Pit Foreman was climbing up on the off-side of a Volvo L-250 Rubber tire loader to tighten a bolt on the tension spring to scaler. While stepping to designated location, EE's right foot slipped and caused EE to fall to the ground where EE landed on their back. EE was transported to the hospital where x-rays revealed EE fractured the L1 vertebrate.

Employee was driving a CAT 775 rigid frame truck from stock pile. While being loaded the truck was hit with the bucket of 988 CAT loader. Employee said they felt something back pop. There was no immediate pain but over a short period of time they felt pain in their back.

On 9/19/2019 EE reported at 10:00am they exited a loader put on a harness and went up on to a conveyor to clear off some rocks from a belt and a screen feeder. EE said they tweaked their back. EE called to report incident at 1:45. EE told that they had been having issues with back at home and wasn't sure where the injury happened.

On 5/24/2019 at approximately 3:45pm. EE was power washing rubber tire loader when EE over extended the wand causing it pull back causing the wand to travel across EE's boot cutting boot and causing 2nd degree burn to right foot.

EE was trying to install a CB radio antenna on a pickup truck using a knife to cut zip ties. Knife slipped cutting the left index finger. Knife cut through the finger nicking the middle finger. EE went to emergicare who said the cut was too deep that EE needed to go to the emergency room. Received 4 stitches, antibiotics and tetanus shot.

EE was washing the cab of the 740 truck. While exiting the cab ee reported that ee was using three points of contact and foot slipped out. One foot was up on the second rung and the other on the ground. EE said that ee initially didn't feel any pain, but as the day progressed driving the truck ee back started hurting.

While working on screen conveyor at lime shed ee tried to catch a falling piece of pipe, that had been hit by manlift above ee to keep it from hitting another employee. Pipe slipped through ee hand causing laceration next to thumb.

Plant operator was helping with a screen change. EE was tightening bolts with right hand and had EE's left arm resting on screen. Being that EE had previously dislocated EE's shoulder in April of 2016, it was weak and again slipped out of socket. this is a reoccurring injury that did not result in lost time.

Employee was conducting a maintenance action on the DBT Crusher. Employee was helping with moving cribbing blocks from the tail pulley of feeder chain and was reaching up when the chain settled and shifted. When employee planted foot it slipped approximately 1 ft. Employee reached up and over head causing shoulder to dislocate.

Operator did not maintain control of 1999 dump truck. Truck drifted into ee car striking truck driver & pushing ee against the car injuring left leg. Truck was on a 3% grade at entrance road to quarry. Operator released the parking brake to back up without maintaining proper pressure on the primary foot operated brake. Extensive damage to ee car and injury to other ee

Employee was changing a grease hose when EE slipped and fell, injuring EE's left shoulder. EE was taken to an emergency and the Employee was treated at the hospital where EE was diagnosed with a separated shoulder and treated by mobilization. Employee was released to return to EE's next scheduled shift.

While backing up the truck to dump, employee stated that the truck downshifted, thus hurting employee's back. Employee was diagnosed with a pulled muscle and missed two days of work.

SNI was cleaning up after finishing changing the conveyor on our #3 belt. He was carrying a board back to the service truck. As he placed the board onto the back of the truck, the back of his right hand made contact with a belt re-grooving tool that was cooling- leaving a 2 inch laceration.

Said Named Individual (SNI) was preparing to replace the bags in the Sly Baghouse. He had just opened the door to the bag area, leaned down to latch the door when the wind caught the door. The door closed on SNI's left arm and hand. SNI lifted the door and notified foreman of incident. SNI was diagnosed with a contusion to his left forearm.

Windborne debris caused minor irritation to the right eye during land clearing operations which resulted in medical treatment and a prescription for eye drops to be issued.

As Contractor backed his tool truck, he struck the bucket of the Excavator. Injured EE was located next to the bucket and was pinched between the service truck and the 345B bucket. EE was transported by EMS to the Hospital. The injury resulted in a completely serparated Pelvis.

EMPLOYEE WAS INSTALLING A ROLLER ON A CONVEYOR & FELT A PULL/TIGHTNESS IN THE RIGHT CENTER OF HIS BACK AFTER PUSHING ON A PRY BAR FOR LEVERAGE. EE REPORTED FOR WORK THE NEXT DAY AS NORMAL & AGAIN ON 11/9/05, WHEN HE CLAIMS TO HAVE RE-AGGRAVATED HIS BACK WHILE SHOVELING MATERIAL FROM A CONVEYOR BELT. EE WAS SENT TO SEE A DR. ON 11/09/05.

EMPLOYEE WAS OPERATING A HAUL TRUCK ON 9/9/05 & A SMALL ROCK FELL FROM THE BED OF HIS TRUCK. HE EXITED HIS TRUCK & ATTEMPTED TO ROLL THE ROCK FROM THE HAUL ROAD WHEN HE EXPERIENCED A STINGING PAIN IN HIS GROIN AREA. HE IMMEDIATELY REPORTED THE INCIDENT TO FOREMAN. HE HAS BEEN INSTRUCTED IN PROPER LIFTING TECHNIQUES & IMPORTANCE OF ALLOWING LOADERS TO CLEAN UP SPILLAGE.

Due to an incident prior to working for the quarry, ee re-injured his lower back while moving flight bars at the DBT crusher. He reported his injury to foreman and went to see chiropractor.

OPERATOR WAS TIGHTENING BOLTS WITH ANOTHER EMPLOYEE ON THE CRUSHER WITH A MANUAL TORQUE WRENCH. THE TORQUE WRENCH BROKE AND CAUSED HIM TO STUMBLE BACKWARDS AND TWIST HIS ANKLE, LEFT LEG. HE WORKED THE REMAINDER OF THE SHIFT AND WAS SEEN BY A DOCTOR ON 5/27/04 DUE TO ANKLE PAIN.

DRIVER ENTERING DRIVERS SIDE OF TRUCK, SLIPPED ON STEP & SPUN AROUND, GRABBED HANDLE ON CAB, STRIKING HIS LT.ELBOW ON POST OF TRUCK SHED STRUCTURE. DRIVER WAS FOLLOWING 3 PT. CONTACT RULE WHEN INCIDENT OCCURED. HE WAS SENT TO DR. & RETURNED TO WORK SAME DAY.

TRUCK DRIVER WAS DESCENDING THE FRONT LADDER OF THE TRUCK. HIS FOOT SLIPPED OR HE MIS-JUDGED THE STEP AND FELL ABOUT 4 FEET TO THE GROUND, LANDING ON HIS FEET. WORKED THE REMAINING SHIFT BUT H AD TO BE SEEN AT DOCTOR THE NEXT DAY BECAUSE OF PAIN & STIFFNESS IN THE LEFT KNEE. LOST TIME. *NOTE* THIS OPERATOR WAS FACING THE LADDER PROPERLY WHEN THIS OCCURRED.*

MECHANIC WAS ASSISTING THE BOBCAT OPERATOR IN REMOVING THE BUCKET. AFTER THE PINS WERE RELEASED AND THE OPERATOR STARTED TO BACK AWAY, HE ACCIDENTLY MOVED THE CONTROL THE WRONG WAY CAUSING THE EDGE OF THE BUCKET TO LIFT. THE MECHANIC WASSTILL TOO CLOSE AND THE BUCKET EDGE STRUCK HIS LEFT LEG.

EE WAS CUTTING SECTIONS OF CONVEYOR BELT W/UTILITY KNIFE. HE HAD TO BEND OVER A LOT AND PHYSICALLY HANDLE THE SECTIONS. THE SECTIONS WERE 10' LONG AND 16" WIDE. THE REPETITION OF THIS MAY HAVE CAUSED THE STRAIN, ALTHOUGH EE IS NOT CERTAIN OF IT.

EE EMPTYING BIN INTO TRK FROM GR17 PLANT USED BRACE TO HOLD OPEN BIN & UNDER HANDLE. BRACE SHIFTED ALLOWING HANDLE TO FALL ACROSS HIS LEFT HAND THIS RESULTED IN NUMBNESS TO LITTLE FINGER & RING FINGER OF LEFT HAND. FAMILY DR PLACED BRACE ON ARM/HAND NO IMPROVEMENT UNDERWENT SURGERY 10/10/00 TO RELIEVE SUSPECTED PINCHED NERVE

WHILE CONDUCTING EXPLOSIVE INVENTORY OF ANFO, EE WAS MOVING 50 LB BAGS. WHILE MOVING A BAG FROM RIGHT TO LEFT EE FELT PULL IN BACK. EE REPORTED INCIDENT 1 WEEK LATER ON 4/10/00.

AT THE TIME OF THE INCIDENT EE WAS PREPARING TO INGRESS INTO THE CAB OF THE 965 AS AN OPERATOR. EE WAS STANDING WITH BOTH FEET ON THGE TRACKS, HE THEN PLACED HIS RIGHT FOOT ON THE CATWALK. WHI LE SHIFTING WEIGHT FROM LEFT LEG TO RIGHT LEG, HIS LEFT FOOT SLIPPED, TWISTING HIS LEFT KNEE. FOLLOWING MORNING WENT TO DR. OFF REST OF WEEK. DIAGNOSIS, KNEE SPRAIN.

EE WAS EXITING FROM AIR SEPARATOR VIA 8' ALUMINUM LADDER WHEN TOP STEP OF LADDER FAILED. EE WAS HOLDING ONTO A SUPPORT WITH LEFT HAND. WHEN STEP FAILED, EE STRAINED LEFT SHOULDER. EE WAS WEARI NG FULL BODY HARNESS, HARD HAT, AND SAFETY SHOES

DID NOT SHUT OFF AIR TANKS TIGHT IN PRESHIFT INSPECTION. HEARD AIR LEAK ON WAY TO PIT. STOPPED AND WENT TO TIGHTEN MORE, SLIPPED OFF DRIVER SIDE CATWALK. FELL APPX, 7 FT TO GROUND. LANDED ON R T SIDE.

BEEN RUNNING LOADER WITH ROCKBREAKER, COMPLAINED ABOUT LOWER BACK PAIN. ALSO RUN PIT LOADER.

WAS GRINDING WELD OFF PIN AT BOTTOM OF BUCKET TO WELD BACK WHEN HE TOOK OFF GOGGLES TO PUT SAFETY GLASSES BACK ON SOMETHING GOT IN RIGHT EYE.

WAS SWINGING SLEDGEHAMMER TO BREAK ROCK IN FEEDER. WHEN TWISTING SOMETHING SNAPPED IN NECK AREA.

AT END OF SHIFT DUMP TRUCK DRIVER WAS WALKING ACROSS COMPANY EE PARKING AREA WHEN HE STEPPED ON A STONE. TWISTED HIS RIGHT ANKLE HE HAS TORN LIGAMENTS IN HIS ANKLE.

WAS UNFASTENING BELT IDLERS FROM SKID. PULLED OUT TAPLES & IDLERS HIT TOGETHER & SOMETHING CAME OFF IDLER HIT RIGHT EYE.

OPERATOR OPENED CRATE TO TAKE OUT RETURN IDLERS TO INSTALL ON CONVEYOR CUAGHT SMALL FINGER BETWEEN IDLER AND CRATE.

CUTTING OFF OLD IDLER SCRAPER TO BE REPLACED WITH NEW STYLE.OLD SCRAPER WAS CUT.SLAG WAS HOLDING IN PLACE WHEN EMPLOYEE WENT TO LOCK PLIERS ON IT,IT FELL ON FOOT.

WHILE HELPING TO LOAD A SHOT, HE STARTED SHAKING, FELL TO THE GROUND BREATHING HARD AND FOAMING AT THE MOUTH. APPARENT SEIZURE.

I NAME WAS GOING FROM NEW OFFICE TO OLD OFFICE HAD HEAD DOWN HIT HEAD ON CORNER OF PHONE BOX

TURNED OPEN LOADER DOOR TO TELL HAUL TRUCK DRIVER WHAT TO DO WITH LOAD. WHEN STEPPED OUT OF LOADER DOOR, WAS STRUGHT WITH SHARP PAIN IN LEFT SIDE OF CHEST.

PULLING BRUSH AND CEDARS THAT HE CUT DOWN

WELDING HANLDLE ON FLOPGATE, TO REPLACE HANDLE THAT THROWS THE DIRECTION. A LARGE PIECE OF SLAG FELL IN BOOT.

I NAME HAD PARTED THE EUCLID R-35 TRUCK HE WAS OPERATING AT THE PRIMARY CRUSHER H OPPER IT WAS LUNCH TIME & THE PRIMARY WAS SHUT DOWN & ITS OPERATOR WAS AT LUNCH I NAME HAD BEEN OBSERVED AT AP PROX 11:50 GREASING THE TRUCK AT NOON HE WAS FOUND AT THE BASE OF THE PRIMARY CRUSHER HOPPER HE APPARENTLY SLIPPED WHILE CROSSING THE DUMP HOPPER & FELL 18-20FT TO THE GROUND GREASE GUN WAS F

HE WAS DRIVING A STOCKOUT TRUCK (EUCLID). HE STOPPED UNDER THE BINS AND WAS GETTING OUT. AS HE STEPPED DOWN THE LADDER ON THE TRUCK TO THE G ROUND, HE STEPPED ON A LAARGE ROCK AND PULLED THE LIGAMENTS IN THE RIGHT FOOT.
