Frazier Quarry Inc · Caught in, under or between a moving and a stationary object

On 5/12/2023 at approximately 4:40pm EE was removing a trough roller from under a crusher plant. While EE was pulling the roller EE mashed right thumb between the roller frame and the frame of the crusher. EE was treated at the ER and received 2 stitches and a fracture to a bone in thumb. EE was put on restricted duty.

Miner was working the maintenance crew installing a belt on a conveyor. EE was threading a belt around a tail pulley. While pulling on the belt the miner strained left hand causing swelling and pain.

INJURED WAS DISMOUNTING BACKHOE AFTER HELPING REPLACE A BLOWN HYDRAULIC HOSE.THE INJURED APPARENTLY LOST HIS FOOTING WHEN HE STEPPED ONTO THE TRACK AND FELL TWO FEET TO THE GROUND,INJURING HIS NOSE AND BRUISING HIS FOREHEAD.AFTER RECEIVING MEDICAL ATTENTION,IT WAS LEARNED THAT HE RECEIVED STITCHES FOR A NOSE LACERATION AND HAD POSSIBLY BROKEN THE NOSE.
