While operating forklift, loading truck, reversed/backing out of truck - uneven surface (6"). Looking back over shoulder hit drop-off & strained back. Driver of van did not have axels forward making uneven surface from loading dock.

CLEANING OUT ROLLER MILL. DUST FELL DOWN AND FELL IN EYE BEHIND SAFETY GLASSES. DID NOT HURT UNTIL LATER THAT NIGHT, SO TRIED TO WASH OUT WITH EYE WASH. REPORTED TO SUPERVISOR NEXT MORNING. TRANSPORTED TO DOCTOR WHO REMOVED TINY PERLITE PARTICLE FROM EYE LID.
