The mine received a Self-insured Employer Decision on 11/29/2021 from a medical report dated 11/15/2021. The report identifies a 19.98% permanent partial disability from noise-induced hearing loss. Employee is currently retired.

The secondary counterweight inadvertently disconnected from the primary counterweight on the North side, which caused the South side chain to break. This allowed the secondary counterweight to fall to the bottom of the shaft and prevent operation of the hoist. The hoist and its structural components were not damaged from this occurrence.

Employee was standing at the Main North 3 Belt Drive area while spotting a splice. EE turned to walk away and EE's feet slipped out from under EE causing EE to fall. At approximately 10:58 AM the Prep Plant belts were shut down for a maintenance outage. At approximately 11:30 AM, this belt was observed to be on fire approximately 170' from the head roller and was extinguished at approximately 12:00 PM. Through investigation, a hot roller was presumed to cause the conveyor belt to ignite.

An unplanned roof fall was discovered in the Main North Intake #2 Entry at 139 block. The approximate dimensions of the roof fall area is 30' long x 16' wide x 7' above the coal seam. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

Employee was attempting to operate a track switch when EE felt a pain in EE's right shoulder. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. **This is a questionable injury. The Employee did not begin missing work until 2/03/21.**

While observing a mower on an embankment, the mower blade threw a rock which struck the injured miner in the right cheekbone. This accident was not the result of a compliance issue.

At approximately 4:20 PM the western slope of the Old Coarse Refuse was discovered to have 3-4 "hot spots". These areas were smoking/smoldering with no visible flames. This was potentially the result of spontaneous combustion, there were not any problems with mining machines, PPE, training or compliance issues.

An unplanned roof fall was discovered on the 6 North Haulage at 21 block. The fall was approximately 16' wide, 70' long and 12' high. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

The employee was running and jumped to touch hanging clothes in the bath house, when they landed they felt discomfort in left knee. It did not involve equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

The Wana Service Hoist primary counterweight became dislodged due to the guide rail becoming "sprung" and causing the rails to be out of gauge. There were no entrapments or injuries involved with the incident. It did not involve any compliance issues.

Employee was looking for a damaged top roller that EE was going to replace during a belt idle period. EE's shirt sleeve was caught by a top roller and EE's arm was pulled between the top belt and a roller causing abrasions to EE's left arm. It did not include any equipment or compliance issues. The employee was setting up the main breaker on BT800 locomotive when EE felt pain between the left index finger and thumb. It did not involve any equipment or compliance issues. The Kuhntown elevator was transporting three employees to the surface when the safety switch on the surface door failed, causing the employees to become entrapped approximately 240' from the surface. Once the safety switch was replaced the employees exited the elevator at 1:34am. It did not involve any injuries, equipment or compliance issues.

The employee was moving the loader cable out of the haul road on the 7W Section in the 2 to 3 crosscut at 6 block, when the employee felt pain in lower back. It did not include any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

Employee was operating a scoop loaded with a LW shield. EE lowered the bill but failed to release the winch. This caused the shield tip to raise. The shield tip caught the scoop canopy, caused it to raise and slide off the side of the scoop. The light caught the employee's hardhat forcing their head down and backwards. Employee was walking across the track to get in the clear when a wooden roof plank unexpectedly broke. This caused roof material to fall, striking head and neck. It did not involve any equipment or compliance issues. It also did not involve any job skills, proficiency, PPE or protective devices.

The employee was removing the connecting staple in a fitting that was connecting a 3/4" emulsion hose, when employee removed the staple the pressure had not been fully released & the hose began to whip. The hoist kicked the breaker 4' from the bottom as it was traveling down. While resetting power, employee attempted to jog the hoist before the system powered up. The PLC was not aware oil was still in the system, which prevented the brakes from properly setting and caused the hoist to drop to the bottom of the shaft. When the hoist dropped, three guide wheels were damaged.

Contractor employee was unloading jumpers to pull belt with, off the flat bed truck and claims that they felt pain in the stomach area. Cause of this injury was not using proper body mechanics and getting help. Compliance with rules/regs,required PPE, mining equipment/systems not a factor. Miner proficiency may be a factor. Diagnosed as a abdominal strain (No hernia).

The employee leaned a super post against the panline and turned to walk away when employee tripped and tried to catch self against the leaning post. The post then fell on employee's left forearm. The ee was attempting to free an obstruction from the rock dust hose by applying air pressure. The obstruction released, the employee lost control of the rock dust hose and was sprayed in the in the right eye with rock dust. It did not involve any equipment or compliance issues. EE was standing beside the mantrip as it was being elevated toward the rail, as it came down the mantrip overshot the rail and struck the employee in the left knee. Employee was standing on coal behind the continuous miner and tossed a ventilation tube toward the face. As EE tossed the tube, EE slipped and felt a pain in lower back. Employee was rock dusting along the belt, near the transfer, when a rock bounced off the belt and struck employee in the lower left leg. It did not involve any equipment or compliance issues. Employee was shoveling at the seal area. As ee twisted to empty shovel, ee lost footing, twisted knee and fell backwards. It did not involve any equipment or compliance issues. Employee was attempting to place the trolley pole on the wire and felt a pain in left shoulder. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. This is a questionable injury.

On 3/13/2019, the north-east side of the mine Impoundment experienced a loss of stability at the out slope in the area of sediment cell #2 between the top of the cap and the embankment. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

The Wana Elevator was transporting two employees to the surface when the north brake release solenoid shorted out at approx. 3:45pm, causing the employees to become entrapped until the coils could be replaced in the solenoid. Once the coils were replaced the employees exited the elevator at 4:26pm. It did not involve any equipment or compliance issues.

Employee was carrying a tool box when employee lost footing and dropped the tool box on the outside of right foot. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

An unplanned roof fall was discovered at 182 block in the #6 Entry of Main North. The fall was approximately 16' wide, 40' long and 12' high. The fall was most likely caused by gradual deterioration of the mine roof. No problems with mining machines, PPE, training or compliance issues.

Employee was using a pick to create a hole in the coal rib to anchor the belt tailpiece. As EE was swinging the pick, EE felt a pull in EE's right shoulder. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices. This is a questionable injury.

EE was laying coal onto a rough area of stockpile, when they started to bounce and failed to maintain control of the Pan. As they bounced they struck their head on the steering wheel, causing a laceration and a fracture to the nose. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.

The elevator was called to bottom and faulted just before it arrived. There were no occupants when this occurred. Through investigation, it was discovered that the braking contactor/resistor and door switch malfunctioned. Items were replaced and back in operation at approx. 7:00am. This elevator is not used as an escapeway.
