Over-exertion (Not Elsewhere Classified)

Employee was exiting the cargo area of his company assigned Sterling Acterra 4900 service truck after retrieving additional tooling, he slipped and twisted his right ankle.

Struck against a moving object

Employee stated that he suffered a jarring injury to his neck while operating a 685 Dresser Haul truck. Medical records concerning this event were not made available until 11/23/10.

Employee jarred his neck while dumping a haul truck. Note: Employee did not begin missing work until 6/22/10.

Fall to the walkway or working surface

Employee tripped/slipped and fell while dismounting a portal bus in the parking lot, injuring his right shoulder. Note: Employee did not start missing work until 6/23/10. Full medical records were not available until 6/29/10.

Struck against a moving object

Employee stated that his loaded haul truck slid into the ditch along the haul road during icy road conditions and he strained his lower back while holding on to the steering wheel. He finished his shift on the day of the incident, returned to work the next day and completed that work shift. Medical records were obtained on 3/22/10. Incident is under investigation.

EE was installing boom back in saddle on the service truck, winching cable up when the hook caught on the bumper of the service truck, came loose and hit the EE in the head over top left eye.

A small piece of wire penetrated the back of the employee's left hand while he was putting his tools away. The wire was surgically removed requiring (3) sutures.

Employee pinched his right hand in the drill entrance door, resulting in a laceration that required (4) sutures.

Inhalation of radiations, caustics, toxic and noxious substances

Employee alleges to contracting coal workers' pneumoconiosis as a result of his employment with Apogee Coal and was granted a 10% award by the WV OP Board.
