Struck by falling object

Mechanic was loading track pin into back of service truck when he dropped it on his left foot.

Over-exertion in wielding or throwing objects

Bending metal plate on truck bed, while swinging the sledge hammer, something in lower stomach pulled.

Fall from machine

Stepped off catwalk of dozer while steam cleaning, falling to ground.

On Oct. 11 & 16, employee went to doctor complaining of constant pain radiating down both legs. In an effort to locate the problem, on Oct. 16 employee was told to have an x-ray but failed to do so. On Nov. 4 due to back pain he had neck/back x-rays taken (results normal). On Dec. 13 employee continued to complain of pain so doctor scheduled an MRI. Report received Jan. 4, 2008.
