Accident type, without injuries

The entry at the headgate of the longwall caved in during the production shift. The top at the cave site was loose laminated shales with coal spars. Those contributed to the fall. The cave was measured at 8 ft high x 17'-18' wide x 15' to 20' long. The fall has been cleaned up.

THIS ROOF FALL WAS FOUND BY THE WEEKLY EXAMINER. IT WAS 26 FT WIDE, 20 FT LONG, AND 9 FT HIGH. LOCATED IN AN OUTBY AREA OF THE MINE. THE FALL IS IN AN INTERSECTION AND FELL ABOVE THE BELT LINE. SOFT LAMINATED SHALE CONTRIBUTED TO THE FALL. THIS AREA IS NOT NORMALLY TRAVELED AND IS NOT A WORK AREA. (7 FT BOLTS)

The roof in #2 entry between #27 and #28 x-cut fell for approx. 28 feet long 6 ft to 9 ft high and 14 ft wide. The roof was fully bolted and screened. Loose and Fractured roof contributed to the unexpected fall. the location is in the 5th left longwall bleeder return was found on weekly examination of the area.

THE EMPLOYEE WAS REMOVING A BROKEN FITTING FROM THE ROOF BOLTER. HE WAS PUSHING ON A WRENCH WHEN HE HEARD A POPPING SOUND & FELT A SHARP PAIN IN HIS RT. SHOULDER. THE EE RETURNED TO WORK IN HIS REGULAR JOB & LATER HAD AN MRI DONE & SURGERY ON 01/19/06. FIRST DAY OUT 1/09/06.

Struck by falling object

The employee was loading 16 foot bacon strips onto the bolter when a partial bundle of 9 foot mats fell off of the cab of the bolter. The mats struck him in the back and pinned him in the bolter's walkway.

THE EMPLOYEE AND CREW WERE EXTRACTING AN OVERCAST WALL. THE OPERATOR HIT THE WALL WITH A WAGNER BUCKET. THE FRAMEWORK THAT WAS BUILT INTO THE WALL POPPED OUT AND FELL TOWARD THE EMPLOYEE. HE TURNED TO RUN AND WAS HIT BY THE FRAME FROM BEHIND. IT DROVE HIM TO THE GROUND. LOST TIME ACCIDENT; FIRST DAY OUT 6/17/05.

The employee was pulling down phone cable that had been bolted to the roof. As he was pulling he felt a burning sensation in his right shoulder. The employee saw the doctor on 08-30-05. Lost time accident.

THE EMPLOYEE WAS MOVING THE MINER CABLE IN THE WORKING FACE, WHEN A RIB SLABBED OFF IN BETWEEN THE RIB PINS AND STRUCK HIM IN THE RIGHT LEG AND HIP. HE TORE THE ACL TENDON IN HIS RIGHT KNEE. HE IS SCHEDULED FOR SURGERY 08/19/05

EE WAS SHOVELING MUCK AND ROCK OFF OF BC-102 BELT LINE. HE SAID HE FELT A TWINGE IN HIS LOWER BACK ON THE LEFT SIDE. HE WENT TO THE DOCTOR WHO PLACED HIM ON LIGHT/RESTRICTED DUTY 5/11/05.

EE WAS SHOVELING COAL AND EXPERIENCED BACK PAIN. EE ALSO COMPLAINED OF BACK PAIN ON 11/17/04 AND 12/17/04 AND ON ALL OCCASIONS THE EE RETURNED TO REGULAR JOB. ON 02/23/05, EE WAS PLACED ON RESTRICTED DUTY AND THEN ON 03/02/05 EE BECAME A LOST TIME ACCIDENT

The employee was loading belt structure into a trailer. He pulled a top roller out of a pile of rollers and felt a pop in his left shoulder. He was seen at the hospital and released to light duty.

THE EMPLOYEE HAD BEEN ROOF BOLTING AND COMPLAINED THAT HE WAS EXPERIENCING PAIN IN HIS RIGHT SHOULDER. HE WENT AND SAW A DR ON 8/11/05 AND WAS GIVEN A LIGHT DUTY RELEASE AND RTW 8/12/05

The employee was rock dusting underground when the dust line surged and threw him into the rib. He fell and landed on a pile of coal or his SCSR.

EE WAS ROOF BOLTING WHEN HE GOT HIS INDEX FINGER CAUGHT BETWEEN THE EXTENSION AND DRILL STEEL OF THE ROOF BOLTER. STITCHES WERE REQUIRED TO CLOSE THE LACERATION. RESTRICTED DUTY 6/27/05.

THE EMPLOYEE WAS JACKLEGGING THE RIBS IN B-MAINS. HE WAS STANDING ON THE HANDLE OF THE JACKLEG AND WAS DRILLING AT A HEIGHT OF ABOUT 12 FEET WHEN THE PIG TOE SPUN AND THREW THE EMPLOYEE TO THE GROUND. HE LANDED ON HIS BACK ON A ROCK. RESTRICTED DUTY 6/15/05.

EE WAS OPENING A DOOR UNDERGROUND SO A TRACTOR COULD GO THROUGH. THE TRACTOR OPERATOR STRUCK THE DOOR AND THE EE'S HAND WAS SMASHED BETWEEN THE DOOR AND THE RIB. THE EE WENT TO THE DOCTOR, X-RAYS WERE NEGATIVE AND NO STITCHES WERE REQUIRED. EE WAS PLACED ON RESTRICTED DUTY 5/13/05.

Struck by flying object

The employee was running the shearer when a piece of coal blew out of the face and struck him in the bottom lip and cracked his upper tooth. Sutures were required for the laceration to his lip and he will see a dentist for his tooth.

WHILE THE EMPLOYEE WAS ATTEMPTING TO UNHOOK A WINCH HOOK, OFF OF THE WAGNER, THE OPERATOR HIT THE WINCH LEVER WITHOUT GETTING THE SIGNAL FROM HIM AND HIS FINGER WAS SMASHED. 1 STITCH WAS REQUIRED TO CLOSE THE LACERATION

THE EMPLOYEE WAS CLEANING OUT THE D-WATERING TANK AND LOST HIS BALANCE. THE WATER HOSE GOT WRAPPED AROUND HIS RIGHT HAND WHEN HE FELL AND TWISTED HIS WRIST

THREE EMPLOYEES WERE WORKING TO PULL THE TAILGATE DRIVE BACK. THE GOB PLATE WAS ATTACHED TO THE SHIELD WITH CHAINS. AS THEY STARTED TO PULL, ONE OF THE CHAINS BROKE AND STRUCK EE IN THE FACE, CAUSING A FRACTURE TO HIS CHEEK AND A LACERATION WHICH REQUIRED STITCHES. REPORTABLE INJURY.

The ee was cutting open a box of tuff stuff with a razor knife. The knife slipped out of the container and cut the ee just above his left thumb. Stitches were required to close the laceration. He returned to his normal shift 3-21-05.

THE EMPLOYEE WAS CUTTING A DUST STRAP WITH A RAZOR KNIFE. THE KNIFE SLIPPED & HE CUT HIS LEFT RING FINGER. THE EE FINISHED OUT HIS SHIFT & THEN HAD FIVE STITCHES TO CLOSE THE LACERATION IN HIS FINGER.

THE EMPLOYEE WAS ON A LADDER RAISING THE BELT LINE WHEN SHE REACHED FOR THE BELT RAIL & SLIPPED. AS SHE FELL OFF THE LADDER HER RT. HAND GOT CAUGHT IN THE COME-A-LONG CHAIN. SHE FRACTURED HER RT. 3RD FINGER. THE EE RETURNED TO HER REGULAR SHIFT.
