILLIMETER) OF LEFT FOOT.
INJURED WAS CLIMBING LADDER WHICH HAD BEEN PLACED AGAINST RAIL COAL CAR. FRONT END LOADER PUSHED CAR 2' TO 3' WHICH CAUSED LADDER TO TIP OVER AND THREW INJURED TO GROUND APPROX 8' BELOW. WHE N HE FELL HE INJURED HIS LEFT LOWER BACK.

FELLOW EE WAS SUING SLEDGE HAMMER TO DRIVE OUT ROLLER MILL LINERS THE HAMMER GLANCED OFF THE LINER AND STRUCK INJURED S LEFT KNEE. LEFT KNEE BRUISED.

HE FAILED TO GET THE ATTENTION OF THE FORKLIFT OPERATOR AND MAKE HIS INTENTION KNOWN CAUSING HIS LEFT INDEX FINGER TO BE CAUGHT AND LACERATED.

THEY ALLOWED THEIR END TO FALL CAUSING END EE WAS HOLDING TO SLIP FROM HIS GRASP AND FALL ONTO HIS LEFT FOOT, STRIKING BEHIND STEEL TOE CAP AND FRACTURING HIS LEFT MIDDLE TOE.
