Struck by falling object

Employee was on a ladder helping another employee take down a come-along. The come-along slipped and fell while they were taking it down. The employee caught the heavy come-along with left hand. The weight made their left shoulder pop. The employee had surgery on 10/15/2025

Fall from ladders

IE was attempting to hang a 6 ton come along to the roof, IE was standing on a 6ft ladder, when the ladder slipped out from under employee. Employee fell breaking left leg and left collarbone.

Over-exertion in lifting objects

Employee was loading bolt resin into the bucket of the underground scoop and strained wrist. IE was put on restricted duty on 10/8/2025 for this injury.

Struck by... (Not Elsewhere Classified)

IE was setting a 9ft bolt and the long wrench broken at the chuck, the head of the wrench cut IE left arm. The IE laceration needed 12 stitches.
