IT IS THE UNCONFIRMED THEORY THAT EE WHILE WALKING AROUND THE BELT TAIL PIECE, CAME INTO CONTACT WITH THE FRAME OF THE TAIL PIECE STRUCTURE WHICH HAD BECOME ENERGIZED. THE EE WAS TRAVELING IN AN OPEN MINE JEEP. A WATER LINE WAS HANGING ACROSS THE MINE TRAKC, MAKING CLEARANCE LOW. HE FAILED TO LOWER HIS HEAD ENOUGH AND STRUCK THE WATER LINE WITH HIS HEAD.

EMPLOYEE WAS PULLING #195 SHILED IN BUT IT WAS IRON BOUND (STUCK) WHEN HE THOUGHT IT BROKE LOOSE THE SHIELD WAS SET ON PULL. BUT THE PAN PULLED BACK CATCHING HIS RIGHT FOOT BETWEEN THE PONTOON ON #196 SHIELD AND THE PAN. (ONE FOOT WAS ON THE PONTOON OTHER WAS ON BOTTOM OR FLOOR)
EMPLOYEE WAS UNHOOKING HIGHLINE TO DROP ON GROUND WHEN THE HIGHLINE FELL BEFORE HE WAS READY CAUSING A HIGHLINE HANGER TO PUNC IN HIS RIGHT HAND BETWEEN THE THUMB AND FOREFINGER.
EE WAS BUSTING UP SLATE IN THE PANLINE AT #3 SHIELD. WHEN HE MOVED A PIECE OF SLATE, ANOTHER PIECE FLIPPED DOWN, SMASHING HIS LITTLE FINGER ON THE RIGHT HAND. (HE MISSED ON DAY TO HAVE HIS FIN GER STITCHED).
EMPLOYEE WAS BUILDING A WALL FOR AN AIRLOCK DOOR(HE WAS USING OMEGA BLOCK OR FIBER BLOCK). AS HE PICKED UP A BLOCK FROM THE SUPPLY CAR HE FELT SOMETHING PULL IN HIS BACK. (EE WORKED UNTIL 11/ 13/01.)
EMPLOYEE WAS FORCING OPEN A COVER ON A MANTRIP HE HAD JUST REPAIRED. HE DID NOT REALIZE THE RUBBER LATCH STRAP WAS HOOKED. WHEN HE FORCED THE COVER UP THE HINGE END OF THE STRAP BROKE LOOSE ST RIKING HIM ON THE RIGHT TESTICLE WITH GREAT FORCE.
EMPLOYEE WAS WELDING ON THE TAIL END RANGING ARM WHEN A PIECE OF SLATE FELL FROM THE ROOF STRIKING A CO-WORKER ON THE SHOULDER THEN HITTING HIM ON THE LEFT KNEE. (WORKED UNTIL 3/18/02.)
AT 0430 A FIRE LIGHT WENT OFF ON 00R 3NE SECTION BELT. THE HOIST OPERATOR CALLED TWO PEOPLE TO INVESTIGATE. THIS SECTION WAS ABANDONED, NO ONE WAS ON THE SECTION. AT 0500 A ROOF FALL WAS FOUND IN THE #1 ENTRY AT THE 2030' MARKER.
A ROOF FALL WAS FOUND BY A FIREBOSS ON 8-13-01. THE ROOF FALL WAS FOUND IN THE RETURN OF THE 00 RIGHT CONTINUOUS MINER SECTION. THE SECTION HAS BEEN OUT OF PRODUCTION AND ABANDONED.
EMPLOYEE WAS WALKING DOWN THE STEPS IN THE SLOPE STEPPED ON A PIECE OF LOOSE COAL AND FELT A POP IN HIS RIGHT KNEE.
A ROOF FALL WAS FOUND BY A FIREBOSS ON 5-28-01. THE ROOF FALL WAS FOUND IN THE RETURN OF THE O RIGHT CONTINUOUS MINER SECTION. SEE DRAWING FOR FALL INFORMATION.
EMPLOYEE WAS CARRYING A BAG OF BLOCK BOND. HIS RIGHT FOOT CAUGHT ON A PLASTIC BAND CAUSING EMPLOYEE TO STUMBLE AND FALL. EMPLOYEE'S LEFT FOOT TWISTED WHEN HE FELL RESULTING IN STRAINED LIGAMEN TS AND A POSSIBLE CHIP FRACTURE IN HIS LEFT FOOT
EE AND 3 OTHER EES WERE CARRYING A RACK WHEEL TO THE SHEARER WHEN EE STRAINED MUSCLES IN HIS LOWER BACK.
AUDIOGRAM RESULTS WERE RECIEVED ON 4-24-01 INDICATING THAT EE HAD A 26.6 DB STANDARD THRESHOLD SHIFT IN HEARING IN HIS LEFT EAR. THIS IS BEING REPORTED AS A HOISE INDUCED HEARING LOSS.
EMPLOYEE WAS CUTTING A PLASTIC BAND ON A SKID OF BLOCK WITH A RAZOR KNIFE. THE RAZOR KNIFE SLIPPED PUNCTURING HIS RIGHT THIGH. STITCHES WERE USED TO CLOSE THE CUT MAKING THIS A REPORTABLE INCI DENT. HE WAS SCHEDULED TO VACATION 4-25-01 TO 5-01-01.
EE WAS REMOVING A BELT ROLLER FROM THE 00 RIGHT STORAGE UNIT. HE DROPPED THE ROLLER ON HIS LEFT HAND LACERATING HIS LEFT INDEX FINGER.
ON 3-16-01 AN IGNITION OCCURRED ON THE FACE OF THE 2NE LONGWALL. THE SHEARER WAS CUTTING FROM THE TAIL TO THE HEAD. AT SHIELD #182 THE HEAD DRUM HIT A CHERT ROLL THAT CAME DOWN INTO THE COAL S EAM APPROX. 8". WHEN THE BITS HIT THE CHERT A SMALL IGNITION OCCURRED. A FLAME THE SIZE OF A BASKETBALL WAS OBSERVED.
EE WAS CROSSING THE TRACK AT THE 3NE JEEP SHOP. HE TRIPPED ON THE RAIL AND FELL ON HIS RIGHT SIDE. A DR.'S EXAMINATION ON 3-16-2001 REVEALED THAT EE HAD A HERNIA ON THE RIGHT SIDE OF HIS GROIN .
EE WAS WALKING DOWN TRACK AND STEPPED ON A CROSSTIE WHICH WAS UNDER WATER AND TWISTED HIS RIGHT KNEE. HE CONTINUED WORKING UNTIL JUNE 6 2001 WHEN HE TOOK OFF TO HAVE.
EE WAS STARTING A NEW HOLE. HE RELEASED THE UP PRESSURE AND THE DRILL STEEL HIT HIM ON THE LEFT HAND RESULTING IN A FRACTURE IN HIS LEFT HAND.
RESULTS WERE RECEIVED ON 2-22-01 INDICATING THAT EE HAD A 25DB STANDARD THRESHOLD SHIFT IN HIS HEARING-LEFT EAR. THIS IS BEING REPORTED AS A NOISE INDUCED HEARING LOSS.
ON 3-16-01 EE COMPLETED AN ACCIDENT REPORT STATING SHE HAD HIT HER LEFT INDEX FINGER ON A CABLE HOOK ON THE SIDE OF THE MINER WHILE PUTTING THE MINER CABLE IN THE HOOK. EXAMINATION REVEALED SH E HAS A BONE CHIP IN THE FINGER. EE CONTINUES TO WORK IN HER MINER HELPER JOB ON OR3NE.
EE WAS OPERATING A MANTRIP ON THE 3R3NE TRACK. A SET OF BELT CLAMPS WERE STICKING OUT THE SIDE OF THE MANTRIP. THE CLAMPS HIT A CRIB ALONG THE TRACK, DISLODGING IT. THE CRIB BLOCKS FELL, STRIK ING EE ON THE RIGHT KNEE.
EE WAS BOLTING IN #1 FACE. HE STARTED TO RAISE THE RIGHT FRONT CANOPY. THE MAST THE CANOPY IS MOUNTED TO STARTED TO FALL OVER. EE HELD THE CANOPY AND MAST UP WITH HIS RIGHT SHOULDER CAUSING A ROTATOR CUFF TEAR IN HIS RIGHT SHOULDER.
AUDIOGRAM RESULTS WERE RECD ON 1/30/01 INDICATING THAT EE HAD A 25DB STANDARD THRESHOLD SHIFT INHIS HEARING THIS IS BEING REPORTED AS A NOISE INDUCED HEARING LOSS
EE WAS HELPING RECOVER BELT STRUCTURE. HE TURNED TO DISCONNECT A SECTION OF STRUCTURE. HE TRIPPED AND FELL ON HIS RIGHT ARM, FRACTURING IT.
EMPLOYEE WAS CROSSING THE SPILL TRAY OF THE LONGWALL FACE CONVEYER TO CHANGE BITS IN THE HEAD DRUM OF THE SHEARER, WHEN HE SWUNG HIS LEFT LEG OVER THE SPILL TRAY HE EXPERIENCED PAIN IN HIS LOW ER BACK EXTENDING DOWN INTO HIS LEFT HIP AND LEG

EE WAS PULLING ON A SPANNER WRENCH REMOVING A CATHEAD FROM THE GATE BOX. THE WRENCH SLIPPED OFF THE CATHEAD AND HIT EE IN THE FOREHEAD RESULTING IN A LACERATION REQUIRING STITCHES.
ON 11/16/00 AT APPROX 3:32 PM A METHANE IGNITION OCCURRED ON THE INE LONGWALL (MMU 202-0). THE IGNITION OCCURRED AT THE TAIL DRUM. THE FLAME FROM THE IGNITION WAS 6" TO 12" LONG AND LASTED FRO 3 TO 5 SECONDS.
EE WAS OWRKING WITH THE LONGWALL SETUP CREW. EE STOOD UP AFTER A MEETING ON THE DAY'S WORK AND EXPERIENCED PAIN IN HIS LOWER BACK. EE WENT TO THE DR AND MISSED SIX DAYS' WORK.
EE WAS HELPING MOVE TRACK RAILS WHEN HE EXPERIENCED PAIN IN HIS RIGHT SIDE EXTENDING DOWN INTO HIS GROIN. A DR'S EXAM REVEALED EE HAD 3 SEPERATE HERNIA'S.
INJURED AND TWO CO-WORKERS WERE INSTALLING A NEWTOP ROLLER IN THE SLOPE BELT. INJURED WAS LAYINGON HIS BACK PUSHING UP ON THE ROLLER. THE ROLLERCAME OUT OF IT'S BRACKET AND HIT HIM ON THE FORE HEAD RESULTING IN A LACERATION NEEDING SUTURES.
EMPLOYEE WAS HELPING RECOVER BELT STRUCTURE OFF THE SOUTH MAINS, WHILE DRAGGING AND LOADING THE STRUCTURE HE EXPERIENCED PAIN IN HIS LOWER BACK, HIP, AND LEGS. HE CONTINUED TO WORK UNTIL 1-4-0 1 WHEN HE WAS TAKEN OFF WORK BY HIS DOCTOR.
ON 10/11/00 A ROOF FALL WAS FOUND IN THE 3NE "A" RETURN. THE FALL WAS LOCATED APPROX 20'INBY SPAD 2409. THE FALL MEASURED 20'LONG, 20'WIDE, & 5'HIGH. THE FALL AREA AS WELL AS AREAS INBY AND OU TBY HAVE BEEN SUPPORTED WITH 4"BEAMS ADN 100 TON JACKS.
EMPLOYEE WAS ENTERING THE SLOPE BELT COMPARTMENTAT THE TOP OF THE SLOPE. AS HE WENT THROUGH THE AIR LOCK DOOR A PIECE OF RUST BLEW INTO HIS RIGHT EYE. HE WAS GIVEN PRESCRIPTION EYE MEDICATION TO RELIEVE THE EYE IRRITATION.
A ROOF FALL OCCURRED IN THE 3 RIGHT 3 NORTHEAST NBELT AND TRACK ENTRY. THE FALL MEASURED 72' LONG; 24'-25' WIDE, AND 12'-18' HIGH.
A ROOF FALL OCCURRED IN THE LONGWALL HEADGATE ENTRY APPROX 70' INBY SPAD #3087. THE FALL SET DOWN ON THE LONGWALL HEADGATE SHIELDS AND APPROX 15' OUTBY THE SHIELDS ON THE STAGELOADER. THE FALL MEASURED APPROX. 20' WIDE, 19' LONG AND 10' HIGH. NO ONE WAS INJURED IN THE FALL.
EE'S BELT CREW WAS WELDING ON THE NORTHEAST "B" BELT CHUTE. DURIN GTHE SHIFT EE'S EYE'S WERE EXPOSED TO THE WELDING ARC AND BECAME IRRITATED. EE RECEIVED PRESCRIPTION MEDICATION FORHTE IRRIT ATION.
EE WAS CUTTING A PLASTIC TIE WITH A RAZOR KNIFE. HE WAS CUTTING TOWARD HISMELF AND CUT THE BACK OF HIS LEFT HAND REQUIRING STITCHES.
EMPLOYEE WAS SITTING ON A SUPPLY CAR AS IT WAS BEING PUSHED. THE PROP SETTERS ON THE CAR STRUCK A 3" WATER LINE COUPLER AND MOVED ON THE CAR HITTING HIM ON THE RIGHT KNEE. MIKE COMPLETED ACCID ENT REPORT ON 8-7-00 AND WENT OFF WORK DUE TO CONTINUING PAIN IN HIS KNEE.
EMPLOYEE WAS UNLOADING MEGA BLOCK OFF OF A SUPPLY CAR AND WHILE THROWING THE BLOCK HE TWISTED HIS LEFT KNEE. HE CONTINUED TO WORK AND DID NOT FILL AN ACCIDENT REPORT OUT UNTIL 7-30-00.
EE WAS GETTING OUT OF A MANTRIP ON BRIGHT. HIS RIGHT FOOT SLIPPED IN MUD WHILE HIS LEFT LEG WAS STILL IN THE MANTRIP. THIS CAUSED EE'S RIGHT KNEE TO HYPEREXTEND. EE CONTINUED TO WORK THRU 9-30 -00. ON 10-3-00 EE HAD SURGERY ON HIS KNEE AND IS NOW OFF WORK.
EE AND A CO-WORKER WERE CARRYING A BELT STAND. THE CO-WORKER DROPPED THE BELT STAND WITHOUT TELLING EE. HIS RIGHT INDEX FINGER WAS CAUGHT IN THE BELT STAND CUTTING THE TIP.
EE WAS CUTTING A PLASTIC TIE WITH A RAZOR KNIFE. THE KNIFE SLIPPED CUTTING EE'S LEFT LITTLE FINGER.
EMPLOYEE AND A CO-WORKER WERE INSTALLING A LARGER MANDOOR IN A STOPPING. HE WAS USING A SLEDGE HAMMER TO MAKE THE OPENING LARGER. HE WORKED THE REMAINDER OF THE YEAR 2000 AND WENT OFF WORK 1-5 -01 FALLING BACK TO THE 6-3-00 INCIDENT.
EMPLOYEE WAS HELPING CARRY A BELT ROLLER ON THE SILO CATWALK. HIS LEFT FOOT BECAME FOULED BETWEEN TWO ROLLERS ON THE CATWALK STRAINING HIS KNEE.HE IS PRESENTLY OFF WORK DUE TO CARPAL TUNNEL SU RGERY AND DECIDED TO HAVE ARTROSCOPIC SURGERY DONE AT THE SAME TIME ON HIS KNEE.
ON 5-16-00 EE WAS DIAGNOSED AS HAVING CARPAL TUNNEL SYNDROME IN BOTH HANDS. ON 7-26-00 EMG RESULT WERE RECIEVED SUPPORTING THE CARPAL TUNNEL CLAIM. HE IS NOW OFF WORK HAVING SURGERY ON BOTH HA NDS.
EE WAS OPERATING A JEFFREY 15 TON LOCOMOTIVE COMING OUTBY ON THE 3R3NE TRACK. THE LOCOMOTIVE DERAILED ON A HILL AND SLID BACKWARDS DISLODGING TWO TIMBERS. EE TWISTED HIS BODY TO PUSH ONE OF TH E TIMBERS OFF OF THE MOTOR AND STRAINED A MUSCLEIN HIS LOWER BACK.
EE WAS SCATTERING TRACK TIES. ONE ROLLED OFF THE PILE AND HIT HIM ON THE RIGHT LITTLE FINGER. X-RAYS REVEALED A SMALL BONE CHIP IN THE FINGER.
EE WAS GRINDING ON A BALLAST CAR WHEEL WHEN A FOREIGN OBJECT GOT IN HIS LEFT EYE SCRATCHING IT. EE WAS GIVEN PRESCRIPTION EYE DROPS.
EE WAS GOING THROUGH A MANDOOR WITH A LOT OF AIR PRESSURE AGAINST IT. WHILE FORCING HIS WAY THROUGH THE DOOR HE EXPERIENCED PAIN IN HIS LOWER BACK. EE CONTINUED TO WORK INTO APRIL BUT NOW HAS WENT OFF WORK DUE TO THE PAIN IN HIS BACK.

A METHANE IGNITION OCCURRED IN THE FACE OF #1 ENTRY ON THE 3R3NE CONTINOUS MINER SECTION. THE IGNITION OCCURRED AT 745 PM AND LASTED 40 TO 60 SECONDS.
EMPLOYEE WAS PULLING THE SHIELDS BEHIND THE SHEARER AS IT WAS CUTTING TOWARDS THE HEADGATE. THE TAILEND DRUM KICKED UP A ROCK WHICH HIT EMPLOYEE'S LEFT RING FINGER. THE ROCK PINCHED HIS FINGER AGAINST THE SPILL TRAY CUTTING THE TIP OF HIS FINGER. THE CUT REQUIRED STITCHES.
EE WAS TIGHTENING BOLTS IN A SUB ASSEMBLY FOR A BELT SCRAPPER. HE SLIPPED OFF A BUCKET HE WAS STANDING ON AND STRAINED HIS LEFT HIP. EE RETURNED TO WORK BUT ON 11-20-99 HE REAGGRAVATED HIS HIP USING A STOPER DRILL AND MISSED 3 DAYS WORK.

THE COUNTERWEIGHT SAFETY SWITCH WAS TRIPPING DUE TO THE COUNTERWEIGHT DOGS BEING OUT OF ALIGNMENT CAUSING THE DOGS TO DRAG ONT HE RAIL. THE ELEVATOR WAS DOWN FOR OVER 30 MINUTES MAKING THIS RE PORTABLE.
ON 3-19-99 THE A.E.P FUEL SUPPLY COMPENSATION DEPARTMENT RECIEVED MEDICAL CERTIFICATION THAT EE HAD BI-LATERAL CARPAL TUNNEL SYNDROME. HE IS PRESENTLY OFF WORK DUE TO THE SURGERY REQUIRED TO R ELIEVE THE CONDITON.
EE WAS PUSHING THE TOP PIECE OF A DRILL STEEL INTO THE DRILL HOLE WHEN HE FELT PAIN IN THE RIGHT SIDE OF HIS LOWER BACK EXTENDING DOWN INTO HIS RIGHT LEG. EE IS NOW TAKING THERAPY DUE TO CONT INUING BACK AND LEG PAIN.

EE WAS WORKING ON HE 3NE SUBMAINS TRACK BOLTING METAL STRAPS UP TO SUPPORT THE RIBS. EE WAS WALKING BACK TO THE TRACK BOLTER AFTER EATING LUNCH. HE STEPPED ON A ROCK IN THE WALKWAY AND TWISTED HIS LEFT ANKLE.

ON 08-28-98 A ROOF FALL WAS FOUND IN THE 2SW SUBMAINS BY A FIREBOSS. THE FALL WAS IN THE #5 ENTRY JUST OUTBY SPAD #11869. THE FALL OCCURRED IN A FOUR WAY INTERSECTION AND WAS APPROXIMATELY 5' HIGH. THE FALL DID NOT EFFECT THE PASSAGE OF MENAND NO EQUIPMENT WAS INVOLVED.
EE HAD JUST TAKEN A CURTAIN DOWN IN #2 HEADING IN PREPRATION TO TURN A BREAK. AS HE WAS WALKING BACK TO THE MINER A PIECE OF SHALE FELL FROM BETWEEN BOLTS HITTING HIM ON TOP OF THE RIGHT FOOT BRUISING IT. EE IS NOW GETTING THERAPY MAKING THIS A REPORTABLE INCIDENT.

HE WAS DRILLING A HOLE FOR A 4' RESIN GROUTED BOLT. HE WAS DRILLING THE LAST FEW INCHES USING A BOLTER WRENCH AS AN EXTENSION ON THE DRILL STEEL. THE HOLE BECAME MIS-ALIGNED. THE DRILL STEEL B ENT AND TEH BOLTER WRENCH BROKE WITH A PIECE OF THE WRENCH HITTING HIM IN THE HEAD.
EE WAS CLIMBING DOWN FROM THE SLOPE CAQR. HE PUT HIS RIGHT FOOT DOWN ON THE SAFETY CHAINS. EE'S RIGHT FOOT SLIPPED AND EE TWISTED HIS RIGHT KNEE.
ORIGINAL ACCIDENT 1-5-93. HE IN 1993 STRAINED HIS BACK DUMPING BUCKETS OF WET MUCK ON THE BELT. OVER THE YEARS HE HAS CONTINUED TO HAVE PROBLEMS WITH HIS BACK. HE WENT OFF WORK ON 8-5-98 DUE T O HIS BACK. HE AND HIS DOCTOR BELIEVE THIS IS DUE TO THE 1993 INCIDENT. THEREFORE THIS IS BEING REPORTED AS AN LTA.

EE WAS PULLING A 12' LONG PIECE OF 4" TUBING OUT OF THE CHANNEL OF A 6" H-BEAM AND STRAINED HIS RIGHT SHOULDER. HE IS NOW RECIEVING CHIROPRACTOR TREATMENTS.
EMPLOYEE WAS A PASSENGER IN A MANTRIP THAT WAS STOPPED AT THE NORTHWEST SWITCH. ANOTHER MANTRIP COMING TO THE SWITCH DID NOT STOP IN TIME AND HIT EE'S TRIP. EE FELT PAIN IN HIS NECK AND IS TAK ING CHIROPRATIC TREATMENTS.
EE WAS CUTTING A SPLICE KIT OFF OF A PUMP THAT HE HAD TAPED ON EARLIER. HE CUT HIS LEFT THUMB WITH A RAZOR KNIFE. THE CUT REQUIRED 5 STITCHES.
EE WAS HELPING UNLOAD ROLLS OF 3" & 3" WATER DISCHARGE LINE OFF OF A JEEP. ONE OF THE ROLLS OF LINE PINCHED THE TIP OF HIS RIGHT LITTLE FINGER RESULTING IN AN ACCUMULATION OF BLOOD UNDER THE F INGER NAIL. THE FINGER NAIL WAS DRILLED AT HOLZER HOSPITAL ER & THE BLOOD RELEASED.
EMPLOYEE WAS RELEASING A COME-A-LONG AND THE HANDLE HIT HIM ON THE LEFT HAND FRACTURING HIS LEFT LITTLE FINGER.
EMPLOYEE WAS WALKING BEHIND HIS RIIF BOLTER. HE TURNED STRIKING HIS FACE ON A METAL ROOF STRAP LAYING ON TOP OF THE BOLTER. HE RECEIVED A LACERATION ON HIS CHEEK REQUIRING EIGHT (8) STITCHES.

EMPLOYEE WAS STANDING NEXT TO THE VENTILATION FAN. THE SECTION SCOOP CAUGHT THE FAN CABLE PULLING IT TIGHT. THE FAN CABLE WHEN PULLED TIGHT CAUGHT EMPLOYEE BEHIND HIS LOWER LEGS FLIPPIN HIM AG AINST THE RIB. EMPLOYEE HAS NOT MISSED ANY WORK BUT IS TAKING THERAPY DUE TO CONTINUING PAIN IN HIS RIGTH SHOULDER.
EE WAS BUILDING A STOPPING WHEN HE NOTICED. PAIN IN HIS LOWER BACK. WHILE CARRYING A BOX OF RESIN TO THE ROOF BOLTER HIS BACK STARTED HURTING WORSE. EE CONTINUED WORKING FOLLOWING THE ACCIDENT BUT DUE TO CONTINUING PAIN HE NOW HAS TAKEN OFFFOR THERAPY.
THE NORMALLY CLOSED CONTACTS ON THE AUXILLARY UPRELAY WERE NOT CLOSING WHEN THE RELAY WAS DE-ENERGIZED. THE CONTACTS ON THE UPRA RELAY WOULD NOTMAKE UP DUE TO WEAR IN THE CONTACT BLOCK. THE EN TIRE RELAY WAS CHANGED OUT.
EE WAS CROSSING UNDER A BELT AT A CROSS UNDER. HE SLIPPED ADN FELL FRACTURING HIS LEFT RING FINGER.
HE WAS FIREBOSSING THE 9R1NW BELT. HE SLIPPED AND TWISTED HIS LEFT KNEE. ON 5-21-97 HE HAD FLUID REMOVED FROM THE KNEE MAKING THIS A REPORTABLE ACCIDENT.

EE WAS OPERATING A SCOOP HAULING GOB ON THE 9 SOUTH LONGWALL TAILGATE. THE SCOOP HIT A DIP IN THE ROADWAY. EE'S LEFT ARM FELL OFF THE DGE OF THE DOOR FRAME. THE REAR STEERING JACK PINCHED THE BOTTOM HALF OF HIS FOREARM BRUISING IT AND CAUSING SWELLING.

EE WAS USING AIR BAGS TO RERAIL A TWI SUPPLY CAR. ONE OF THE AIR BAGS SLIPPED OUT FROM UNDER THE CAR. HITTING EE IN THE CHEST AND ABDOMEN RESULTING IN CONTUSIONS AND SORENESS.
EE WAS SITTING ON THE FLOOR OF THE SHOP WELDING ON A SECTION OF PANLINE. HE TWISTED THE TRUNK OF HIS BODY AND EXPERIENCED PAIN IN HIS LOWER BACK LEFT SIDE. HE CONTINUED TO EXPERIENCE PAIN AND IN APRIL STARTED TO SEE A CHIROPRACTOR.
EE WAS MAKING A CABLE SPLICE. AS EE WAS HITTING A CRIMPING TOOL WITH A HAMMER A SMALL PIECE OF METAL IMBEDDED IN HIS LEFT HAND. INITIALLY THE DR. WAS NOT GOING TO REMOVE THE METAL. ON 4-11-97 DR. CHANGED HIS MIND AND REMOVED THE SMALL PIECE OF METAL
EMP WAS HELPING BUILD A STOPPING ON THE 9R1NW CONTINUOUS MINER SECTION. HE PICKED UP A BLOCK ANDFELT PAIN IN HIS RIGHT ELBOW. ON MARCH 6, 1997 EMP STARTED STRENGTHENING THERAPY ON HIS ELBOW MA KING THIS A REPORTABLE INCIDENT.
EE WAS COUPLING A 15 TON MOTOR TO A ROCKDUSTE.R HE KICKED THE COUPLER ON THE ROCKDUSTER TO ALIGH IT WITH THE ONE ON THE LOCOMOTIVE. THE TOE OF HIS RIGHT BOOT CAUGHT BETWEEN THE TWO COUPLERS FRACTURING TWO TOES ON HIS RIGHT FOOT.
EE WAS UNLOADING HIGH VOLTAGE OFF A SUPPLY CAR AS IT WAS BEING PULLED SLOWLY ALONG THE TRACK. HE WAS ON HIS KNEES ON THE CAR. AS THE CAR WAS GOING THROUGH AN AIR LOCK DOOR EE HIT HIS HEAD O N THE BELTING AROUND THE DOOR OPENING STRAINING HIS NECK.
EE AND A CO WORKER WERE INSTALLING A CONVEYOR CHAIN IN A FEEDER ON THE 8 RINIW SECTION. EE AND CO WORKER WERE PULLING AND LIFTING ON THE CHAIN WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK.
1L2SW TRACK AT TURN OF +3200 FT BREAK. THE ROOF FALL OCCURRED IN A 4-WAY INTERSECTION WHERE THE TRACK TURNS FROM E-W TO N-S. THE AREA HAD BEEN SUBJECTED TO A SQUEEZE CONDITION WITH THE FLOOR H OVING UPWARD. THE FLOOR HAD 8" H BEAMS ON THE 4 FT CENTERS W/SUPER PROPS ON EACH END. ROOF BOLTED W/4 FT FULLY GROUTED BOLTS. THE HEIGHT WAS 6 FT. THE FALL EXTENDED 10-15 FT. INTO E-W ENTRY.
EE AND OTHER EE WERE PULLING A 4/3 CABLE OFF A SPOOL WHEN EE EXPERIENCED A BURNING SENSATION IN THE RIGHT GROIN AREA. EXAMINATION BY A SURGEON REVEALED HE HAD A HERNIA WHICH WOULD REQUIRE SURG ERY.
EE WAS GETTING INTO THE COPERATOR'S DECK OF A SCOOP. WHEN HE PUT HIS FOOT IN THE SEAT TO GET IN THE SEAT BROKE FREE OF IT MOUNTING AND EE FELL BACKWARDS STRAINING HIS BACK. HE IS TAKIGN THER APY (ELECTRICAL STIMULUS) AT THIS TIME.

ON 12-26-96 A ROOF FALL WAS FOUND ON 3R1NW TRACK BY THE CO FIREBOSS. THE FALL MEASURED 23 FT WIDE X 18 FT LONG AND 2 FT TO 2.5 FT THICK. THE FALL WAS CLEANED UP AND THE AREA REBOLTED.
EMPLOYEE WAS REPAIRING A 3" DISCHARGE LINE ON A PUMP. HE STEPPED ON SOMETHING UNDER THE WATER TWISTING HIS LEFT KNEE. EMPLOYEE HAS NOT MISSED ANY WORK BUT STARTED TAKING THERAPY ON HIS KNEE ON 12-31-96.
AT THE END OF THE SHIFT EE WALKED AROUND THE END OF HIS SHUTTLECAR TO SHUT THE BELT SWITCH OFF. HE STEPPED IN A HOLE TWISTING HIS LEFT KNEE. EE CONTINUED TO WORK THRU 1-28-97. ON 1-29-97 HE HA D SURGERY TO REPAIR A PARTIALLY TORN LIGAMENT MAKING THIS AN LTA.
THE INJURED WAS WALKING FROM THE DINNERHOLE TO THE FACE. HE STEPPED IN A HOLE SWHICH CAUSED HIM TO TRIP. AS HE STUMBLED HE FELT A SHARP PAIN IN HIS LOWER BACK. EE IS NOT RECIEVING CHIROPRACTIC TREATMENT.
EE WAS LOOKING FOR PARTS IN THE LONGWALL STORAGE AREA. HE STEPPED ON A COWL ARM RING WHICH FLIPPED UPA ND STRUCK HIM ON THE INNER PART OF THE RIGHT KNEE. EE'S HAS NOT MISSED ANY WOK BUT HAS NOW STARTED THERAPY ON HIS KNEE.
EE WAS PULLING ON A SHIELD HOSE ON THE 8 SOUTH LONGWALL TEARDOWN. THE HOSE CAME FREE SUDDENLY AND SNAPPED BACK HITTING EE IN THE MOUTH BREAKING THE TIP OFF OF ONE OF HIS FRON UPPER INCISOR TEE TH.
INJURED WAS PREPARING TO ATTACH THE PETITO MULE BOOM HOOK TO A RING ON THE 4W HEAD DRIVE.HE ASKED FOR THE BOOM TO BE EXTENDED.THE PETITO OPERATOR MOVED THE BOOM TO THE LEFT CATCHING INJURED'S RIGHT ARM BETWEEN THE BOOM AND HEAD DRIVE FRACTURING HIS RIGHT FOREARM.
INJURED WAS SPREADING SHEETS OF Q-DECKING ON AN OVERCAST.A SMALL PIECE OF ROCK FELL FROM THE TOP HITTING HIM ON THE RIGHT HAND FRACTURING HIS RIGHT INDEX FINGER.
EE WAS DRIVING A SHUTTLECAR THROUGH A SET OF FLYPADS. A RAGGED PIECE OF FLYPAD KNOCKED HIS HARD HAT AND SAFETY GLASSES OFF GETTING DIRT IN HIS LEFT EYE. EE RECEIVED PRESCRIPTION EYE MEDICATION FOR HIS EYE MAKING THIS A REPORTABLE INCIDENT.
YEE WAS PULLING WIRE ROPE OFF OF A SCOOP WINCH WHIL STANDING IN THE SCOOP BUCKE.T EE'S RIGHT FOOT SLIPPED ON TEH METAL SCOOP BUCKET TWISTING HIS RIGHT KNEE. X-RAYS ON 8-28-96 REVEALED A BONE CHIP IN THE RIGHT KNEE JOINT.
EE WAS GOINDING THE 1/2" WIRE ROPE ONTO THE WINCH DRUM WITH HIS LEFT HAND AND OPERATING THE WINCH CONTROL WITH HIS RIGHT HAND. EE'S LEFT HAND BECAME CAUGHT BETWEEN THE ROPE IN HIS HAND AND THE ROPE ON THE DRUM. HIS LEFT HAND AND ARM WERE PULLED OVER THE TOP OF THE DRUM FRACTURING HIS LEFT FOREARM.
EE WAS PREPARING TO TRAM THE SHEARER TO THE TAIL. A FALL OCCURRED BEHIND THE SHIELDS KNOCKING THE #12 SHIELD FORWARD. EE'S LOWER LEGS ABOVE THE ANKLES WERE CAUGHT BETWEEN A TRAPPING SHOE LYING ON THE PONTOONS OF SHIEDL #12 AND THE CABLE TRAYS ON THE PANLINE FRACTURING HIS RIGHT LEG.
EE WAS STRETCHING OUT BATTERY CAR CABLES TO 7501 LOCOMOTIVE. WHEN HE WAS PULLING ON THE CABLES HE EXPERIENCED PAIN IN HIS LEFT KNEE. EE CONTINUED TO WORK BUT AN MRI TAKEN THE LAST OF JULY REVE ALED A TORN CARTILAGE AND EE HAS NOW GONE OFF WORK FOR SURGERY.
A ROOF FALL WAS FOUND ON THE 3R1NW TRACK BY A FIREBOSS. THE FALL MEASURED 20' X 20' X 4' THICK. THE FALL FELL ON THE TRACK, TROLLEY, A HIGH VOLTAGE CABLE AND A 6" WATERLINE.
EE WAS STARTING TO REPOSITION A SHEAVE THAT WAS BEING USED TO MOVE A SHEARER DRUM. HE WAS HOLDING ONTO THE 3/4" WIRE ROPE WITH HIS LEFT HAND. THE DRUM SHIFTED AND PULLED EE S LEFT HAND INTO TH E SHEAVE PULLEY MASHING HIS FINGERS.
EE WAS TIGHTENING BOLTS ON A BELT DRIVE INSTALLATION. HE WAS PUSHING ON A 1 1/4" BOX END WRENCH WITH HIS RT FOOT. HIS FOOT SLIPPED OFF THE WRENCH STRAINING HIS RT KNEE. DUE TO CONTINUED PAIN I N HIS KNEE EE HAD IT EXAMINED ON 9-12-96. HE WILL HAVE SURGERY ON 10-15-96, MAKING THIS AN LTA.
INJURED WAS LAYING TRACK RAIL.A C0-WORKER WAS BARRING A RAIL INTO PLACE.THE RAIL ROLLED HITTING INJURED ON TOP OF THE RIGHT KNEE.
A ROOF FALL WAS FOUND ONE BREAK OUTBY SPAD # 11793 IN THE #6 ENTRY OF THE 25W SUBMAINS. THE ROOFFALL MEASURED APPROXIMATELY - 20 1/2' BY 20 1/2'BY 8' HIGH.
EE WAS CUTTING A PLASTIC TIE OFF OF A CONTROL CABLE AND CUT HIS LEFT HAND.
AT APPROX. 4:50 PM ON 3-22-96 THE THRUSTER BRAKE UNIT MALFUNCTIONED. A NEW UNIT WAS INSTALLED AND THE ELEVATOR WAS PUT BACK IN SERVICE AT 7:00 PM ON 3-22-96. (PUMP HAD FAILED ON ORIGINAL UNIT)
HE WAS WALKING TO THE SECTION FROM THE MANTRIP STATION. HE STEPPED ON A ROCK TWISTING HIS LEFT KNEE. HE HAS NOT MISSED ANY WORK BUT ON 3-25-96 HE STARTED TAKING THERAPY ON HIS KNEE MAKING THIS A MEDICAL ACCIDENT.
EE WAS DRILLING MESSINGER WIRE HANGER HOLES. HE MOVER THE BOLTER TO THE NEXT HOLE. WHEN HE SET THE STAB JACK THE CANOPY RAISED. HE HAD HIS RIGHTHAND ON THE LIP OF THE CANOPY & CAUGHT THE TIP O F HIS RIGHT INDEX [FINGER] BETWEEN THE CANOPY & THE TOP.
EMPLOYEE WAS WALKING THROUGH MUD IN THE #3 ENTRY. HIS RIGHT FOOT BECAME STUCK AND WHEN HE PULLED IT OUT OF THE MUD HE TWISTED HIS KNEE.
EMP AND 2 COWORKERS WERE PUTTING THE RUB RAIL BACK IN PLACE ON THE 1071 CONTINUOUS MINER ON THE 11L25W SECTION. ONE END OF THE RUB RAIL FELL OUTOF ITS BRACKET AND STRUCK EMP ON TOP OF RIGHT FO OT RESULTING IN CONTUSION AND SWELLING.
EMPLOYEE WAS PUTTING A BELT CLAMP ON A BELT RAIL IN PREPRATION TO MAKE A BELT SPLICE. WHEN HE LIFTED ON THE CLAMP HE FELT PAIN IN HIS LOWER BACK. EMPLOYEE CONTINUED TO WORK WITH PAIN, ON MARCH 15, EMPLOYEE HAD AN MRI WHICH REVEALED A RUPTURED DISC AND EMPLOYEE WENT OFF WORK FOR THERAPY.
HE WAS REMOVING A COVER FOR THE CONTACTOR BOX ON MANTRIP 7763. THE COVER WAS STUCK AND WHEN HE JERKED ON IT HE STRAINED HIS RIGHT SHOULDER.
UPON INSPECTION IT WAS FOUND THAT DNRA RELAY NEEDED TO BE REPLACED. ELEVATOR WAS DOWN FOR OVER 30 MINUTES REPLACING THE RELAY MAKING THIS A REPORTABLE INCIDENT.

EE HOOKED A 500' PIECE OF 48" BELT TO A LOCOMOTIVE SO IT COULD BE PULLED OUT OF THE WORK AREA. AS HE WAS WATCHING THE BELT BEING PULLED IT FLIPPED OVER & WAS PULLED OVER HIS RIGHT FOOT & LEG. HIS FOOT WAS TWISTED RESULTING IN A FRACTURE & SEVERE DISLOCATION.
EE WAS WALKING DOWN THE LONGWALL FACE AND HIT HIS HEAD ON A SHIELD CANOPY STOVING HIS NECK. EE HAD THREE THERAPY TREATMENTS ON HIS NECK.
EE WAS REMOVING STRUCTURE FROM A BELTLINE AND RECIEVED A STRAIN TO HIS SHOULDER. HE HAS NOT MISSED RECIEVED A STRAIN TO HIS SHOULDER. HE HAS NOT MISSED ANY WORK BUT DUE TO CONTINUING SORENESS WILL START THERAPY 11-20-95 MAKING THIS A REPORTABLE ACCIDENT.
EE WAS WALKING THE 25W RETURN. HE STEPPED ON SOME UNEVEN BOTTOM TWISTING HIS RIGHT KNEE. HE CONTINUED TO WORK UNTIL 1-25-96 WHEN HE WAS TAKEN OFF WORK FOR REST AND THERAPY. SURGERY MAY BE REQU IRED.

EE WAS HELPING MOVE A PIECE OF 48" BELT THAT WAS LAYING IN THE TRACK. HE WAS PULLING ON THE BELT WITH A PAIR OF CHANNEL LOCKS. THE CHANNEL LOCKS SLIPPED AND EE FELL BACKWARDS. UPON MEDICAL EXA MININATION IT WAS DETERMINED HE HAD A HERNIA.
EMPLOYEE WAS INSPECTING THE INSIDE OF THE UNDERGROUND COAL BINS AND HIT HIS ELBOW ON THE SIDE OF THE BIN. ON 9-7-95 EMPLOYEE HAD FLUID REMOVED FROM THE JOINT MAKING THIS A REPORTABLE INCIDENT.
