Employee was putting buckets on the elevator and a piece of ware plate broke loose from above which fell to the bottom landing on EE's leg/ankle causing a compound fracture and breaking the leg/ankle.

Dumping fine material with dump truck and closing hooks didn't open to lock down back door on dump bed. Employee put his hand in between door & bed of dump box when hooks opened up smashing little finger.
Employee was in route from the Tucker Hill Quarry to Highway 31 when he failed to negotiate a turn due to excessive speed and rolled his vehicle. Employee was not using his safety equipment(seatbelt) and hurt his elbow
Hopper door on railcar was stuck open; he put rail bar in the roller but it would not close so he stomped the bar with his foot and his boot was wet and it slipped off and hit him in the chin.

It was a windy night, worker was performing normal job duties and perlite blew through vent, had holes in goggles and went into right eye, flushed out eye but perlite would not come out.
EMPLOYEE WAS TRYING TO TAKE THE MUDFLAPS OFF BACK OF TRAILER, THE DRIVER OF THE TRAILER STARTED TO BACK UP & IT CAUGHT HIS RT. HAND BETWEEN FLAPS, HANGER TIRES AND LANDING GEAR. THUS RESULTING IN 3 SMASHED FINGERS, WHICH WERE JUST BRUISED & NOT BROKEN.
