A roof fall occurred in the crosscut #48 of 7th left section. The fall was above the anchorage point. The fall was estimated at 17' wide, 20' long & 20' high. The fall is a result of loose laminated shales. This was the case above the anchorage zone also. The fall was cribbed off & will not be cleaned up.

The roof in cross-cut #26 of B-Access fell approx 17' wide x 8' high. This fall is a continuation of another fall. The area is timbered off on all accessible sides. The fall was above the anchor zone.
At approximately 1 minute after midnight the roof in the face of #1 to #2 entry x-cut fell. The fall was approximately one foot above the anchor zone of the bolts. The fall was approximately 17 ft wide, 8 ft high and 20 ft. long. The fall was part of an unbolted cut and two rows of supported roof. The roof bolters witnessed the fall from a distance.
At approx 5:15 am on 7/18/06 The roof in 7th left at crosscut 55 between No 2 & 3 entry began to fall. At 5:30 the roof was still falling and continued to fall as a dribble. At 6:30 am the fall was above the anchorage zone. The roof fall 14 ft wide 16 ft long and 8 feet high. The area was being rebolted with 12in cable bolts when it sarted to fall.
At Approx. 7:15 pm roof fall occurred in the No 3 entry of the 5th left longwall tail gate. Access was blocking tailgate travel way and restricted the ventilating air current. The blockage plan was put into effect.
The employee was working in the longwall section on the panline when a rock fell from the roof and struck him in the back of the neck and pinched his left forearm on the panline.
EE was cutting mesh out. He was sitting in the mustang skid steer when a rock fell from the roof. The rock glanced off the mustang skid steer bucket and fell into the cab, hitting the EE in the right knee. MSHA restricted duty 11/17/06.
The EE was standing on the cab of the shuttle car to hang a laser on the roof. When he went to get off, his boots were muddy and he slipped and slid off the shuttle car, scraping his back on the way down on the corner of the shuttle car. Abrasions and bruising down the middle of his back. ***MSHA RESTRICTED DUTY***3/20/06
The employee was roof bolting when the drill steel became stuck by loose rocks in the roof. The employee was pulling on the drill steel to loosen it when he strained his left shoulder.
The employee was tramming the shuttle car in the section when she hit a pot hole. She bounced up and hit her head on the cab causing pain in her neck and shoulders. She was placed on restricted duty 01-10-06.
Employee was using a grinder to clean up a torch cut on a conveyor bent. The grinder kicked back and struck the employee's cheek. Employee was wearing proper safety equipment (shield, safety glasses, & gloves).
The employee was getting out of the mantrip underground when he hit his head against the rock dust bag holder. He cut his left eye on the brow. Five sutures were required to close the laceration. **MSHA reportable injury**
While employee was operating the shuttle car, small rocks were coming off the tail of the continuous miner. The employee leaned forward when a bigger rock bounced off the cab of the shuttle car and hit him in the upper back and ribs. The employee requested to see a doctor on 6/13/06 and was diagnosed with a rib fracture. **MSHA reportable injury**
EE lifting utility trailer off the hitch of the hauler with a handy-man jack. The handel slipped out of his hand and struck him in the right side of the rib cage. EE went hospital on 3/28/06 because he continued to have pain. He fractured rib on the right side.EE returned to regular duty.
The EE was using a come-a-long to lift the center tube on the Peterson Filters. The hook on the come-a-long slipped 1 link and pinched his palm between the hook and the chain it was hooked on. The EE received 8 stitches to close the laceration on the palm of his right hand. Released back to full duty.
