Carrying a bucket of tuff stuff from kithen to shuttle car, slipped on slick floor spraining right knee.

While loading cleaning supplies from the warehouse to a pickup, while walking felt something pop in her foot. After she left work she went to see her physician were it was discovered that she had a broken bone in her right foot.

Right knee has been stressed from weak left knee. Going through a man door at xc 43 planted left foot turned in though door twisted right knee.

Loading cable bolts from a Wagner scoop to the roof bolter the employee strained his back. This employee originally injured his back in January of 2012 did not seek medical attention until June 19th of this year that is when it became a reportable accident.

Pulling lift cylinders down to the roadway from beltline to roadway.

Packing an acetylene bottle from one location to another, set the bottle down to adjust it, felt pain in his lower back.

Struck against stationary object

Hauler door swinging shut went to catch it and punched the door unintentionally in an attempt to catch it. The accident happened on 10/12/2012 took EE to the doctor on 10/23/2012 after he complained of his hand still hurting where they found a fracture of the right pinky finger.

Fall onto or against objects

While walking employee slipped on wet metal and fell into the base for the head roller.

Caught in, under or between a moving and a stationary object

Removing scraper box from head of BC-107 belt conveyor when the box slipped pinching the employee's right hand, which gave him a pressure laceration to his right middle finger.

Over-exertion in pulling or pushing objects

No Accident time determined. Employee allegedly strained his right knee while pulling a 14/3 electrical cable. This alleged accident was not reported to his supervisor as required. During investigation of alleged accident, his co-worker was unaware of any incident by employee. Operator concluded no incident occurred. Employer made aware of alleged incident on 6/29/12.

Over-exertion (Not Elsewhere Classified)

Roof Mesh standing up against rib started to fall over, the employee allegedly tryed to stop the mesh from falling by putting his shoulder up against it. We do not believe this to be a work related injury , and is under investigation by Rock Wood Casualty.
