Employee reports that has he was moving dimension stone rocks in a production area, two rocks pinched his right middle finger. The result was a fracture in his finger tip.

Employee mentions that he was placing processed stone on to pallets and felt a quivering in the center of his lower back before it gave out on 6/12/07. The employee thought he was simply tired and sore. After rest and over a week of time his back was still sore with pain extending down his right leg. Incident reported on 6/21/07 to a Foreman.
Employee was standing outside of his assigned CAT 771D Off - Road Haul Truck and mentioned that a gust of wind blew some fugitive dust around which entered his right eye. He reported the incident to a Foreman on Monday, 6/4/2007. He was treated by a physician and a corneal abrasion of the right eye was diagnosis. Employee was given antibiotics.

EE was reaching from the exit belt side of the rock splitter machine underneath of the veneer gauge to remove a stone and mentions that his right pinkie finger was smashed between a rock being processed by the machine and the veneer gauge. *NOTE* He was wearing his leather gloves at the time of the incident.

EE WAS IN PROCESS OF REMOVING DRILL STEEL FROM BOREHOLE WHEN HIS LEFT HAND BECAME CAUGHT IN FEED CHAIN/FEED SPROCKET ASSEMBLY. HE CUT OFF THE TIP OF MIDDLE FINGER ON LEFT HAND. EE WAS GIVEN FI RST AID TO STOP BLEEDING AND TRANSPORTE TO E.R. BY OUR ONSITE EMT.
EE WAS OPERATING AN AIR TRACK DRILL & WAS IN PROCESS OF CHANGING A DRILL STEEL WHEN THE CHAIN BROKE OPERATING THE UP/DOWN MOVE OF HAMMER & CAUSED THE HAMMER TO FREE FALL THIS PINNING EE'S LEFT HAND BETWEENT HE HAMMER & DRILL STEEL
EE WAS ENROUTE FROM 1/4" DUST PILE W/LOAD TO SHOPSITE IN PIT. AS HE ATTEMPTED TO TURN LEFT IN THE SE CORNER OF QUARRY, HE ROLLED THE OFF ROAD HAUL TRUCK ONTO HIS RIGHT SIDE
EMPLOYEE WAS IN THE PROCESS OF MOVING A HYDRAULIC DRILL ON TO A STRIP SHOT. THE LEFT TRACK CAME UPON A ROCK AND TILTED MACHINE TO THE RIGHT. HE TRIED TO BACK UP, BUT THE MACHINE CONTINUED TO THE RIGHT AND OVERTURNED AWAY FROM THE OPERATOR.WHEN HE REALIZED IT WAS GOING OVER HE JUMPED FROM THE MACHINE. HE HAD A BRUISE ON HIS RIGHT FOREARM AND RIGHT HIP.
EE WAS DRILLING SHOT HOLES AND DURING THAT PROCESS HE TWISTED HIS RIGHT KNEE. THIS WAS APPROX 2 MONTHS PRIOR HE SAID IT WAS NO BIG DEAL & CONTTO WORK. KNEE THEN BECAME INFLAMED UNDER THE KNE ECAP & WOULD SWELL UP. ON 23/25/99 HE DECIDED HEHAD BETTER GET MED ATTENTION

EE WAS IN PROCESS OF RETRIEVING A BIT & STEEL THAT HAD BEEN BROKEN OFF IN BOREHOLE. HE HAD HOLD OF THE STEEL & COUPLING USED TO RETRIEVE THE BROKEN STEEL & AS HE APPLIED DOWNWARD PRESSURE WITH THE DRILL HAMMER, THE STEELS SHIFTED PINCHING HIS FINGER BETWEEN THE STEEL & THE DRILL MAST. IT SEVERED HIS MIDDLE FINGER/LEFT HAND BELOW THE BASE OF HIS FINGERNAIL.

EMPLOYE EWAS USING A SHORT HANDLE SHOVEL BECAUSE OF ACCESS BEING LIMITED. THIS RESULTED IN HIM HAVING TO BEN DOVER. WHEN HE TURNED TO THROW A SHOVEL FULL OF DUST, HE STRAINED HIS BACK AND LO ST HIS FOOTING. THIS RESULTED IN LOWER BACK PAIN. HE WAS ABLE TO CONTINUE WORKING WITH THE HELP OF A BACK BELT.
