Accident type, without injuries

Roof Fall in the #3 entry, crosscut #10,from crosscut going inby, of the 5th Left longwall section. Approximate dimensions of the roof fall: 60 ft (length) 8 to 15 ft. (high) and 18 ft (wide). Exact time of the roof fall is unknown.

Over-exertion in lifting objects

Employee was lifting a belt roller, the roller slipped and while catching the roller they felt pain in the shoulder and arm. Employee was referred to an orthopedic doctor and following an MRI review (09/18/2018) it was determined they required surgery (11/01/2018) for the injury.

Over-exertion (Not Elsewhere Classified)

Contractor employee was loading belt structure onto a trailer. As ee stepped off of the back of the trailer onto some uneven floor, ee rolled ankle. Contractor employee is placed on restricted duty.

Struck against a moving object

Employee was sitting in the cab of Company #443 haul truck a 785 Caterpillar haul truck, being loaded with dirt. An unseen rock in the bucket struck the truck bed jolting the employee.

Employee was removing wires from a communication box, the box was sitting on a table. The employee used a razor knife to cut a couple of wires, the knife slipped and cut employees wrist area. The wound required stitches and employee was placed on restricted duty.
