Employee was hosing the floor in the breaker building and as he started out the back side doorway that had a piece of curtain hung for a door he caught his left forearm on a jagged piece of metal hidden by the curtain causing a laceration which required 8 stitches to close.

Injured was cleaning up coal he had spilled. He was shoveling it back into the bed of his trailer. When letting his tailgate back down he had propped open he had a shovel in his left hand and the tailgate in the right hand straining a tendon in his right arm.
EE was working inside the #1 scalping screen, upon exiting the screen, he twisted his left knee while trying to make it from the discharge lip of the screen over to the crusher chute then onto the breaker building floor.
Injured was fixing lights using a utility knife, when the knife slipped, cutting his left finger. Injured went to the hospital and received 3 stitches. Management was not aware of the accident until 10-1-10.

Injured was cutting conveyor belt with a utility knife when he cut himself on the right thigh resulting in a laceration which required staples to close. Injury was to a contract laborer, operator was made aware that staples were administered on 4-2-09

Getting in water truck, feet slipped and fell and hurt back.

Injured was fixing lights on a Benson trailer #132 standing on a ladder. He slipped and fell off ladder onto a concrete pad. Took to hospital by supervisor. Has a slight concussion to head and hurt right elbow.
Employee was washing a truck and slipped and fell on snow covered bank. Injured did not report injury until 1/29/07.

Employee was installing raw coal screens. He was using a long handled sledge hammer to pound them into place. As he swung the hammer downward, the hammer handle came in contact with the screen deck causing his right middle finger to strike the screens causing a laceration.
Standing on a bucket of degreaser to reach maint. board. Foot went through the top of the bucket & splashed other leg causing 2nd degree burns.

Employee was pushing refuse with a cat D8 dozer. He stopped to spot the refuse truck and started to get a burning, watering sensation in his left eye. He finished his shift and went to hospital and had a metallic foreign object removed from his eye.
EE called supervisor 3-12-05 that he had pulled a muscle & could return to work on 3/15/05. He also told supervisor he had pulled the muscle lifting a tire.
EE went to supvervisor on 1-8-05 & gave him a dr's excuse to be off til 1-15-05. ee came back on 1-15-05 wihtout a release. he was told he could not come back to work w/o release. ee came backwith a light duty release. hwm does not offer light duty. he then for the first time said that he hurt himself lifting a tire. he has not returned to work.
IN PREPARATION OF FUELING DOZER, EMPLOYEE STEPPED ON BLADE ARM TO DISMOUNT DOZER. EMPLOYEE SLIPPED & PULLED HIS HAMSTRING ON LEFT LEG. EMPLOYEE WENT TO DR 1/6/05 (FIRST LOST DAY).

Miner was using cable lug-all to line a chute. Tool slipped & struck miner in mouth and chipped a tooth.
EE WAS DOING MAINTENANCE WORK USING A PRY BAR. THE BAR SLIPPED CAUSING SUDDEN JERK ON ARM & SHOULDER.
MINER REACHED OUT TO GRAB THE HAND RAIL TO MOUNT A FRONT END LOADER AND SLIPPED ON A PATCH OF ICE. HE FELL TO THE GROUND AND FRACTURED THE MIDDLE FINGER ON HIS LEFT HAND.

MINER STRUCK SPROCKET SEGMENT ON DOZER TRACK WITH SLEDGE HAMMER. A SMALL PIECE OF METAL BROKE OFF, FLEW UP AND LODGED IN HIS CHIN. HE WAS TAKEN TO HOSPITAL WHERE THE METAL WAS REMOVED.
EE WAS ATTEMPTING TO DESCEND A FLIGHT OF STAIRS.HE SLIPPED ON AN OBJECT UNDER THE SNOW AND FELL DOWN THE STEPS RESULTING IN A SPRAINED SHOULDER.
MINER WAS WELDING ONT HE INSIDE OF SCALPING SCREEN. WHILE HOLDING HIMSELF IN POSITION, HIS LEFT FOOT SLIPPED OFF THE CENTER FRAME SUPPORT STRIKING HIS LEFT SHIN ON A SUPPORT BAR.

MINER WAS WORKING ON ROTORY BREAKER IN A KNEELING POSITION. WHEN HE ATTEMPTED TO STAND UP HE COULD NOT STRAIGHTEN HIS LEFT LEG.
INJURED WAS SEARCHING FOR A BOLT BY PLACING. HIS RIGHT INTO A SMALL AREA BETWEEN THE BELL HOUSING AND TRANSMITION OF A TRACTOR AND TRAILER MACK TRUCK, THE TRANSMITION SHIPPED PINCHING THE THUM B OF THE INJUREDS RIGHT HAND BETWEEN THE BELT HOUSING AND TRANSMITION CAUSING A BROKE BONE IN THIS FINGER.
EE WAS BURNING RUSTY PIPE WHEN SPARK WENT UNDERNEATH SAFETY GLASSES.

EMPLOYEE WAS GREASING DUMP TRAILER HE TOLD ME HE HAD BEEN ADJUSTING NOZZLE TO SEE IF GREASE WAS COMING OUT. HE THEN STARTED TO GREASE SLACK ADJUSTER ON TRAILER AND GREASE SQUIRTED IN HIS LEFT EYE. HE WAS TAKEN TO HOSPITAL WHERE THEY CLEANEDTHE EYE.
VICTIM SLIPPED WHILE DESCENDING STEPS TO CLEAN COAL TUNNEL. FELT PAIN IN BACK.
WHILE REMOVING A WEAR RING FROM A PUMP, THE RING FELL AND PINCHED THE LITTLE FINGER OF THE LEFT HAND BETWEEN THE RING AND THE SHAFT,
LIGHT FIXTURE FELL FROM CEILING(PRIOR DAMAGE TO HANGING DEVICE) STRIKING VICTIM CAUSING STOVED NECK WITH MINOR CUT TO NOSE AND LETE SIDE OF NECK.
WHILE USING A SLATE BAR TO ROTATE A SCREEN MECHANISM TO REMOVE TIMING PINS, THE BAR SLIPPED CAUSING THE SHAFT TO SWING BACK. THE PIN SNAGGED THE VICTIM'S GLOVE AND PULLED HIS HAND IN BETWEEN T HE PIN AND MECHANISM RESULTING IN INJURY TO THE 2ND FINGER OF THE RIGHT HAND.
WHILE INSTALLING CHUTEWORK, EMPLOYEE REACHED THROUGH STEEL WHERE ANOTHER WAS DRIVING AN ALIGNMENT PIN IN A HOLE WITH A HAMMER STRIKING THE INJURED EE'S FINGER.
EE COAL TRUCK DRIVER WAS EXITING HIS MACK CH 612 COAL TRUCK IN THE PARKING LOT AT THE TRUCK SHOP LOCATED AT THE PREPARATION PLANG AS HE PLACED HIS LEFT FOOT ON THE GROUND HE SLIPPED ON SOME IC E AND TURNED HIS LEFT ANKLE FRACTURING IT.
WHILE USING A BAR TO CLOSE A GATE ON A COAL CAR,THE VICTIM SLIPPED AND TWISTED HIS LEFT ANKLE.

WHILE REPOSITIONING HIMSELF IN A CONFINED SPACE,THE INJURED EMPLOYEE FELT PAIN IN HIS BACK DURING ROTATION OF HIS TORSO 90 DEGREES TO THE RIGHT.
DOOR 305 WAS BEING PLACED ON FINISHED PRODUCT PILE. WHEN LAYING WITH BOOM TRUCK, INJURED WAS GUIDING INTO PLACE WHEN DOOR CLOSED CATCHING FINGER BEHIND DOOR.
FOREMAN OBSERVED THE DRYER BELT SPILLING COAL DUE TO ICE BUILDUP ON THE TAIL ROLLER. HE PICKED UP A SLATE BAR, INSERTED IT IN FRONT OF THE TAILROLLER GUARD BETWEEN THE BELTS IN AN ATTEMPT TO CHIP OFF OF THE RUNNING TAILROLLER. THE TAILROLLER CAUGHT THE SLATE BAR AND IT KICKED SIDEWAYS STRIKING EE ON THE RIGHT FOREARM FRACTURING IT.

SHOVELING GOB TURNED AROUND HIT HEAD INTO BEAM,CUT EYE.
WHEN REMOVING A SMALL PORTABLE PUMP FROM A CEMENT SUMP IN THE BREAKER BUILDING, THE INJURED HAD THE PUMP OUT OF THE SUMP WHEN HE STEPPED INTO THE SUMP AND FELL ONTO HIS SIDE RESULTING IN TWO F RACTURED RIBS. THE SUMP OPENING MEASURED 16" WIDE X 4 1/2' LONG AND WAS 3 1/2' DEEP.
HAD RUBBER BOOTS ON, GOING DOWN STEPS AT PREP PLANT. THEY WERE WET, SLIPPED AND FELL HITTING BACK.
WHEN USING A WRENCH TO TIGHTEN A BOLT ON A MOTOR CLUTCH THE WRENCH SLIPPED OFF THE BOLT CAUSIN GTHE INJURED TO STRIKE HIS RIGHT INDEX FINGER ON THE MOTORS FAN BLADE CAUUSING A LACERATION WHICH REQUIRED 6 STITCHES.
INJURED WAS OPENING NEW PAINT CAN AND RAN HIS FIGNER ACROSS SHARP EDGE ON LID.
RUN KNEE (RT) INTO CHANNEL PAN SECTION.
INJURED WAS PUSHING A WHEEL BARROW, IT TIPPED OVER AND THE INJURED TRIED TO CATCH IT WHEN HE FELL INTO THE CORNER OF MAGNITE SEPARATOR AND CUT ARM.
INJURED AND ONE OTHER WERE TAKING FRONT COVER OFF OF ELECTRICAL BREAKER BOX, WHEN THE COVER CAME LOOSE AND FELL STRAIGHT DOWN ABOUT 1 FOOT AND LANDED ON THE TOES OF THE INJURED.
