Return off the longwall was being re-habilitated due to upcoming longwall move. A roof fall occurred inby the area of rehibilitation for an area of roof 20 ft. long x 11 ft wide and 4 ft in height fell. There was still a four foot walkway that had been supported that did not fall.

All incidents in 2008
Miner alleged that he slipped while walking in entry way and felt pain in his back Was not reported to supervisor for two days later. The miner had been previously injury in mining accident in Utah in May, 2007, resulting a back injury. Investigation found this to be a reaggrevasion of already injured back. This is being classified as an occupational illness.
Employee was working on top of feeder (030101) 4th right section. When employee was getting down from the feeder he jumped the last two feet, lost his footing and hyper-extended his left shoulder. Employee is a mechanic and carries many of his tools on his mine belt which contributed to the amount of weight pulling on his body. Injury has now required surgery.
Employee was packing a rock over the stage loader to put on the pan. He slipped and fell. Landed on his stomach pinching hand between rock and panline. Finger on left hand was cut requiring five stitches.
Stepped up on step stool when installing windshield. Step stool slid out from under my feet. Caught self from falling and hurt back.
Employee was getting into # 1585 service truck when he heard a pop noise and felt pain in left knee.The employee did not have surgery on his left knee until 03-24-2009 this is the reason for reporting now.
Employee was working on the belt line in muddy conditions. There was pipe in the walk way, the employee was walking across the pipe when his foot slipped and was caught between two pieces of pipe. His body rotated and his left Knee did not which cased a tear to his maniskus. As time progressed this injury worsened and requiring surgery, making this injury lost time.
After employee finished shift, he left in personal vehicle and slid on icy spot and wrecked personal vehicle. Although, Bridger does not agree this incident as reportable, Bridger is reporting in good faith
While grabbing 8"" steel pipe with left hand to position the pipe for fitting with the other pipes, the miner felt a strain i his lower left back region. Miner did not miss any work and has been placed on restricted duty.
Injured miner was carrying a roller along with a fellow miner. The roller got hung up on injured miner's coat causing a poor carrying position causing pain in the lower back. Injury happened on 6-25-08 but no restricted duty until 8-26-08.
While tramming the miner around a corner the cable was pulled down from roof striking employee in the face cutting eye brow over left eye.
Employee was moving a steel shaft with a forklift. The employee got off the forklift to adjust the cribbing, the left fork kicked up and struck the employee in the left shin. The employee recieved eight stitches.
Employee was unloading 6' aqua pipe off of the haulage equipment. He was located on top of the vehicle. The pipe started to roll, hitting the employee and knocking him to the ground. He was taken to the hospital where he received 5 staples to repair a wound to the back of the head.
Removing old belt roller. Roller was hard to get out so he was tapping it with a hammer. Employee tried to grab roller when another employee hit his finger with a hammer causing a laceration to his fingers which required stitches.
