HE FELT PAIN IN HIS RIGHT ELBOW, WHICH HAD PREVISOULY BEEN INJU RED IN MAY 1994. HE DID NOT RECEIVE MEDICAL ATTENTION AT THE TIME OF THE INJURY. HE WENT TO A PHYSICIAN ON 12/8/94 WHO ADVISED HIM TO GO TO THERAPHY TREATMENTS FOR 4 WEEKS.

EMPLOYEE PASSED OUT AT HOME AFTER WORKSHIFT. EMPLOYEE WAS WELDING CULVERT JOINTS TOGETHER IN CONVEYOR TUNNEL, WHICH IS APPROXIMATELY 200' LONG. A FRAM WAS PROVIDED FOR VENTILATION. EMPLOYEE ALSO STATED THAT HE WELDED BRACES ON THE STAKER CONVEYOR. DR'S REPORTT STATED THAT THE CAUSE OF ILLNESS WAS UNKNOWN.
