Struck by flying object

Employee was using a chisel to knock the bearing collar off pin on 992 loader. When he struck the chisel with a hammer a metal sliver broke off the chisel & went into employee's forearm.

EMPLOYEE WAS HITTING ON TRACK PIN WITH A 16LB SLEDGE HAMMER WHEN A PIECE OF THE PIN BROKE LOOSE. THE BROKEN PIECE HIT HIM JUST ABOVE LEFT KNEE, LODGING NEAR HIS FEMUR BONE. (NOTE: THIS WAS INITIALLY CHARGED AS A FIRST AID CASE, HOWEVER 4 DAYS LATER HIS DOCTOR DETERMINED THAT METAL NEEDED REMOVED)

EE informed supervisor. toehr on that he tripped over a shank, causing him to fall on another shank, injuring rt. side and lower back.

EMPLOYEE WAS PREPARING TO REMOVE UPPER SEGMENT BOLTS ON D10 DOZER WHILE POSITIONING AN AIR TORQUE WRENCH SOCKET ONTO ONE OF THE BOLTS WITH HIS LT. HAND, THE TORQUE WRENCH TRIGGER INADVERTENTLY STRUCK THE SEGMENT CAUSING THE WRENCH TO TURN ON. FINGERS WERE PINCHED BETWEEN THE TORQUE WRENCH BRACE & THE MACHINE SPROKET.

EE DISMOUNTED TRUCK LADDER TO LOWER PLATFORM (LOCATED ON BUMPER). HE UNHOOKED THE BOTTOM ""SWING DOWN"" STEP. AS EE PUSHED ON THE PLATFORM STEP WITH HIS LEFT HAND, THE STEP LIFT CHAIN PINCHED HIS RIGHT MIDDLE FINGER.

ON 5/25/05 EMPLOYEE LEFT WORK COMPLAINING OF BACK PAIN. ON 6/30/05, MRI REPORT WAS RECIEVED SHOWING SPINAL STENOSIS, SPONDYLOLYSIS AND DEGENERATIVE CHANGES HAD OCCURRED TO HIS BACK
