While changing forks out for the bucket on a CAT skid steer, the injured employee went to knock the pin up so the forks would release. When EE did, the forks came off the frame and landed on EE's left foot, breaking the big toe and causing a laceration

Employee claims that shoulder began to hurt after EE returned home from work. EE alleges that EE may have strained shoulder while using a sledge hammer to drive pins. EE has a slide sledge on truck and has been instructed not to use the sledge hammer. Employee did not report the injury until 1/16/2017.

At approximately 2:00 pm a group of teenagers trespassing on the property discovered and reported a deceased person on the mine property. This was called in to MSHA under the guidelines of "Immediately Reportable" incident. Investigations by local authorities identified the victim's residence and notified family. Medical examiners report identified cause of death as a stroke.

On 09/21, EE complained of pain again and was again taken for medical evaluation, this time, EE was placed on minor restrictions. EE was released to full duty-no restrictions on 09/28
