Fall to the walkway or working surface

EE arrived at the work site prior to the start of his shift. EE proceeded to head towards the mechanic shop EE parked his car in a location that had been plowed last week it was not the designated parking area. The EE got out of his car and proceeded to walk over an area of accumulated snow and ice. While doing so lost his footing and fell on his side.

Caught in, under or between a moving and a stationary object

EE used the fork lift to place a steel wear plate on an excavator bucket to be welded. The plate of steel was placed on the lip of the bucket and needed to be adjusted into place. While pushing into place by hand EE pinched his right thumb causing a laceration requiring medical attention.

Struck by... (Not Elsewhere Classified)

The point (tip) of the rock breaker hammer was jammed in-between two of the fingers at the primary feeder. The EE placed his hand on one of these fingers. The rock breaker hammer moved down and struck the back of the EE's hand. We brought the EE to get X-rays on the hand which came back negative - nothing broken. Hand was bruised.
