While laying under a dump truck employee went to turn and reach for a cable and their foot got caught under the axel. The foot and knee didn't twist with the rest of their body, causing them to twist their knee. Employee did not report injury until 2/3/2026

Employee was taking a guard off of the screening plant conveyor that is a part of the sand system. When taking off the guard, right hand was between the framework. The guard went across right hand causing some skin to get scrapped off the top of ring and middle finger.
While exiting the CAT 980 loader, employee was using 3 points of contact and facing the machine as required. During the descent, they believed they were on the last step and attempted to place a foot on the ground. However, they were actually on the second to last step and missed the final step, resulting in a fall to the ground that rolled their ankle.
Employee was hosing the ground under the catwalk and a 3/4" rock fell from a pile and struck them on their shoulder.
Employee was holding a piece of angle and flipped it upside down to hold it in place to tack weld. As EE flipped it over, EE's wrist felt a pop and sharp pain that ran up to EE's elbow.

Employee was cutting a bearing race off with a grinder. EE forgot EE's safety glasses and a piece of metal came back and hit EE in the eye.
EE left an open drink by the tool room and went to work elsewhere. A second employee used Permatex Super Penetrant near the open drink and left to work elsewhere. When EE later took a drink, EE stated EE tasted the Permatex. Employee reported feeling nauseated and motion sick. Employee was taken to urgent care, where EE was evaluated and released with no restrictions.
Employee works as a mechanic at Delta. EE was driving a wheel bearing race into a wheel hub, for the tag axle, and was hitting the driver, using a MAC anti-vibe 48oz ball peen hammer. EE was wearing safety glasses and gloves. The hammer bounced and came back down, hitting EE's left pointer finger, with the shaft of the hammer. The hammer handle is manufactured slim.

When completing a daily inspection on the little impact crusher, the right door was opened and there was some dirt and rock on top of the door that fell into the hinge area and kept the door from opening all the way past center. As the material settled, the door closed unexpectedly. Employee had reached in prior and the door closed on the tip of right thumb.
The employee had previously used brake cleaner to clean the oil and grease from the area. After failing to remove the washer, the employee attempted to use a min-torch to heat the washer. When the employee ignited the mini-torch, the residual brake cleaner vapors ignited and cause a small "fire ball" that burned two fingers of the left hand.
Employee was lifting a 4' x 5' screen cloth to the elevated shaker screen. When the employee released the screen cloth, it moved as it settled into place, causing EE's hand to move with the screen as it settled into place. This caused the sheet metal web attached to the screen cloth to cut the outside palm of EE's hand.

Employee was making a hydraulic hose, using a die. When employee went to set the die on the hose, it slipped, and the die smashed the middle finger on employee's left hand.
EE was picking up 3 boxes of filters. The boxes were sitting on a piece of concrete about knee height. EE lifted all 3 boxes and felt a pain in groin area.
Employee was getting out of the water truck and slipped due to mud on EE's shoes. Injured right shoulder when EE grabbed the handrail.
Employee was laying on a creeper replacing a clutch brake. The tool slipped out of EE's hand, bounced on the ground and hit EE in the left eye.
While wearing Bouton Optical Clear safety glasses, grinding 3/4" thick piece of steel, secured in a vise, on a workbench, with a 4 1/2 inch Makita corded grinder, a small piece of steel ricochet behind EE's safety glasses and went into EE's right eye.

Employee was unloading two pieces of flat steel out of a pickup truck with an overhead crane. The chains slipped and moved to the center of the steel, causing it to slide and strike the employee's right wrist. Resulted in a contusion/sprain to wrist.
Was stepping up into CAT haul truck prior to entering the cab. Went to turn and boot did not turn with leg. Felt a pop in knee. Knee sprain.
Was welding a trailer. A spark came off and melted through employee's shoe and burned in between employee's toes.

Climbed down step ladder to retrieve a screw driver and reached up and grabbed rung on step ladder to climb back up and a hammer fell off the dump truck box and hit the top of right hand causing a displace fracture of middle Phalanx middle finger.

EMPLOYEE WAS USING A PRY-BAR WHILE REPLACING AN EQUALIZER ON TRAILER TO LOOSEN FITTINGS. WHEN APPLYING PRESSURE TO PRY-BAR WITH RIGHT HAND THE FORCE OF BAR SLIPPED AND CUT LEFT FINGER ON THE CORNER OF A HANGER ON THE TRAILER. EMPLOYEE REQUIRED TO HAVE STITCHES ON FINGER. NO RESTRICTIONS.
After installing a new floor for log washer chute, a two foot piece of angle iron was tack welded into place and needed to be removed. Tack was small and just needed to be wiggled to break loose. Employee wiggled and pulled back on the weld. When it broke free, the end of angle iron hit right cheek and caused a small laceration requiring two stitches.

Employee set up 8' step ladder under Auger on loam surface to clean out debris. Employee did not wait for fellow employee to secure the ladder and proceed to climb the ladder up to the third rung. The ladder shifted in the loam causing the ladder to fall and employee jumped to the ground. Ladder landed on employee's leg resulting in leg fracture.

Taking anvils out of samll impact crusher and a co-worker was using a sledge hammer on the anvils to get them out of impact. A piece of the cast iron from the anvil flew and hit employee in the face cutting his nose and right eybrow area requiring stitches.

Bent over to pick up a tow cable and felt sharp pain in lower back. Was stuck in the mud at silt dump site and the cat came over with the tow cable to pull him out. 1" cable dropped and when I bent over to pick it up off the ground I felt a sharp pain in the lower back.
On the 7th was pulling on a fire hose and twisted body while pulling. Hose stuck and it pulled him back jerking his back.
