Employee slipped while mounting truck, injuring his shoulder.

Fall from machine

Employee was injured when he jumped from the boarding deck of a 685 haul truck.

Employee stated that he backed his dozer over a rock and jarred his back. Employee did not begin missing work until 9/26/12.

Employee was changing a hydraulic hose on a Caterpillar D8 Bull Dozer, in the process of tightening the ""C"" flange clamp on the SAE Code 62, Caterpillar, 4-bolt flange, his wrench slipped causing him to hyper-extend his right arm, he felt a pop in his right shoulder tearing his rotator-cuff/the rotator cuff tendon/glenohumeral joint

Employee was injured when the dozer he was operating overturned.

Employee was walking along a drill bench when he stepped on a rock and injured his left foot. Employee did not begin missing work until 5/13/12.

Employee suffered a torn meniscus tendon in his right knee while dismounting a haul truck. Employee did not begin missing work until 4/7/12.

Employee slipped while attempting to mount a D11 dozer and injured his left shin. Employee did not begin missing work until 1/25/12.

Employee was removing hydraulic cap bolts (cap was pressurized with oil) and one came loose. The hydraulic pressure cut his work pants and skin on his right knee/shin. He went to MedExpress on 7/29/12 and was released to modified duty; no bending stooping or kneeling until follow up appt 8/4/12.

Over-exertion in lifting objects

Employee was unloading parts from the back of a truck when he felt a pull in his lower back and down his left leg. He declined medical attention initially. On 3/19/12, he was experiencing back pain and went to see a chiropractor for treatment.

Absorption of radiations, caustics, toxic and noxious substances

Employee was executing repairs to a Komatsu PC5500 hydraulic excavator, during the course of the repair the Automatic Fire Suppression systems was activated and discharged fire extinguishers, employees were covered in the fire suppression material resulting to irritation to eyes and respiratory system

Employee was executing repairs to a Komatsu PC5500 hydraulic excavator, during the course of the repair the Automatic Fire Suppression systems was activated and discharged fire extinguishers, employees were covered in the fire suppression material resulting to irritation to eyes and respiratory system

Inhalation of radiations, caustics, toxic and noxious substances

Employee alleges to contracting coal workers' pneumoconiosis as a result of his employment with Apogee Coal and was granted a 10% PPD award by the WV OP Board.
