Accident type, without injuries

Roof fall 70 feet inby crosscut #13, #1 entry, 9th Right Section. The fall was approximately 16 feet wide, 17 feet long and 8 feet high.

Roof fall at crosscut #1, 7th Right Sub-Mains, entry #5. Approximate dimensions: 18 feet wide, 19 feet long and 12 feet high.

Roof Fall at crosscut #18, Main West section, #3 entry. Approximate dimensions: 20 feet wide, 30 feet long, and 10 feet high.

A roof fall occurred in the 10th Right Section, #2 Entry, #7 crosscut, in an intersection. The fall measured approximately 17 feet wide by 16 feet long and approximately 8 feet high. The roof fall was reported to the MSHA Hotline.

A roof fall occurred on the longwall recovery face in 8th Right section crosscut #10, the fall was over anchorage. Dimensions of fall are approximately 27'long x 5'wide and height is unknown. The fall was on the LW recovery face at shields 71 thru 75. The contained material fell from the face and leaned up against the Pettito Mule.

Roof Fall in the 9th Right Section. Crosscut #13, #3 entry. Roof fall was approximately 18 feet wide, 17 feet in length and approximately 8 feet high.

Struck against a moving object

Employee was building drill bench, pushed thru the berm at the edge of the highwall and traveled over the highwall.

Fall from scaffolds, walkways, platforms

Employee fell backwards from an elevated platform and hit his head on the ground causing a concussion and laceration to his head.

Caught in, under or between a moving and a stationary object

Employee was on the longwall face where the shearer was cleaning up the tailgate area. As the shearer was leaving tailgate area towards the headgate a 4"" line was struck by shearer and the 4"" line pinched employee against a shield.

Struck by falling object

Two contractors were replacing screw jacks, in the process of handling one of the jacks, it slipped from one contractor's grasp and hit him on the foot. He has been placed on restricted duty.

Contractor was helping in the installation of water line. He was asked to get components for the water line, contractor turned around and slipped and then felt pain in his back.

Over-exertion in pulling or pushing objects

Employee was opening a man door in the shop wash bay. The door was jammed so the employee pulled hard on the door and felt a sharp pain in his left shoulder.

Employee was exiting a ladder and over extended his left leg on the last step and felt pain in his lower back.

2 employees were dragging a pump, controller and power cable along the offside of the belt, Employee tripped and tried to catch himself. Within doing so he hurt his lower back.

Contact with hot objects or substances

He stopped #504GR to tighten the radiator cap. when he tried to tighten the cap it blew off and the fluid hit him in the face.

Over-exertion in pulling or pushing objects

Employee was changing wheel unit on shuttle car, he was using an Allen wrench to remove bolts. In removing the bolts he felt pain in his shoulder and the pain increased as he removed the next bolt after Allen wrench slipped. This has been a chronic situation with employee, this is being deemed an illness.

Employee was mounting company #103 dragline and felt a pop in his left shoulder

Contact with hot objects or substances

Contract employee was using a cutting torch on the Main West four head roller when a hot piece of slag went into the employee's mouth. Employee was was restricted to light duty for one shift.

Struck against a moving object

Employee was operating a tow vehicle, as the vehicle was traveling it hit a hole in the roadway causing the employee to hit his hit on the cab of vehicle. Employee was placed on restricted duty on 10/22/2013.

Contractor was lifting a Kennedy mandoor onto a trailer, the mandoor slipped while in his grasp and the edge of the door cut his hand. His laceration required stitches to close and was placed on restricted duty.

Contractor was roof bolting, his top drill steel became stuck in the roof mesh, while bringing steel down, the wrench came out of the chuck, he reached to put wrench back in place and the drill steel struck his fingers. He was placed on restricted duty.

Caught in, under or between a moving and a stationary object

Employee was helping in the setting of screw jacks in the beltline entry. The tube portion of the jack was set up right and they were placing the screw portion onto the tube portion. Employee's thumb was caught between the two pieces. He has been placed on restricted duty.

Struck by falling object

Employee was helping with miner cable, he had just pulled the cable into a crosscut. Employee had pulled on mesh and a piece of rib struck the employee. Employee was placed on restricted duty.

Over-exertion (Not Elsewhere Classified)

Employee was walking in the 11th Right Section, while walking in muddy conditions, he lifted his foot and felt pain in his back and leg. Further medical examination on 7/24/2013 placed the employee on restricted duty resulting in late reporting of this injury.

Struck by falling object

Employee was removing a grate over a sump in order to clean the sump. Employee used a 1/4 inch round stock hook approx. 3 feet long with a 1 inch 90 degree bend on the end of the hook. The grate slipped off the hook and struck the employees right foot just behind the steel toe of his boot resulting in a fracture to his Medial Metatarsal of his right foot.

Employee was walking down the beltline entry, he slipped and reached out to catch himself. He doing so he grabbed beltline components and hurt his shoulder. He has been placed on restricted duty.

While carrying a 50 pound bag of rock dust, employee slipped on his right foot causing employee to hurt his back. He has been placed on restricted duty.

Fall to the walkway or working surface

Employee tripped while stepping over dragline power cable and fell on his left hand and left ribs. The employee was seen by the on shift ERT. The employee did not go to the hospital. On 01/10/2014 the employee seen his doctor and it was determined that the employee received an Acute, non-displaced, fracture of his left 8th 9th 10th ribs.

Employee was removing a discharge hose from a water pump, the hose end flew up and struck employee in the face. The laceration required stitches to close.

Struck against stationary object

Was walking between 220 and scoop which had roof mats loaded on it. He was watching his footing and never seen the roof mats. He ran into them, cutting his face.

Employee was removing a ""T"" off of the water line, after the ""T"" was removed the water line came up and struck the employee in the face. The employee received a laceration to the face and required stitches to close the laceration.
