Accident type, without injuries

AT THE TIME OF THIS REPORT WE BELIEVE A LARGE VOLUME OF ROCK HAS BURST FROM ABOVE THE PERIMETER HIGH WALL. THE BURST MATERIAL & RESULTANT SLIDE EXTENDS INTO THE TREE LINE ABOVE THE QUARRY RIM. NO INJURIES HAVE BEEN EXPERIENCED AND NO EQUIPMENT DAMAGED AS A RESULT OF THIS INCIDENT. FURTHER INFORMATION WILL BE AVAILABLE ON INSPECTION AT 12-2-2005

Fall from scaffolds, walkways, platforms

While checking oil on loader stepped back and fell off elevator platform.

Contact with hot objects or substances

WELDING OVER HEAD, PULLED SHEILD UP HITTING WELDING SLAG, WENT OVER PRESCRIPTION GLASSES. HOT SLAG IN LEFT EYE.

Fall onto or against objects

Employee was getting out of his service truck #59-152 when he slipped on the ice. He struck his back on the side of the truck.

LOADER OPERATOR WENT TO GARAGE WITH LOOSE CUTTING EDGE, MECHANIC WAS IN THE BACK OF THE GARAGE. OPERATOR PICKED UP AN IMPACT WRENCH TO TIGHTEN BOLTS & PUT HIS HAND UNDER EDGE TO HOLD BOLT WITH SHARP EDGE. BOLT SPUN & CUT HIS FINGER WITH NO GLOVES ON.

Over-exertion in pulling or pushing objects

Manually pulling product hose (50-80lbs.) out of boreholes 40-60 tmes a day, hurt back/neck.
