Employee was shoveling off spill material by 7 foot crusher and sustained lower back pain.

Struck against stationary object

The Employee was washing down the primary concrete pad when the nozzle of the hose blew apart. The hand still holding the hose sent employee's hand to strike an I-beam.

Employye was using a utility knife to cut a piece of grease line off a bulk roll, the blade of the knife came out of the holder a cut the employees right thumb(employee is left handed) . The employee recieved three stitches in his right thumb and returned to duty.
