At approximately 10:35pm a Roof Fall was observed by an out-by employee in B-Mains number 5 entry between xc 13 to xc 14. The Fall measured 25' in length, 16' wide, and approximately 6' to 7' in height.

A roof fall occurred at x-cut 14 between entry #2 to entry #1 in the 13th left beltline above the anchorage of the fully grouted 9' headed rebar bolts.
IE was walking into a ditch when IE stepped on a loose rock rolling left ankle causing a fracture.
IE was washing down the slot storage when IE slipped on the ice causing IE to land on right hip and right elbow causing an injury to right shoulder. The next regularly scheduled shift was 01/20/2020 which the IE was not able to work.
IE stepped into a broken wheel chock that EE did not observe on the mine floor causing a sprain to EE's left ankle.
IE left approximately 10' of steel in the roof while checking suction placed left hand on drill head steel came out of the roof striking IE's left ring finger causing a fracture.
IE slipped exiting the hourly showers striking their head on the floor causing a laceration to the back of head the laceration required 11 staples to close the wound.
