A ROOF FALL WAS DISCOVERED AT 12+50 STATION ON #2 TRACK ENTRY AT #1 PORTAL. THIS AREA WAS MINED IN EARLYB1972. 8' CONVENTINAL BOLTS WERE USED ON DEVELOPMENT WITH WOODER HEADERS AND 6X6 PLATES. ADD'L SUPPORT WAS ADDED USING 10' COMBINATION BBOLTS AND SCREEN. FALL WAS 42' LONG X 15 1/2' WIDE AND 6-7 1/2' HIGH.

IN THE PROCESS OF REMOVING THE PITOT TUBE, HE FIRST SET DOWN THE MAGNEHELICGUAGE WITH HIS RIGHT HAND ON THE CONC RETE PAD. AT THIS TIME, THE PITOT TUBE END CAUGHT IN THE FAN BLADES CAUSING IT TO TWIST SUDDENLYIN HIS LEFT HAND INJURING 3 FINGERS AND THE PALMOF HIS HAND

PERPARATION PLANT 8TH FLOOR AT A CONDUIT SPLICE BOX. IN THE PROCESS OF REPLACING THE COVER ON THE SPLICE BOX, HE CAME INTO CONTACT WITH A POWER WIRE CAUSING A BURN TO HIS LEFT THUMB.

3G LONGWALL FACE. HE WAS FOLLOWING TH SHEAR TO THE TAILGATE, ADN WAS SNAKING PANLINE OVER BETWEEN #46 AND #50 SHIELDS TO OBTAIN ADDITIONAL TRAVEL SPACE. THE CLEARANCE WAS TIGHT TO TALLOW MAI NTENANCE TO MAKE REPAIRS ON A RAM THAT WAS OUT OF THE SADDLE ON #54 SHIELD. HIS RIGHT FOOT WAS ON THE BACK SECTION OF TH ERAM AND AS THE RAM EXTENDED HIS FOOT SLIPPED AND CAUGHT THE EDGE OF T
3G LONGWALL SECTION FACE AT SHIELD #141 - HE WAS PUSHING THE PANLINE UP FOLLOWING THE SHEAR TOWARDS THE HEADGATE. WHEN HE CROSSED UNDER SHIELD #141 [SHIELD WAS SUSPENDED BETWEEN SHIELDS 140 & 142] & PLACED IT IN THE PUSH MODE. THE SHIELD RELEASED & THE CANOPY STRUCK HIM ON THE BACK OF HIS HEAD. HE STATED THAT HE WAS UNAWARE OF THE SHIELD BEING SUSPENDED.

EAST MAINLINE TRACK-WAS OPERATING A 50 TON HAULAGE MOTOR PULLING TRIP OF LOADED COAL CARS TOWARDTHE ROTARY DUMP. LOADED COAL CARS (13) DERAILED DIRECTLY BEHIND THE HAULAGE AND THEN STRUCK THE HAULAGE MOTOR CAUSING THE OPERATOR TO BE JARRED IN THE OPERATOR COMPARTMENT.
BOLTING ON A CONT MINER WHENTHE DRILL STEELS GOTCAUGHT INTHE TOP. EE BROUGHT THE DRILL STEELS OUT WITH THE DRILL AND THE DRILL STEELS CAME APARTEE REACHED UP TO HOLD THE TOP STEEL WHILE HE DRO PPED THE CHUCK. THE BOTTOM STEEL CAME OUT AND THE TOP STEELWAS ON A BIND AND CAUGHT HIS LEFT INDEX FINGER. EE WAS WEARING METACARPAL GLOVES AT THE TIME OF THE INJURY.
EE WAS WORKING AS THE HELPER FOR ANOTHER EE THATWAS OPERATING A MICON DRILL UNIT. THE OPERATOR WAS DRILLING A 12' BELT HOLE TO INTALL A CABLE BOLT. THE INJURED EE WAS USING HIS LEFT HAND ATT EMPTING TO LINE UP & ATTACH A 2FT EXTENSION TO THE SCROLL AUGER ALREADY IN THE BOLT HOLE. THE DRILL UNIT SURGED UPWARD & HIS HAND CAUGHTON THE SCROLL AUGER & WAS PUSHED INTO THE EDGE OF THE
EE WAS THE OPERATOR OF A JEEP STOPPED ALONG 5 NORTH TRACK WAITING TO SWITCH INTO A TRACK SPUR CHUTE. ANOTHER JEEP CAME OUT OF THE SPUR CHUTE &WAS TO SWITCHOUT ON THE STRAIGHT TRACK INBY THE SWITCH. THE JEEP OPERATOR OF THIS JEEP DIDN'T CHANGE THE DIRECTIONAL LEVER SWITH & TRAMMEDINTOTHE STOPPED JEEP
WAS WALKING ALONG HAULAGE TRACK TO HIS MOTOR HE WAS OPERATING, STEPPED OVER COAL CAR COUPLER ONTO MINE FLOOR AND FELT PAIN IN HIS RIGHT FOOT.

WAS WORKING AT TOP OF SCALPING TOWER CHIPPING ICE OFF BELLY PAN OF SLOPE BELT. A PIECE OF ICE (3' X 2' X 6") CAME LOOSE AND CAUGHT HIS FINGER BETWEEN THE ICE AND THE FRAME OF THE BELLY PAN.

EMPLOYEE WAS STANDING ALONG SIDE THE BOOM TRUCK.AS THE OPERATOR PREPARED TO LIFT, HE SET OUTRIGGERS. EMPLOYEE WAS INATTENTIVE TO THE OUT RIGGER DRSCENDING. OUT RIGGER CAME DOWN ON HIS LEFT FOO T.
HE WAS SEATED IN THE OUTBY END OF OPEN MANTRIP (JEEP) WHEN THE TROLLEY POLE CAME OFF THE TROLLEY WIRE, HE REACHED TO PUT THE POLE BACK ON THE WIRE AND PHONE CABLE CAUGHT HIM UNDER HIS CHIN KNO CKING HIM INTO BACK SUPPORT OF SEAT.
OPERATING A JEEP IN 5A 5 SOUTH TRACK ENTRY, JEEPRAN INTO A STONE AND CAME TO SUDDEN STOP CAUSINGEMP TO BE THROWN OUT OF INBY END OF JEEP 10' INTO CENTER OF TRACK ENTRY.

HIT IN HEAD WITH TREE LIMB WHILE CLEARING AND GRUBBING AT REFUSE AREA.
EE WAS OPERATING BOLTER ON CONT MINER, WHILE DRILLING A BOLT HOLE, A PIECE OF STONE FELL FROMTHE MINE ROOF STRIKING EE ON THE NECK AND UPPER BACK AREA CAUSING CONTUSIONS.
EE WAS HOLDING A LADDER FOR ANOTHER EE WHO WAS USING A HOTSTICK TO DISENGAGE A FUSE IN SUBSTATION. FUSE CAME OUT AND FELL STRIKING HIS UPPER RT ARM, CAUSING A LACERATION REQUIRING SUTURES.
EE CLAIMS HE WAS CLEANING THE DUST BOX ON A ROOF BOLTER AND TOOK HIS GLOVE OFF AND STUCK HIS LEFT HAND UP IN THE TOP OF THE DUST COLLECTIMG BOX AND CUT HIS LEFT RING FINGER ON A HOSE CLAMP.
EE WAS BURNING A 6" NIPPLE OVERHEAD WITH AN OXYGEN ACETYLENE TORCH. EE WAS USING FACE SHIELD AND SAFETY GLASSES.
EMPLOYEE CLAIMS HE WAS WALKING FROM SPOTTER LOADOUT TO JEEP AT WEST MAINS CALL POINT. HESTATED HE STEPPED ON A RAIL AT WEST MAINS CALL POINT AND TWISTED HIS LEFT ANKLE.

EMPLOYEE WAS RIDING OUTBY END OF JEEP FACING DIRECTION OF TRAVEL. FOREMAN STOPPED TO GET CLEARENCE AT 222 HOLE. WHEN HE GOT CLEARENCE HE MOVED ABOUT 25 FT. WHEN EMPLOYEE STRUCK HIS HEAD. FOREM AN HAD JUST TOLD THE PEOPLE ON THE JEEP TO WATCH THEIR HEADS, LOW ROOF.
